The relationship between laminoplasty opening angle and increased sagittal canal diameter and the prediction of spinal canal expansion following double-door cervical laminoplasty.

PURPOSE: To clarify the relationship between laminoplasty opening angle (LOA) and the increase in sagittal canal diameter (SCD) in double-door cervical laminoplasty (DDCL) and to predict the increase in SCD using the resulting formula. METHODS: We analyzed 20 patients with multilevel cervical spondylotic myelopathy who underwent DDCL between September 2010 and January 2013. The pre- and post-operative parameters of the cervical spinal canal were measured by computed tomography. We deduced a formula describing the relationship between LOA and the increase in SCD and used it to predict the incr…

Dural sac area is a more sensitive parameter for evaluating lumbar spinal stenosis than spinal canal area: A retrospective study.

Narrowing of the dural sac cross-sectional area (DSCSA) and spinal canal cross-sectional area (SCCSA) have been considered major causes of lumbar central canal spinal stenosis (LCCSS). DSCSA and SCCSA were previously correlated with subjective walking distance before claudication occurs, aging, and disc degeneration. DSCSA and SCCSA have been ideal morphological parameters for evaluating LCCSS. However, the comparative value of these parameters is unknown and no studies have evaluated the clinical optimal cut-off values of DSCSA and SCCSA. This study assessed which parameter is more sensitive.…

Surgical outcomes for lumbar spinal canal stenosis with coexisting cervical stenosis (tandem spinal stenosis): a retrospective analysis of 565 cases.

BACKGROUND: Concurrent cervical and lumbar spinal canal stenosis is known as tandem spinal stenosis (TSS). As research on TSS is limited, there is no consensus on the optimal surgical approach to this problem. We evaluated the prevalence and clinical characteristics of TSS in patients with symptomatic lumbar spinal canal stenosis (LCS). METHODS: The authors performed a retrospective analysis of the outcomes of 565 patients who underwent lumbar surgeries performed for symptomatic LCS. In all the patients, both the cervical and lumbar regions were evaluated preoperatively, and we compared TSS p…

Impact of pedicle-lengthening osteotomy on spinal canal volume and neural foramen size in three types of lumbar spinal stenosis.

OBJECTIVES: Pedicle-lengthening osteotomy is a novel surgery for lumbar spinal stenosis (LSS), which achieves substantial enlargement of the spinal canal by expansion of the bilateral pedicle osteotomy sites. Few studies have evaluated the impact of this new surgery on spinal canal volume (SCV) and neural foramen dimension (NFD) in three different types of LSS patients. METHODS: CT scans were performed on 36 LSS patients (12 central canal stenosis (CCS), 12 lateral recess stenosis (LRS), and 12 foraminal stenosis (FS)) at L4-L5, and on 12 normal (control) subjects. Mimics 14.01 workstation wa…

Saccular Limited Dorsal Myeloschisis with a Short Stalk at the Base of the Sac Causing Spinal Cord Deviation out of the Spinal Canal.

Saccular limited dorsal myeloschisis is a condition in which the spinal cord is tethered by a fibroneural stalk that links the cord to a saccular skin lesion, probably due to a small segmental failure of the disjunction between the neural and cutaneous ectoderm during primary neurulation. We previously reported that the dorsally bent cord deviated from the canal into the sac when the tethering effect of the limited dorsal myeloschisis stalk was strong. These cases of saccular limited dorsal myeloschisis should be prenatally differentiated from myelomeningoceles; however, this is complicated by

Risk factors for underestimating lumbar spinal canal stenosis severity using MRI: A retrospective analysis.

STUDY DESIGN: A retrospective analysis. PURPOSE: This study aimed to identify the risk factors for underestimating the severity of lumbar spinal canal stenosis (LCS) using magnetic resonance imaging (MRI). OVERVIEW OF LITERATURE: In recent years, the indications for surgery for LCS have been increasingly determined by MRI alone, without myelography. However, as most MRI scans are performed in the supine position, it is challenging to detect dynamic stenosis using MRI alone. Thus, if the diagnosis is based on MRI alone without myelography, the severity of LCS may be underestimated. METHODS: …

Acquired Simple Bone Cyst Associated With Lumbar Spinal Canal Stenosis Progression: A Case Report.

A simple bone cyst (SBC) in the posterior lumbar bone structure is very rare. Here, we report a case of SBC at the L5 lumbar lamina with venous obstruction associated with ligamentum flavum thickening. A 59-year-old woman presented with intermittent claudication due to low back pain and bilateral sciatica. A lumbar MRI showed L4-5 lumbar spinal canal stenosis and a T2-weighted image hyperintense lesion in the L5 lamina. Imaging four years earlier showed no lesions in the L5 lamina. Her symptoms improved after lumbar decompression surgery. The L5 lamina lesion was SBC, leading to a diagnosis of…
