BACKGROUND AND PURPOSE: Patients with ischemic or hemorrhagic stroke are at risk for systemic complications. The reasons why gastrointestinal bleeding occurs after stroke are unknown and have intuitively been attributed to stress ulcers. No study to date has addressed causes of gastrointestinal hemorrhage in stroke.

We have encountered 38 cases of juvenile ischemic strokes during the past 10 years, which included 16 cases of moyamoya disease and 22 cases of non-moyamoya strokes. The etiology could not be determined in most non-moyamoya strokes, except in 4 cases of cardiogenic embolism and 1 case of post-meningitic stroke. We further examined 13 cases of idiopathic strokes, which showed clinical symptoms of so-called acute infantile hemiplegia. We report on four patients with unilateral tremor stemming from cerebrovascular accidents. In two patients with proven lesions of the thalamus, the tremor was irregular and, in addition, there was dystonic posturing of the affected arm. Tremor and dystonic posturing had appeared after the stroke. In the other two patients tremor had occurred immediately at the onset of the stroke and lasted only a few days. The tremors were of small amplitude and high frequency, and lesions could not be found on CT or MRI in these two patients. None of our patients showed signs of the so-called rubral tremor.

A 25-year-old woman with a history of basilar migraine attacks ultimately resulting in a basilar artery migraine stroke is described. Brain computed tomography showed bilateral hypodense lesions in the cerebellar hemispheres. A survey of literature on basilar artery migraine is presented and the relation between stroke and migraine is discussed.

We compared the relative elevations of the two diaphragms in the supine chest films of 62 patients with recent hemiparesis due to a supratentorial stroke with that of 92 acute coronary patients without neurologic disease. Patients with severe hemiparesis or lesions larger than 40 ml exhibited an excessive elevation of the diaphragm on the paretic side compared with controls. The European Stroke Prevention Study showed: 1. That the association Persantin-Aspirin reduces stroke and vascular death risks with 36,5% in a population of 2500 patients. Risk reduction persisted during the two years follow-up and was the same for men and women. 2. That this reduction is higher than the reduction obtained in all other studies with anti-aggregation prevention. 3. That studies with less than 600 patients cannot lead to significant differences.

Ten cases of death due to heat stroke are described. They were all young men who collapsed during running exercise or route march and died in hospital later. Post-mortem examination was carried out in all cases. Death was due to disseminated intravascular coagulation with widespread microthrombus formation and coagulative necrosis involving many organs. Meteorological studies showed that at the time of the collapse the environmental temperature was higher than average although it may have been in the morning or evening.
