The operation known as canal-expansive laminaplasty has the following advantages over wide laminectomy: (1) osseous protection of the spinal cord is retained, (2) the invasion of the cord by scar tissue is minimized, and (3) the stability of the spine is retained. We are reporting the clinical, computed tomographic, and computed myelographic findings before and after this operation in twenty-four patients with cervical myelopathy due to cervical spondylosis, spinal stenosis, or ossification of the posterior longitudinal ligament. The clinical results were excellent in four patients, good in ei

INTRODUCTION: Postural abnormalities (PA) are disabling features of Parkinson's disease (PD). Indirect analyses suggested a higher prevalence of PA among Asian patients compared to Caucasian ones, but no direct comparisons have been performed so far.
METHODS: An international, multicenter, cross-sectional study was performed in 6 European and Asian movement disorders centers with the aim to clarify differences and similarities of prevalence and characteristics of PA in Asian vs. Caucasian PD patients. Axial PA, encompassing antecollis (AC), camptocormia (CC), and Pisa syndrome (PS), and appen

STUDY DESIGN: Case-series.
OBJECTIVE: To report our modified iliac-screw insertion technique and its clinical outcomes.
SUMMARY OF BACKGROUND DATA: Iliac-screws are one of the preferred methods for modern spinopelvic-fixation. However, the technique is not without complications, predominantly because of iliac-screw head prominence, leading to pain and revisions. Conventional iliac-screw entry point is sited superficially at the posterior-superior-iliac-spine (PSIS) contributing to screw-head prominence. We propose a more low-profile, subcrestal entry point that is more medial and inferior to

The authors report on 2 young patients who developed drug-resistant idiopathic dystonic camptocormia (bent spine) and were treated successfully by deep brain stimulation (DBS) of the globus pallidus internus (GPi). The first patient, a 26-year-old woman, suffered for 3 years from such severe camptocormia that she became unable to walk and was confined to bed or a wheelchair. The second patient, a 21-year-old man, suffered for 6 months from less severe camptocormia; he was able to walk but only for short distances with a very bent spine, the arms in a parallel position to the legs, and the hand

BACKGROUND: Anterior or posterior approaches have been shown to be effective in the treatment of cervical stenosis and myelopathy (CSM). There exists, however, a group of patients in whom both the anterior and posterior approaches are necessary to treat the stenosis and deformity. To better identify the indications and outcomes of the contemporaneous anterior + posterior approaches (CAP), we retrospectively reviewed the records of patients who have been treated with this method.
METHODS: Between 2006 and 2018, 37 patients were treated with CAP for kyphosis, stenosis, and subluxation, with a m

OBJECTIVE: To analyze the factors influencing the occurrence of C5 nerve root palsy after posterior approach cervical single-door enlargement kyphoplasty and construct a Nomogram-related prediction model.
METHODS: A total of 255 patients with cervical spondylotic myelopathy who underwent posterior cervical single-door laminoplasty between May 2019 and February 2023 were selected as the research subjects. They were divided into the occurrence group (45 patients) and the non-occurrence group (210 patients) based on whether C5 nerve root palsy occurred after the operation. The general data of pa

INTRODUCTION: Cervical lordosis, a core indicator of spinal sagittal balance, is closely associated with cervical dysfunction when abnormal (straightening or kyphosis). The muscular system, including the upper trapezius (UT) and sternocleidomastoid (SCM), contributes to the mechanical stability of the cervical spine. However, how abnormal curvature affects dynamic muscle fatigue mechanisms remains unclear. This study investigated how abnormal curvature influences the performance of the UT and SCM during movement and their correlations.
METHODS: Employing a cross-sectional case-control design,

Human spinal biomechanics are profoundly complex and not well understood, especially in terms of the dynamic spine function. Translation of biomechanics to disease is difficult, particularly since cause must be separated from effect. Primary dynamics predisposing to the onset of chronic spinal disorders, e.g., adolescent idiopathic scoliosis (AIS) or ankylosing spondylitis (AS), must clearly be differentiated from secondary alterations. This commentary addresses primary biomechanics that may predispose to these idiopathic diseases. A novel hypothesis is proposed, based upon inferences regardin

PURPOSE: Surgical invasiveness indices have been used in adult spine surgery to characterize the invasiveness of complex procedures and for risk stratification. This has not been studied in the pediatric population. The purpose of this study was to develop and validate a surgical invasiveness index for pediatric spinal deformity surgery.
METHODS: The National Surgical Quality Improvement Program (NSQIP) Pediatric database was queried between the years 2016-2022. Patients were included if they were <18 years of age, received posterior or anterior-posterior spinal fusion surgery, and had a diag

INTRODUCTION: Significant spinal corrections require a rigid pelvic foundation (PF) and back muscles related to sagittal alignment. However, criteria for deformity or muscular volume that allow fusion without a PF are lacking. This study aims to assess the feasibility of achieving fusion-ending lumbar spine, focusing on preoperative alignment and back muscle condition, and to analyze how poor alignment progresses over 2 years and the related mechanical failure. METHODS: Spinal instrumentations involving ≥ 4 levels from thoracolumbar to L5 were categorized from the 2-year sagittal vertical axis

OBJECTIVE: This study aimed to assess the ability of experienced spine surgeons to predict the occurrence of proximal junctional failure (PJF) within the first 2 years following ASD surgery, based on patient, surgical, and pre-discharge radiographic evaluation.
METHODS: This case-based survey sent to spinal deformity surgeons included ASD patients who underwent correction-fusion surgery with a minimum follow-up of one year. Twenty patients who underwent posterior fusion and required PJF revision were identified. Another 20 patients with similar characteristics but without PJF served as a cont

BACKGROUND AND OBJECTIVES: For patients with surgical adult spinal deformity (ASD), our understanding of alignment has evolved, especially in the last 20 years. Determination of optimal restoration of alignment and spinal shape has been increasingly studied, yet the assessment of how these alignment schematics have incrementally added benefit to outcomes remains to be evaluated.
METHODS: Patients with ASD with baseline and 2-year were included, classified by 4 alignment measures: Scoliosis Research Society (SRS)-Schwab, Age-Adjusted, Roussouly, and Global Alignment and Proportion (GAP). The i

Camptocormia is defined as an abnormal flexion of the thoracolumbar spine of 45°, or more, that typically increases during walking or standing and completely disappears in the supine position. Camptocormia may occur in patients with Parkinson's disease; when it does, it is usually associated with greater disease severity. Respiratory complications, which may be secondary to abnormal chest function, are one of the most frequent causes of death in patients with Parkinson's disease. No data on lung volumes are available for Parkinson's disease patients with camptocormia. The aim of this study was

BACKGROUND: Parkinson's disease (PD) is occasionally complicated by camptocormia. In a previous study, we classified camptocormia into upper and lower types based on the inflection point, and reported that lidocaine injection into the external oblique muscle, but not into the internal oblique or rectus abdomen, improved upper camptocormia in PD. The effect of a single lidocaine injection disappeared over a period of few days. In this study, we used repeated lidocaine injections into the external oblique for 4-5 days and evaluated the effects of such treatment for up to 90 days.
