Hyperinfection in strongyloidiasis has been associated with corticosteroid treatment. Other immunodepressive conditions also seem to facilitate the state of hyperinfection. Two cases of fatal strongyloidiasis associated with diabetes mellitus and malnutrition are reported. The patients presented with repeated vomiting and upper gastrointestinal bleeding respectively. Strongyloides stercoralis usually causes chronic asymptomatic infection in humans. However, in patients with AIDS, malignancy, and individuals receiving corticosteroids, disseminated infection can develop, associated with an extremely high mortality rate and frequent treatment failure with thiabendazole. Recently, ivermectin was found to be very effective in such patients. The authors report a case of strongyloidiasis in a patient with hypogammaglobulinemia in which ivermectin failed to clear Strongyloides larvae from stool, despite repeated courses of treatment throughout 14 months.

In summary, we present this as the first reported case of both isosporiasis and strongyloidiasis complicating HTLV-I-associated ATLL. Prompt diagnosis and treatment of these parasites in the immunocompromised host are necessary to prevent severe wasting and dehydration. Large numbers of Americans were exposed to this potentially lethal parasite during the Southeast Asian war. Albendazole, a new broad spectrum anthelmintic, has been tested in 66 adult patients infected by S. stercoralis. 77.2% cure rate was observed following 800 mg once a day for 3 consecutive days, based on repeated fecal examinations using a concentration technic (Junod) and a Baermann test, carried out during 6 to 9 months following treatment. Associated ancylostomiasis caused by N. americanus and trichuriasis were also completely cured (100%). Based on clinical side effects and laboratory investigations, the drug did not produce any significant adverse reactions.

Patients harbouring worm infections usually have very marked elevations of serum immunoglobulin E (IgE) concentration. However, in a group of twenty ex-Far East prisoners of war with strongyloidiasis only two (10%) had raised levels due to the worm, and these were only moderately elevated. This poor IgE response may be related to the very longstanding nature of the infection (at least 30 years). The patients may have a form of immunological incompetence, or may have become immunologically tolerant to the parasite.
