Uterine sarcomas.

Uterine sarcomas account for approximately 3%-7% of all uterine cancers. Since carcinosarcomas are currently classified as metaplastic carcinomas, leiomyosarcomas remain the most common subtype. Exclusion of several histologic variants of leiomyoma, as well as atypical smooth muscle tumors (so-called "smooth muscle tumors of uncertain malignant potential"), has highlighted that the vast majority of leiomyosarcomas are high-grade tumors associated with poor prognosis even when apparently confined to the uterus. Low-grade endometrial stromal sarcomas are indolent tumors associated with long-term…

Leiomyoma cutis: a focused review on presentation, management, and association with malignancy.

Cutaneous leiomyomas (CLs) are rare, sporadic, or inherited tumors of smooth muscle origin associated with various disorders. Hereditary leiomyomatosis and renal cell cancer (HLRCC) is the primary tumor predisposition syndrome associated with inherited CLs, affecting 180 families worldwide, with significant mortality. CLs are subdivided into piloleiomyomas, genital leiomyomas, and angioleiomyomas based on their smooth muscle of origin, as well as their clinicopathologic features. Piloleiomyomas, derived from arrector pili muscle, are solitary or multiple firm papulonodules located typically on

Practical issues in uterine pathology from banal to bewildering: the remarkable spectrum of smooth muscle neoplasia.

Among mesenchymal tumors of the uterus, smooth muscle neoplasms are most common. The wide morphologic spectrum, especially within the catego … imited role in this differential diagnosis. Endometrial stromal tumors are by far, less common than smooth muscle tumors, but can be confused with leiomyosarcomas if they are associated with an undifferentiated uterine sarcoma and the low-grade component is overlooked or they have a myxoid/fibroblastic morphology. The differential diagnosis may be confounded if the l …

Renal cell carcinoma with angioleiomyoma-like stroma: clinicopathological, immunohistochemical, and molecular features supporting classification as a distinct entity.

Rare renal epithelial neoplasms have been recognized to have an angioleiomyoma or leiomyoma-like proliferation of stromal smooth muscle; however, the nature of these tumors and their relationships to other renal cell carcinomas are poorly understood. We analyzed 23 such tumors for their clinicopathological, immunohistochemical, and cytogenetic features using fluorescence in situ hybridization. Twelve showed a homogeneous combination of features and were reclassified as renal cell carcinoma with angioleiomyoma-like stroma. These were composed of neoplastic glandular structures lined by cells wi

Leiomyosarcoma of the Bone: A Case Report on the Critical Role of Biomarkers in Differential Diagnosis.

Leiomyosarcoma of the bone (LMSB) is a rare type of sarcoma characterized by smooth muscle differentiation arising within the bone. We present the case of a 38-year-old female patient diagnosed with LMSB in the left distal femur. Histological examination revealed spindle-shaped and pleomorphic cells, with no evidence of cartilage or bone matrix formation. Immunohistochemical analysis demonstrated the tumor cell were positive for Smooth Muscle Actin, Desmin, and H-caldesmon. The integration of clinical presentation, imaging, morphological, and immunohistochemical findings confirmed the diagnosi

Monotypic angiomyolipoma of the nasal cavity: An extremely rare cause of nasal mass with recurrent epistaxis.

Monotypic angiomyolipoma is usually found in the kidneys and is composed predominantly of epithelioid cells which show positivity for melanocyte and smooth muscle markers. It can pose a diagnostic challenge due to a range of differential diagnosis. We report the second case of monotypic angiomyolipoma of nasal cavity and first from India in a 54-year-old male who presented with a nasal polyp. Grossly the tumor was well circumscribed and un-encapsulated. Microscopy showed a large number of epithelioid cells mixed with a few spindle cells, varying sized blood vessels, and focal areas of adipose

Differentiating a large abdominal cystic lymphangioma from multicystic mesothelioma: report of a case.

We report a case of retroperitoneal cystic lymphangioma in a 30-year-old woman who presented with abdominal distention and pain. Imaging studies revealed a large, thin-walled multicystic mass occupying the whole abdomen. Based on a preoperative diagnosis of multicystic mesothelioma, we performed laparotomy, which revealed a tumor arising from the gastropancreatic ligament in the posterior wall of the omental bursa. We resected the tumor completely, without the adjacent viscera. The final pathological diagnosis was cystic lymphangioma, based on the immunohistochemical findings of positive CD31

Immunoexpression of p53 in cutaneous and subcutaneous leiomyosarcomas.

The diagnosis of malignancy in cutaneous and subcutaneous smooth muscle tumors is based on subtle criteria. Therefore, any ancillary technique useful in this differential diagnosis is always welcome. In this report, we study the immunoexpression of p53 in 19 malignant smooth muscle tumors of the skin (15 cutaneous leiomyosarcomas, 2 subcutaneous leiomyosarcomas, and 2 cutaneous metastases of leiomyosarcoma), as well as in 1 leiomyoma with cellular atypia, therefore complementing a previous study on p53 immunoexpression in leiomyomas of the skin. The p53 staining was positive in 12 (63.16%) of …

Tentorial Angioleiomyoma: A Rare Neurosurgical Entity. Case Report and Review of the Literature.

BACKGROUND: Angioleiomyoma (ALM) is a soft tissue neoplasm rarely described in the intracranial site. Because of their uncommon presentation, atypical neuroradiologic and pathologic features, ALMs are often misdiagnosed. CASE DESCRIPTION: We describe the neuroradiologic, clinical, and pathologic data of a 37-year-old male patient suffering from a tentorial ALM. He was admitted at our hospital because of a posterior cranial fossa mass. Magnetic resonance imaging (MRI) showed a left tentorial tumor, hypointense on T1-weighted sequences, with heterogeneous contrast enhancement after gadolinium i

What is your diagnosis? Gastric leiomyoma.

Smooth muscle tumors are the second most common benign tumors of the stomach after adenomatous polyps. Most often they present as solitary submucosal lesion. Occassionally they may be multiple. They usually present as a smooth mass, although they may be pedunculated and rarely present as gastroduodenal intusuception, when they are located in the distal antrum and prolapse into the duodenum. Leiomyomas may be submucosal, intramural or subserosal. Because of this peculiarity they may grow predominantly intraluminal as well as extraluminally.

Unusual solid submucosal angioleiomyoma of the upper lip: A case report and review of the literature.

RATIONALE: Angioleiomyoma (ALM) is a rare benign tumor originating from smooth muscle tissue, and its occurrence in the oral cavity is extremely uncommon. Although a few cases involving the upper lip have been reported, most lesions are exophytic or superficially located. Here, we report an unusual solid, submucosal ALM in the upper lip, highlighting its distinctive clinical, imaging, and histopathological features. PATIENT CONCERNS: A 43-year-old male presented with a firm, painless swelling in the upper lip that had gradually enlarged over the past 8 months. The lesion measured 4.0 × 2.0 cm

ERα inhibited myocardin-induced differentiation in uterine fibroids.

Uterine fibroids, also known as uterine leiomyomas, are a benign tumor of the human uterus and the commonest estrogen-dependent benign tumor found in women. Myocardin is an important transcriptional regulator in smooth and cardiac muscle development. The role of myocardin and its relationship with ERα in uterine fibroids have barely been addressed. We noticed that the expression of myocardin was markedly reduced in human uterine fibroid tissue compared with corresponding normal or adjacent myometrium tissue. Here we reported that myocardin induced the transcription and expression of differenti

Incidental Splenic Leiomyoma in a Child Uncovered During Workup for Eosinophilic Gastroenteritis.

Eosinophilic gastroenteritis (EGE) is a rare, heterogeneous inflammatory disorder characterized by eosinophilic infiltration of the gastrointestinal tract in the absence of secondary causes. Clinical presentation is variable and often mimics other gastrointestinal conditions, making diagnosis challenging. Splenic leiomyoma, in contrast, is a rare benign smooth muscle tumor, typically reported in immunocompromised patients. Pediatric cases are exceedingly uncommon. We describe a 9-year-old boy presenting with persistent diarrhea and peripheral blood eosinophilia. Endoscopy showed esophageal fur…

Pulmonary benign metastasizing leiomyoma: a case report and literature review.

Benign metastasizing leiomyoma (BML) is a rare condition that occurs in all age groups and that is particularly prevalent among women of late childbearing age. All patients have a history of uterine leiomyoma and/or myomectomy, often associated with distant metastases from the uterus, which commonly occurs in the lung. We report the case of a 32-year-old young woman suffering from chest stuffiness, labored respiration and weakness after a myomectomy performed one month earlier. The chest CT showed a diffuse miliary shadow in both sides of her lungs, but serum tumor markers such as CA125, CA199

Epithelioid leiomyosarcoma in the visceral peritoneum of an American badger (Taxidea taxus).

A 12-year-old female American badger was presented to the Taipei city zoo veterinary ward with anorexia and weakness. Treatments were ineffective, and the badger died of chronic interstitial nephritis and uremia. At necropsy, numerous firm white nodules, measuring 0.5-2.0 cm, were present on the surface of the liver, stomach, spleen, small intestine, pancreas, and diaphragm. Most nodules were encapsulated and well demarcated from the organs to which they were attached. A poorly demarcated mass, measuring 0.5 cm in diameter, had invaded the hepatic parenchyma and appeared to be the origin of al

Spindle cell pseudotumor due to Mycobacterium avium-intracellulare in patients with acquired immunodeficiency syndrome (AIDS). Positive staining of mycobacteria for cytoskeleton filaments.

A rare spindle-cell pseudotumor caused by Mycobacterium avium-intracellulare (MAI) that mimics a mesenchymal tumor, was recently reported (7,14). We report on three such pseudotumors in patients with the acquired immunodeficiency syndrome (AIDS), two involving lymph nodes and one involving the bone marrow. In the course of investigating the first-encountered example of this tumor for evidence of smooth-muscle origin of the spindle cells, it was noted that these cells stained positively for desmin by immunoperoxidase techniques (IPX), as did a variety of other cytoskeleton filaments of all size
