Stage-specific polysomnographic and MRI markers across phenoconversion in isolated REM sleep behavior disorder.

OBJECTIVES: To identify stage-specific neurophysiological and structural markers across non-converted isolated rapid eye movement (REM) sleep behavior disorder (iRBD), phenoconverted iRBD, and established α-synucleinopathy. METHODS: We retrospectively analyzed 86 patients who underwent video-polysomnography (PSG) and magnetic resonance imaging (MRI), classified at baseline as iRBD or established α-synucleinopathy, with longitudinal follow-up to define non-converted (n = 27), phenoconverted iRBD (n = 25), alongside established α-synucleinopathy (n = 34). Pairwise comparisons assessed stage-spe

Rapid Eye Movement (REM) Sleep Behaviour Disorder in Moebius Syndrome: A Rare Pediatric Case.

Moebius syndrome is a rare, non-progressive congenital disorder, most commonly characterized by impairment of the abducens (VI) and facial (VII) cranial nerves, resulting in facial palsy and limited ocular abduction. A broad spectrum of associated clinical manifestations includes craniofacial abnormalities, limb malformations, and neuromotor developmental delay. Sleep disorders are frequently reported in these patients, most often related to sleep-disordered breathing. In contrast, rapid eye movement (REM) sleep behavior disorder (RBD) is exceptionally rare, both in pediatric patients and in a

Prodromal Lewy Body Disorder Features in REM Sleep Behavior Disorder With Biomarker-Defined Synucleinopathy.

OBJECTIVE: Isolated rapid eye movement sleep behavior disorder (iRBD) is a prodromal state for Lewy body disorders and exhibits biological heterogeneity that may influence clinical expression and progression. We examined clinical features in individuals with iRBD and biomarker-defined synucleinopathy. METHODS: Parkinson's Progression Markers Initiative (PPMI) is a longitudinal, multi-center observational study. Participants included polysomnogram (PSG)-confirmed iRBD individuals who were cerebrospinal fluid (CSF) α-synuclein seed amplification assay positive with no clinical diagnosis of Park

Machine learning to diagnose, classify and predict phenoconversion in isolated REM sleep behavior disorder.

Rapid eye movement (REM) sleep behaviour disorder (RBD), particularly its idiopathic/isolated form (iRBD), is a prodromal marker for α-synucleinopathies, including Parkinson's disease, dementia with Lewy bodies and multiple system atrophy. Machine learning (ML) offers opportunities to improve diagnosis and risk stratification in this high-risk group. We conducted a systematic review of PubMed, Embase (Ovid) and Medline (Ovid) from 2014 to September 2025, following PRISMA guidelines. From 335 records identified, 202 remained after duplicate removal and 75 studies on adult humans with clinically

Orexin, Sleep, and Cognition in Alzheimer Disease: Non-REM Oscillatory Activity and Neural Resilience.

BACKGROUND AND OBJECTIVES: Sleep-wake dysregulation and elevated CSF orexin have been implicated in Alzheimer disease (AD). Sleep spindles (SPs) and slow oscillations (SOs) are linked to cognition and neurodegeneration; however, their relationship with CSF orexin concentrations in symptomatic AD has not been characterized. We investigated whether nonrapid eye movement (NREM) SP-SO activity is associated with CSF orexin and whether these oscillatory features moderate associations between orexin, cognition, neuropsychiatric symptom severity, and AD biomarkers. METHODS: This prospective observat

REM-related obstructive sleep apnoea in neuromuscular diseases: A 10-year retrospective cohort study.

PURPOSE: Neuromuscular diseases (NMDs) are reported to have increased obstructive apnoea/hypopnoea during rapid eye movement (REM) sleep; however, the prevalence of REM-related obstructive sleep apnoea (OSA) and its clinical correlates in NMD remain undefined. We evaluated REM sleep, REM-related OSA frequency, and associated with demographic and clinical factors. METHODS: We retrospectively evaluated patients with NMDs who underwent polysomnography (PSG) between 2015 and 2024. Demographic data, morning arterial blood gases, spirometry and PSG data were collected. REM-related OSA required AHI …

Motor events during REM sleep are associated with occipital lobe activity in Parkinson's disease.

STUDY OBJECTIVES: Abnormal motor events during REM sleep are common in Parkinson's disease (PD), but few studies have evaluated motor events comprehensively. This study aimed to categorize different types of motor events and explore their relationships with clinical symptoms, sleep structure and cortical electrophysiological characteristics. METHODS: 116 PD patients (49 women and 67 men) underwent a clinical assessment and video-polysomnography. REM sleep-related motor events were classified into elementary motor events and complex motor events. Patients were categorized into three groups: PD

REM Sleep Behavior Disorder Diagnostic Code Accuracy and Implications in the Real-World Setting.

BACKGROUND AND OBJECTIVES: Isolated REM sleep behavior disorder (iRBD) carries increased risk of neurodegenerative parkinsonian disorder or dementia (NPD) but is difficult to accurately screen for in the community. Health care data offer the opportunity to identify large numbers of iRBD cases among outpatients. We aimed to determine the positive predictive value (PPV) of an RBD International Classification of Disorders (ICD) code for actual iRBD based on manual review of the electronic health record (EHR), examine risk of NPD diagnosis, and explore whether a statistical model developed using s

Symmetric and Profound Monoaminergic Degeneration in Parkinson's Disease with Premotor REM Sleep Behavior Disorder.

BACKGROUND: Rapid eye movement sleep behavior disorder (RBD) may precede or follow motor symptoms in Parkinson's disease (PD). While over 70% of idiopathic RBD cases phenoconvert within a decade, a small subset develops PD after a more extended period or remains nonconverted. These heterogeneous manifestations of RBD in PD prompt subtype investigations. Premotor RBD may signify "body-first" PD with bottom-up, symmetric synucleinopathy propagation. OBJECTIVE: Explore brainstem and nigrostriatal monoaminergic degeneration pattern differences based on premotor RBD presence and duration in de nov

The paradoxical impact of periodic limb movements on isolated REM sleep behavior disorder patients.

Recently, the α-synuclein origin and connectome model described two types of Parkinson's disease: "brain-first" and "body-first" subtypes. We aimed to investigate the role of periodic limb movements during sleep (PLMS) in identifying these subtypes starting from a prodromal stage of α-synucleinopathies. 191 patients with isolated REM sleep behavior disorder (iRBD) underwent video-polysomnography (vPSG), questionnaires, clinical interview, and neuropsychological battery. Patients who presented PLMS index (PLMSi) > 15 were compared with patients presenting PLMSi ≤ 15 on clinical questionnaires,

Deciphering the Link: Correlating REM Sleep Patterns with Depressive Symptoms via Consumer Wearable Technology.

This study investigates the correlation between REM sleep patterns, as measured by the Apple Watch, and depressive symptoms in an undiagnosed population. Employing the Apple Watch for data collection, REM sleep duration and frequency were monitored over a specified period. Concurrently, participants' depressive symptoms were evaluated using standardized questionnaires. The analysis, primarily using Spearman's correlation, revealed noteworthy findings. A significant correlation was observed between an increased REM sleep proportion and higher depressive symptom scores, with a correlation coeffi

Isolated REM sleep behavior disorder is associated with altered 24-h rest-activity measures.

INTRODUCTION: Isolated REM sleep behavior disorder (iRBD), the loss of motor inhibition during REM sleep, is a symptom of prodromal Lewy body disease, with over 80% of iRBD patients progressing to Parkinson's disease or dementia with Lewy bodies. Disruption of rest-activity patterns, an established predictor of Parkinson's disease, has not been well characterized in patients with iRBD. Here, we tested the hypothesis that accelerometer-based measures of 24-hour activity would indicate greater fragmentation and variability in patients with iRBD relative to matched healthy controls. Comprehensive Analysis of Brain Volume in REM Sleep Behavior Disorder with Mild Cognitive Impairment.

BACKGROUND: Rapid-eye-movement sleep behavior disorder (RBD) is a major risk factor for Parkinson's disease and dementia with Lewy bodies. More than a third of RBD patients have mild cognitive impairment (MCI), but their specific structural brain alterations remain poorly understood. OBJECTIVE: This study aimed to investigate the local deformation and volume of gray and white matter tissue underlying MCI in RBD. METHODS: Fifty-two idiopathic RBD patients, including 17 with MCI (33%), underwent polysomnography, neuropsychological, neurological, and magnetic resonance imaging assessments. MCI

Alpha-synuclein seeds in olfactory mucosa of patients with isolated REM sleep behaviour disorder.

Isolated REM sleep behaviour disorder (RBD) is an early-stage α-synucleinopathy in most, if not all, affected subjects. Detection of pathological α-synuclein in peripheral tissues of patients with isolated RBD may identify those progressing to Parkinson's disease, dementia with Lewy bodies or multiple system atrophy, with the ultimate goal of testing preventive therapies. Real-time quaking-induced conversion (RT-QuIC) provided evidence of α-synuclein seeding activity in CSF and olfactory mucosa of patients with α-synucleinopathies. The aim of this study was to explore RT-QuIC detection of α-sy

Mild cognitive impairment and abnormal brain metabolic expression in idiopathic REM sleep behavior disorder.

BACKGROUND: Mild cognitive impairment (MCI) is a common feature of isolated rapid-eye-movement sleep behavior disorder (iRBD). Here, we assessed cognitive functions and MCI in a prospective iRBD cohort and investigated their association with disease-specific brain metabolic patterns. METHODS: Forty-four patients with polysomnography-confirmed iRBD performed a standardized battery of neuropsychological examinations every two years. We used previously established spatial covariance patterns from de novo drug-naïve Parkinson's disease with concomitant RBD (denovoPDRBD-RP) and iRBD (iRBD-RP) usin
