Endoscopic sphincterotomy: follow-up evaluation of effects on the sphincter of Oddi.

Endoscopic sphincterotomy (ES) alters the structure and motor function of the sphincter of Oddi (SO). The magnitude and duration of these changes, however, have not been critically examined. Before ES, 22 patients with common bile duct stones were evaluated by endoscopic retrograde cholangiography. The pressure gradient between the common bile duct and the duodenum, the SO basal pressure, and the SO peak phasic pressures were obtained. After ES, the electrosurgical incision length was determined using the extended papillotome and an inflated Fogarty balloon as reference. A high correlation exi…

Endoscopic retrograde sphincterotomy in the management of common bile duct stones.

Endoscopic retrograde sphincterotomy (ERS) was performed in 53 patients with choledocholithiasis, 26 of whom had an intact gallbladder. An adequate sphincterotomy was achieved in every patient, and all stones were removed in 90%. There was 9% morbidity but no mortality. ERS is a major advance in the management of common bile duct stones with significantly lower morbidity and mortality than those associated with conventional surgery. In particular, the role of ERS in pre-cholecystectomy patients is discussed.

[Oddi sphincterotomy by surgical retroperitoneal approach to the 2d part of the duodenum].

Retroperitoneal sphincterotomy using lombotomy is an original surgical approach for treatment of retained stones of the common bile duct. Indication for this technique can be found in patients for whom a transperitoneal approach is contre-indicated and when endoscopic sphincterotomy is impossible.

Inhibition of gallstone formation by sphincterotomy in the prairie dog: reversal by atropine.

Sphincterotomy has previously been shown to decrease gallbladder volume and gallbladder bile stasis while inhibiting gallstone formation in the prairie dog. We tested the possibility that atropine administration might reverse the effects of sphincterotomy on gallbladder mechanical function and gallbladder bile stasis and thereby reverse the inhibition of gallstone formation seen after sphincterotomy. Sixteen prairie dogs underwent sphincterotomy and were placed on a high cholesterol diet known to consistently induce gallstone formation. After 6 wk, in addition to the lithogenic diet, 8 animals…

Duodenoscopic sphincterotomy for removal of bile duct stones in patients with gallbladders.

Duodenoscopic sphincterotomy was attempted in 71 elderly patients with gallbladders who presented with acute symptoms caused by common bile duct stones. Sphincterotomy was possible in all but one patient, and duct clearance was achieved in 61 (86%); failures were usually due to the size of the stones. Two patients required blood transfusions for immediate bleeding, and two underwent cholecystectomy for acute cholecystitis developing within 7 days of sphincterotomy. One patient with a retained stone was judged to be unfit for surgery and died 6 weeks after sphincterotomy. Eleven patients had el…

A newly devised cutting probe for endoscopic sphincterotomy of the ampulla of vater.

A new diathermy cutting probe for endoscopic sphincterotomy has been devised. This consisted of a standard Teflon catheter with a diathermy wire and a metal-marked long tip. Using an Olympus duodenoscope and this cutting probe, endoscopic sphincterotomy has been successfully performed in 38 out of 40 patients with common bile duct stones without any serious complications. Advantages and clinical results of this instrument are presented.

Outcomes of precut sphincterotomy techniques in cases of difficult biliary access.

GOALS: Our study aims to define success and complication rates of precut sphincterotomy with the needle-knife and transpancreatic papillary septotomy (TPS) techniques as experienced at a single, high-volume endoscopy centre. BACKGROUND: Complication rates rise with increasing number of failed attempts at biliary cannulation; therefore, early precut sphincterotomy (PS) has been recommended. Selecting the ideal method for PS can be challenging and there is a paucity of data to help guide this decision. STUDY: We performed a retrospective analysis over 37 months of endoscopic retrograde cholang…

Transpancreatic Sphincterotomy Is Effective and Safe in Expert Hands on the Short Term.

In cases of difficult biliary cannulation, transpancreatic sphincterotomy (TPS) can be an alternative approach of biliary access. However, its success and safety profile have not been studied in detail. A systematic review and meta-analysis were performed to study the overall cannulation success and adverse events of TPS. These outcomes were also compared to other advanced cannulation methods. A systematic literature search was conducted to find all relevant articles containing data on TPS. Successful biliary cannulation and complications rates [post-ERCP pancreatitis (PEP), bleeding, and perf…

Need for pancreatic stenting after sphincterotomy in patients with difficult cannulation.

AIM: To investigate the need for pancreatic stenting after endoscopic sphincterotomy (EST) in patients with difficult biliary cannulation. METHODS: Between April 2008 and August 2013, 2136 patients underwent endoscopic retrograde cholangiopancreatography (ERCP)-related procedures. Among them, 55 patients with difficult biliary cannulation who underwent EST after bile duct cannulation using the pancreatic duct guidewire placement method (P-GW) were divided into two groups: a stent group (n = 24; pancreatic stent placed) and a no-stent group (n = 31; no pancreatic stenting). We retrospectively …

The effect of lateral internal sphincterotomy on resting anal sphincter pressures.

BACKGROUND/AIM: Anal fissures are one of the most common proctologic disorders. This study aimed to investigate alterations in anal sphincteric resting pressures after lateral internal sphincterotomy, which was performed for chronic anal fissure treatment. MATERIALS AND METHODS: Fifty-six (68.3%) male and 26 (31.7%) female patients were included in this study. Patients complicated with fistula-in-ano, abscess, cancer, inflammatory bowel disease, dermatitis, poor general condition, allergy to any of the standard medications, previous history of anal trauma, previous anal surgery, or diagnosis …

Easy sphincterotomy in patients with Billroth II gastrectomy: a new technique.

BACKGROUND/AIMS: ERCP and endoscopic sphincterotomy in patients with Billroth II gastrectomy are technically more difficult due to the reversed anatomy. We developed a new guidewire sphincterotome that includes two 15-mm non-isolated metal parts, one of which is located 12 cm from the distal tip and the other in the proximal end. The aim of this study was to evaluate the feasibility of and outcomes with the new sphincterotome for sphincterotomy in patients with Billroth II gastrectomy. METHODS: Between January 2004 and March 2007, 11 patients with Billroth II gastrectomy underwent endoscopic …
