Simultaneous repair of pectus excavatum and congenital heart disease.

Most repair of heart lesions and pectus deformity has been performed in adult patients using long incisions, sternal splits, excision of deformed cartilages, and sternal turnover operations that could result in poor cosmesis and chest growth in children because of sternal devascularization. We performed simultaneous pectus repair and atrial septal defect closure in 2 children using a short longitudinal incision and avoiding a transverse or longitudinal sternal split. After extraperichondrial excision of the deformed cartilages and mobilization of the sternum from the neurovascular bundles, a t…

Creation of an intermammary sulcus in congenital synmastia.

Synmastia is a condition where both breasts appear conjoint without an intervening intermammary sulcus. This is usually the result of technical complications during breast implant surgery caused by over-dissection of the medial pocket over the sternum in the subglandular plane, or by an over-division of the pectoralis muscle origin along the sternum in a submuscular plane. As a congenital problem, this is very rare with only a single previous report of an attempt to correct this anomaly. We report a rare case of synmastia in a 19-year-old Indian woman. She had no previous surgery or family his…

[The outcome of modified procedure for pectus carinatum correction].

OBJECTIVE: To improve the surgical procedure to correct pectus carinatum. METHODS: From 1990 to 2003, 9 patients with pectus carinatum were treated, whose ages ranged from 3 years and 6 months to 16 years. The conventional operation was performed on 4 patients, the modified procedure on 5 patients. The modified procedure included: (1) the shortening rib periosteum was not sutured transversally; (2) the corrected position of the sternum was stabilized with the metal strut. RESULTS: In 4 patients corrected by the conventional procedure, the sternum depression as pectus excavatum occurred in 1 …

Multiple heart and pericardial lacerations due to blunt trauma from assault.

A case of 80-year-old woman who died of hemorrhagic shock as a consequence of multiple heart and pericardial lacerations as well as myocardial contusion due to blunt trauma from assault is reported. At postmortem examination, fractures of the body of the sternum and bilateral ribs were detected. A laceration of the anterior left-sided pericardium, a 2.8-cm linear tear in the right venous sinus from the origin of the inferior to the origin of the superior vena cava, a 3-cm laceration between the venous sinus and the right auricle, and a 2-cm tear in the anterior wall of the left atrium, behind …

Sternotomy wounds: rectus flap versus modified pectoral reconstruction.

BACKGROUND: Infected sternotomy wounds occur in 0.5 to 8.4 percent of open heart operations. They are complex problems, with a mortality rate of 8.1 to 14.8 percent despite flap closure. For closure, the pectoralis major flap has had considerable success. However, in providing coverage to the lower third of the sternum, it may be deficient. Modifications to improve closure have included a rectus flap or an anterior rectus fascia extension to the pectoralis flap. METHODS: This retrospective study of 130 consecutive sternotomy wounds compares 41 bilateral pectoralis major muscle flaps with a mo…

[Early and long-term results of sternocostal elevation combined with bridge external traction for funnel chest in children].

The purpose of the present study is to evaluate the effects of addition of a bridge external traction to our conventional sternocostal elevation technique in the surgical management of funnel chest in children. During the period from 1970 to 1979, a total of 29 children with funnel chest were treated surgically using sternocostal elevation technique (Group I). Operative technique consisted of total subperichondral resection of deformed costal cartilages (usually 3rd to 8th, bilaterally), transection of deformed portion of the sternum in 2-3 points and fixation of the sternum in elevated positi…

The surgical approach for reconstruction of the sternal and epigastric defects in children with Cantrell's deformity.

Children with Cantrell's deformity manifest a midline defect of the lower sternum and epigastrium. A surgical technique is described that uses the lowermost sternocostal cartilages in reconstructing the defected sternum and simultaneously reapproximating the rectus abdominis muscles in the middle line for repair of the epigastric defect. Solid cardiac protection and superior cosmetic results were obtained without the use of foreign prosthetic material.

A vascularised rib strut technique for funnel chest correction.

We have refined the Ravitch technique of sternal elevation for surgical correction of funnel chest. Our major modification is introducing a living rib strut for supporting the elevated sternum. The left 5th rib with a vascular pedicle from the internal thoracic artery is turned 180 degrees beneath the sternum. We have used this method in 10 cases. The results have all been satisfactory.

[An unusual complication after tracheal resection for cicatricial stenosis].

The authors describe the case of a patient where after segmental resection of the trachea on account of cicatricial postcannulation stenosis of the trachea osteomyelitis of the sternum developed with a tracheocutaneous fistula. The cause of the fistula was probably a small dehiscence not detected after operation at the site of the anastomosis with subsequent development of chronic inflammatory changes in the anterior mediastinum and sternum. The authors describe the diagnostic procedure and surgical solution of this uncommon complication.

Surgical treatment of pectus carinatum.

Pectus carinatum, just like its sister deformity pectus excavatum, is a condition with an undefined developmental mechanism and debated surgical techniques. Elongation of the costal cartilages and elongation and anterior displacement of the sternum characterize the different varieties of pectus carinatum. Repair of the anomaly involves positional correction as well as the shortening of the sternum and the maintenance of its corrected position by action of the rectus abdominis and pectoralis muscles.

The Zakopane Pulmonary Hospital experience on subxiphoid thymectomy.

BACKGROUND: To present the technique of minimally invasive extended thymectomy performed through the subxiphoid-bilateral subcostal video-assisted thoracoscopic surgery (VATS) approach, with double elevation of the sternum for nonthymomatous myasthenia gravis (MG). METHODS: The whole dissection was performed through the 4-7 cm transverse subxiphoid incision with single 10 mm extra-long bariatric laparoscopy ports inserted subcostally to the right and left chest cavities for videothoracoscope and subsequently for chest tubes. The sternum was elevated with two hooks connected to the sternal fra…

Osteolytic lesion indicating Pseudomonas sternal osteomyelitis.

Two cases of osteolytic lesion of the sternum were caused by Pseudomonas aeruginosa. Both were in heroin addicts, and both occurred as a delayed reaction to injury in an automobile accident. Bony curettage and appropriate antibiotic therapy were sufficient for diagnosis and cure. Tobramycin sulfate (3 mg/kg/day), a new aminoglycoside, was successfully used in both instances. Excision of the sternum was not necessary.

Longitudinal reinforcement for treatment of sternal dehiscence.

Re-fixation of the sternum after sternal dehiscence is still a problem following cardiac surgery. An original technique that allows reinforcement against various causes of dehiscence is described. The technique consists of 3 additional overlapping longitudinal wires set on both sides of the sternum. Over 2 years, this technique was applied in 34 patients, with complete sternal re-fixation in all, and no signs of recurrence on follow-up.

Marlex mesh support for the correction of very severe and recurrent pectus excavatum.

A method for the correction of very severe and recurrent pectus excavatum is presented. The technique consists of mobilization of the sternum, transverse osteotomy, parasternal resection of the costal cartilages (modified Ravitch procedure), followed by placement of Marlex mesh behind the sternum and suturing the edge of the Marlex mesh to the peripheral stump of the resected ribs. This method has been used with good results in 6 patients, 2 of them with recurrent deformities.

Forced Straightening of the Back Does Not Improve Body Shape.

BACKGROUND: Sitting for a long time causes various postural problems, such as slump sitting. It has been reported that employing a corrected sitting position while lifting the sternum is effective in improving this form of posture. We investigated how a corrected sitting posture with the lifting of the sternum is different from a forced position that is applied through the command and passive positions. MATERIALS AND METHODS: The postural angle of 270 subjects aged 19-23 years was measured in the passive, forced, and corrected positions using a Saunders inclinometer and a Formetric 4D system.…

[Preliminary results in the correction of the pectus excavatum with the Acastello modified Welch technique].

BACKGROUND: Congenital malformations of the chest wall are a heterogeneous group of diseases affecting the costal cartilage, ribs, sternum, scapula and clavicle. The pectus excavatum is characterized by a posterior depression of the sternum. Acastello-Welch technique consists in a partial resection of the costal cartilages adding some bars or plates unilaterally fixed to the sternum in each hemithorax. MATERIALS AND METHODS: From October 2008 to March 2011 we evaluated 108 patients with congenital malformations of the chest wall. Forty-seven patients (44%) had a pectus excavatum and 12 were t…

Pectus carinatum.

In the mirror of the pertinent literature, we present our experiences gained in 161 operations for pectus carinatum. The Type I (keel chest) deformity is corrected by bilateral resection of the costal cartilages, transverse osteotomy of the sternum, detachment of the xiphoid process, and resection of the lower end of the body of the sternum. The sternum is maintained in its corrected position by utilizing the pulling force of the rectus muscles through the reattached xiphoid and by tacking the pectoralis muscles together in front of the breast bone. Type II (pouter pigeon breast) is handled by…
