Anatomic variation in the pterygopalatine angle of the maxillary sinus and the maxillary bulla.

Bulges of the most posterior ethmoid air cells into the maxillary sinus were termed maxillary bullæ by Onodi. With few exceptions, they have since been ignored by anatomists through time. Likewise, Sieur cells-the spheno-ethmoido-maxillary air cells-are uncommonly found in anatomical texts. We therefore aimed to perform a retrospective cone beam computed tomography study on 50 patients to document the possibilities of anatomic variation in the situs of the orbital process of palatine bone-a variation related anatomically with the pterygopalatine fossa (PPF) and the respective angle of the maxi

[The giant fungal body in the sphenoidal sinus and the destruction of the skull base].

The objective of the present study was to estimate the efficacy of the optical systems with the variable visual field angle applied for the endoscopic interventions on the paranasal sinuses. The authors report a clinical observation of the patient presenting with the giant fungal body in the sphenoidal sinus responsible for the partial destruction of the bone canal of the optic nerve and the internal carotid artery. The patient was treated by endoscopic shenoidotomy through the paraseptal approach with the use of a sinuscope with the variable visual field angle. It was shown that the use of op

Sphenoid wing meningiomas with osseous involvement.

BACKGROUND: Sphenoid wing meningiomas (SWMs) with osseous involvement are neurosurgically challenging because of their position within the skull base and their high rates of recurrence. Sufficient resection of these meningiomas requires extensive surgical exposure. We report on 82 patients with meningiomas infiltrating the sphenoid wing, analyzing radiological appearance and its influence on surgical management. METHODS: Charts of the patients including surgical records, clinical reports, histological examinations, imaging studies, and meticulously focusing questionnaires were reviewed. RESU

Computerised tomographic detection of intracranial complications of paranasal sinus infections.

Ninety-four patients were referred for CT examination of the paranasal sinuses within a five year period. Only 11 (11.7%) of them had intracranial complications. These include cerebral, subdural and epidural abscesses, frontal bone osteomyelitis. The maxillary and ethmoidal sinuses were mostly involved and can be implicated as the sinogenic causes of intracranial infections. Sphenoidal sinus was not involved in any of the patients.

Pneumoceles of the paranasal sinuses (a case report).

We report a case of an infection of a large sphenoidal sinus communicating with a hyperaerated mastoid. Endoscopic drainage of this large cavity was successful. We suggest that EES should have a place in the treatment of the sinusal pneumocele. Surgery should however be reserved for cosmetic purposes, infection, algetic symptoms and ocular problems. A cosmetic correction with osteotomy and bone resection can be associated with the endoscopic surgery.

[Atypical osteomyelitis of the base of the skull and malignant otitis externa].

Osteomyelitis of the skull base is a rare, but serious disease whose incidence is tending to increase. It affects the marrow of the temporal, occipital and sphenoid bones and is generally secondary to Pseudomonas aeruginosa infection of the external auditory meatus. Based on a clinical case, the authors recall the difficulty of establishing the positive diagnosis and the possible confusion with neoplastic disease, hence the fundamental role of biopsies. They also review the literature on this subject.

Orbital Lymphoma Presenting As Recurrent Orbital Cellulitis: A Diagnostic Challenge.

Orbital cellulitis is an infection of the orbital tissue behind the orbital septum. We report a case of a 71-year-old Malay lady, a passive smoker for 20 years, presenting with recurrent orbital cellulitis at the same site. She initially presented with right periorbital swelling, redness, and reduced vision worsening over two weeks, along with a firm swelling over the right temple. CT of the brain and orbit revealed a homogenous mass extending from the right sphenoid bone to the right orbit. The initial diagnosis was right sphenoid meningioma or metastasis. Her symptoms improved after 10 days …

The CT and MRI observations of small cell neuroendocrine carcinoma in paranasal sinuses.

BACKGROUND: Primary small cell neuroendocrine carcinoma (SNEC) of paranasal sinuses is an extremely rare malignant tumor known for its aggressive clinical behavior. METHODS: Nineteen patients with SNEC in paranasal sinuses by magnetic resonance imaging (MRI) (n = 19) and computerized tomography (CT) and MRI (n = 18) were retrospectively studied. CT and MRI were undertaken to investigate tumor features. RESULTS: The lesions were located in the ethmoidal sinus (n = 6), maxillary sinus (n = 4), and bilateral sphenoid sinus (n = 9). All lesions showed a symmetry or 'pigeon' pattern in the bilate

[Giant chondroma of anterior cranial fossa].

A rare case of the giant chondroma is presented. The patient, 29, carried it over the last 24 years. At his admittance to the neurosurgery department the tumor attained huge dimensions and occupied the basement of the anterior cranial fossa on the left side penetrating into the orbital plates, frontal sinus and ethmoidal labyrinth. Surgery was performed yielding a good result.

Long-term outcomes after surgical correction of anterior plagiocephaly secondary to isolated frontosphenoidal synostosis: a systematic review and two illustrative cases.

PURPOSE: Anterior plagiocephaly ( AP), secondary to isolated frontosphenoidal synostosis (IFS), represents one of the rarest forms of craniosynostosis documented in medical literature. Therefore, there is a lack of comprehensive understanding regarding the long-term (> 2 years) postoperative outcomes of this minor suture synostosis. METHODS: This study presents the long-term outcomes of two patients previously treated for IFS, detailing their clinical, imaging, and postoperative characteristics. Additionally, a systematic review was conducted following PRISMA criteria to summarize existing li

Acute Enophthalmos After Lumbar Puncture in a Patient with Type 1 Neurofibromatosis Related Sphenoid Wing Dysplasia.

A 19-year-old woman with type 1 neurofibromatosis related sphenoid wing dysplasia underwent a lumbar puncture (LP) after an unexplained syncopal event. The cerebrospinal fluid work-up was unremarkable. However, 30-hours post LP, she developed marked enophthalmos ipsilateral to the calvarial dysplasia. The enophthalmos gradually resolved within 72 hours. We surmise that the patient's enophthalmos was due to an LP induced decrease of intracranial pressure in conjunction with the orbital bone anomaly.

Trigeminal neuralgia-like pain in an aged woman with fibrous dysplasia of the skull base.

A 72 year old woman with attacks of severe lancinating pain in the right frontotemporal region of her face had, on CT scan of the skull base, fibrous dysplasia of the right sphenoid bone, involving the areas traversed by the trigeminal nerve. Her pain ceased following treatment with methysergide. In patients with hemifacial pain but no evident facial or skull deformities, CT scan of the skull base may be helpful in elucidating the diagnosis.

Sphenoethmoidal mucoceles with intracranial extension.

Mucoceles of the paranasal sinuses are benign, expansile, locally destructive lesions that are thought to occur secondary to obstruction of the sinus ostia. Nasal and/or ocular signs and symptoms are the usual clinical manifestations of mucoceles arising in the ethmoid and sphenoid sinuses, and intracranial extension is rare. We describe three cases of widely destructive sphenoethmoidal mucoceles extending intracranially, with bone erosion and dural exposure in all cases and brain necrosis in one case.
