Viable group A streptococci in macrophages during acute soft tissue infection.

BACKGROUND: Group A streptococcal severe soft tissue infections, such as necrotizing fasciitis, are rapidly progressive infections associated with high mortality. Group A streptococcus is typically considered an extracellular pathogen, but has been shown to reside intracellularly in host cells. METHODS AND FINDINGS: We characterized in vivo interactions between group A streptococci (GAS) and cells involved in innate immune responses, using human biopsies (n = 70) collected from 17 patients with soft tissue infections. Immunostaining and in situ image analysis revealed high amounts of bacteria…

[60 year old patient with soft tissue infection of the right leg].

Group A streptococcal necrotizing fasciitis is a rare disease associated with high mortality. Since severe toxic shock syndrome is a common complication, only immediate and aggressive surgical intervention, adequate antimicrobial therapy and supportive intensive care can be life-saving. We report about successful treatment of a 60-year old patient with necrotizing fasciitis and multiple organ failure.

Muscle flap salvage of spine wounds with soft tissue defects or infection.

STUDY DESIGN: This retrospective study was designed to analyze the results of 22 patients treated for postoperative soft tissue defects of the spine. OBJECTIVE: To demonstrate the utility of flaps in the salvage of spine wounds. SUMMARY OF BACKGROUND DATA: In the literature, the treatment of postoperative spine infections is with serial débridement, antibiotic irrigation catheters, drains, and occasional removal of spinal implants. Muscle flaps have received scant mention in the surgical literature for spine coverage. METHODS: Group 1 (n = 15) had postoperative wound infections or dehiscenc

Soft tissue infection with Absidia corymbifera in a patient with idiopathic aplastic anemia.

We describe a case of primary cutaneous mucormycosis (zygomycosis) in a patient with idiopathic aplastic anemia which responded to surgical debridement and therapy with liposomal amphotericin B. The tissue removed at surgery showed dense infiltration with fungal hyphae on histopathological examination. Primary cultures of tissue on solid media were negative, but Absidia corymbifera was isolated from unprocessed tissue placed in brain heart infusion broth.

Oral soft-tissue manifestations as presenting symptom/sign of HIV infection.

This prospective study reports oral manifestations noted in 17 patients as the first indications of HIV infection, and serves to remind health workers of the need to examine the oral soft tissues thoroughly. This study also demonstrated that other HIV-associated oral lesions are often seen in addition to the presenting symptom or sign, which was highly variable and not lesion-specific. Appropriate management of the patient after recognition of the significance of these signpost oral lesions could affect quality of life, influence the course of the disease and reduce the risk of transmission. The Enigmatic Presentation of Pneumorrhachis in Necrotizing Soft Tissue Infection: A Case Report.

Pneumorrhachis, the presence of air within the spinal canal, is an uncommon radiologic finding, most often resulting from trauma or iatrogenic procedures, or exceedingly rarely, infectious causes. We present a case of necrotizing soft tissue infection overlying the sacrum, not associated with a preexisting pressure ulcer/wound, leading to pneumorrhachis, pan myelomeningitis, and epidural abscess formation. Initial diagnosis was assumed to be a buttock abscess, but additional symptoms of severe weakness and lethargy elicited further studies. Prompt diagnosis was achieved with computed tomograph

The Role of Hyperbaric Oxygen Therapy in Management of Necrotizing Soft Tissue Infection.

Necrotizing soft tissue infection (NSTI) is a life-threatening, high morbidity pathology that requires aggressive, multidisciplinary management. Surgery and antibiotic administration are core components of treatment. Adjunctive incorporation of hyperbaric oxygen therapy (HBOT) can further enhance treatment and recovery. Benefit is achieved through multiple effects brought about by increase of local and systemic oxygen tension. Direct effects include bacteriostasis, disruption of bacterial toxin production, and attenuation of inflammation. Indirect benefits include demarcation of viable tissue

Treatment and clinical outcomes in lower extremity necrotizing soft tissue infection.

BACKGROUND: Necrotizing soft tissue infection (NSTI) is a rapidly progressing infection of the soft tissues under the skin, which poses a life-threatening risk if not treated promptly. Due to the non-specific nature of symptoms at presentation, the diagnosis is often delayed, leading to a high mortality rate and increased risk of amputation. Early treatment requires both surgical intervention and antibiotic therapy. This study aims to investigate the effects of clinical parameters, applied treatments, and initial interventions on mortality and amputation rates in patients with NSTI. MATERIALS

Primary Cutaneous Mucormycosis: A Necrotising Soft Tissue Infection with Poor Prognosis.

BACKGROUND: Cutaneous mucormycosis is an unusual fungal infection that continues to occur. It needs aggressive surgical debridement and timely administration of antifungals due to its high fatality rate. High clinical suspicion on the part of a surgeon is required to prevent the same. CASE PRESENTATION: We present two cases of cutaneous mucormycosis in which the patients succumbed to death, highlighting the seriousness of the condition. One patient had a lower leg ulcer and was diabetic, and the other patient had a gluteal abscess following an intramuscular injection. Tissue samples grew Rhiz

Type 2 Necrotizing Soft Tissue Infection: A Disastrous Diagnosis Not To Be Missed.

Necrotizing soft tissue infections (NSTIs) are severe, life-threatening forms of infection. Tissues from the epidermis to the deep musculature may be affected. This includes necrotizing forms of cellulitis, myositis, and fasciitis. Delayed diagnosis can lead to widespread tissue loss, limb loss, and mortality, representing a group of infectious surgical emergencies requiring time-sensitive aggressive debridement. This article presents a unique case with a particularly ambiguous and vague presentation of type 2, group A streptococcal NSTI in an intravenous drug abuser. This rapidly spreading in…

A Case of Necrotizing Soft Tissue Infection Secondary to Perforated Colon Cancer.

Necrotizing soft tissue infections are aggressive infections that cause necrosis of muscle, fascia, and tissue. They typically follow fascial planes that lack insufficient blood supply. Early drainage and debridement are essential for survival in these patients. This is a case of a patient who presented in diabetic ketoacidosis with a necrotizing soft tissue infection localized to the left flank and abdomen with underlying colon cancer pathology. The patient was a 54-year-old female who initially presented with acute dyspnea and left flank pain for two weeks. On admission, she was afebrile, ta…

Role of Pseudomonas aeruginosa Glutathione Biosynthesis in Lung and Soft Tissue Infection.

The opportunistic pathogen Pseudomonas aeruginosa is a leading cause of morbidity and mortality worldwide. To survive in both the environment and the host, P. aeruginosa must cope with redox stress. In P. aeruginosa, a primary mechanism for protection from redox stress is the antioxidant glutathione (GSH). GSH is a low-molecular-weight thiol-containing tripeptide (l-γ-glutamyl-l-cysteinyl-glycine) that can function as a reversible reducing agent. GSH plays an important role in P. aeruginosa physiology and is known to modulate several cellular and social processes that are likely important duri

Necrotizing Soft Tissue Infection of the Breast during COVID-19 Pandemic.

Necrotizing soft tissue infection has been historically recognized as a severe, rapidly spreading soft tissue infection associated with a very high risk of mortality. Cases of primary necrotizing fasciitis of the breast are rarely described but often fatal. We present a case of necrotizing soft tissue infection of the right breast extending to the anterior abdominal wall in a 39-year-old obese female, with a history of tobacco use. The patient presented 10 days after symptom onset due to concerns and anxiety related to COVID-19 exposure. This delay allowed for further extension and smoldering
