The long term effectiveness of intensive stuttering therapy: a mixed methods study.

PURPOSE: The purpose of this study was to gain a deeper understanding of client perceptions of an intensive stuttering therapy program that utilizes a multi-faceted approach to therapy. The study also proposed to gain a deeper understanding about the process involved in long-term maintenance of meaningful changes made in therapy. METHODS: The study used a mixed methods design. The core method was a phenomenological approach using semi-structured interviews. Objective clinical data was gathered concurrently. This included archival records and current measures on a variety of assessments to eva…

Investigating optimal intervention intensity with the Lidcombe Program of early stuttering intervention.

This paper addresses optimal intervention intensity in stuttering with reference to the Lidcombe Program of early stuttering intervention. This is an operant program in which the parent provides the actual treatment, for proscribed periods each day, in the child's everyday environment. The parent learns how to do this during weekly visits with the child to the speech-language pathologist. This program was chosen because it is supported by considerable research evidence. This evidence includes randomized controlled trials and file audits. Individual children vary in the time taken to reach the …

Self-modelling as a relapse intervention following speech-restructuring treatment for stuttering.

BACKGROUND: Speech restructuring is an efficacious method for the alleviation of stuttered speech. However, post-treatment relapse is common. AIMS: To investigate whether the use of video self-modelling using restructured stutter-free speech reduces stuttering in adults who had learnt a speech-restructuring technique and subsequently relapsed. METHODS & PROCEDURES: Participants were twelve adults who had previously had speech-restructuring treatment for stuttering and relapsed. They were video recorded for 1 hour within the clinic, practising their speech-restructuring technique. The videos …

Stuttering priapism complicating warfarin therapy in a patient with protein C deficiency.

Priapism is a rare disorder defined as a persistent penile erection that continues hours beyond, or is unrelated to, sexual stimulation. There are two types of priapism; ischemic low-flow type or non-ischemic high flow, with differing etiologies. Priapism associated with thrombophilia is a well-recognized entity. However, the pathogenesis of this association is not fully understood. We report a rare case of recurrent (stuttering) priapism in a patient with protein C deficiency while maintained on Warfarin therapy. This therapy was also complicated by Warfarin-induced skin necrosis.

Stuttering in adults: the acoustic startle response, temperamental traits, and biological factors.

UNLABELLED: The purpose of this study was to investigate the relation between stuttering and a range of variables of possible relevance, with the main focus on neuromuscular reactivity, and anxiety. The explorative analysis also included temperament, biochemical variables, heredity, preonset lesions, and altered auditory feedback (AAF). An increased level of neuromuscular reactivity in stuttering adults has previously been reported by [Guitar, B. (2003). Acoustic startle responses and temperament in individuals who stutter. Journal of Speech Language and Hearing Research, 46, 233-240], also in…

Predicting stuttering from linguistic factors for German speakers in two age groups.

UNLABELLED: Brown's factors [J. Speech Disorders 10 (1945) 181] predict the likely loci of disfluency in English-speaking adults who stutter … amples, because of cross-linguistic differences in syllable onset properties. It was predicted that stuttering would be more likely in later sentence positions in German because in German the verb is usually near the end of a sentence. There were no obvious reasons to expect differences on the two remaining factors, content words and word length. With childr …

Speech timing in children after the Lidcombe Program of early stuttering intervention.

It is known that operant treatments can control stuttering in children. However, at present it is unknown why such treatments are effective. Changes in the usual way of speaking are frequently observed after behavioural treatments for adults who stutter, and it is possible that operant treatments for children also invoke such changes. To explore this idea, selected acoustic measures of speech timing were made in eight preschool children before and after receiving the Lidcombe Program, which is an operant treatment for stuttering. No systematic changes were detected after treatment. Considering…

Brain correlates of stuttering and syllable production. A PET performance-correlation analysis.

To distinguish the neural systems of normal speech from those of stuttering, PET images of brain blood flow were probed (correlated voxel-wise) with per-trial speech-behaviour scores obtained during PET imaging. Two cohorts were studied: 10 right-handed men who stuttered and 10 right-handed, age- and sex-matched non-stuttering controls. Ninety PET blood flow images were obtained in each cohort (nine per subject as three trials of each of three conditions) from which r-value statistical parametric images (SPI¿r¿) were computed. Brain correlates of stutter rate and syllable rate showed striking …

Speech naturalness and prolonged-speech treatments for stuttering: further variables and data.

It is well known that unusual speech quality may result from stuttering treatments that are based on prolonged speech. However, empirical information concerning the speech quality associated with those treatments is lacking. The present study was designed to contribute such empirical information. Results indicated that speech quality assessments of posttreatment clients, using Martin, Haroldson, and Triden's (1984) speech naturalness scale, gave similar results regardless of whether they were based on monologues or conversations. The speech quality of those clients remained stable at the concl…

Monitoring early stuttering onset before treatment is due: A cross-sectional study.

INTRODUCTION: Early intervention for stuttering is essential. When immediate treatment is not indicated, therapists often monitor stuttering over time. However, how such monitoring is implemented in daily practice has not been studied in-depth. METHOD: 35 Belgian stuttering therapists completed a questionnaire addressing three questions: How do therapists monitor early stuttering? When do they end monitoring? When do they provide recommendations to parents, and which ones? RESULTS: Monitoring early stuttering varies significantly. Monitoring can be proactive (therapist-led, scheduled session…

Directed content analysis of stuttering discourse on a Ghanaian social media platform.

PURPOSE: Social media platforms are influential online spaces for public discourse, including health-related discussions. This exploratory study examined how members of the Ghanaian Facebook community "Tell It All" expressed their beliefs about the causes of stuttering, management strategies, and their attitudes toward people who stutter. METHOD: Using a search-based sampling approach, 12 positive hits out of 29 original user posts from 2021 to 2025 were selected for analysis. Data from 658 comments related to these 12 original posts were analyzed using a deductive-inductive qualitative conte…

Functionally Oriented Genetic Analyses Reveal Potential Transcriptomic and Neurological Mechanisms of Stuttering.

Speech and language are fundamental to the human experience, allowing for the sharing of thoughts and emotions through the coordination of many neurological and linguistic systems. Disruptions in these processes can lead to speech and language disorders, including stuttering, which is characterized by prolongations, blocks, and repetitions of speech sounds. To date, almost 60 genome-wide significant loci have been associated with stuttering. Alas, most of these signals appear in non-coding regions of the genome and thus remain largely uncharacterized. In this study, we probed functionality by …

Stuttering: Our Current Knowledge, Research Opportunities, and Ways to Address Critical Gaps.

Our understanding of the neurobiological bases of stuttering remains limited, hampering development of effective treatments that are informed by basic science. Stuttering affects more than 5% of all preschool-age children and remains chronic in approximately 1% of adults worldwide. As a condition that affects a most fundamental human ability to engage in fluid and spontaneous verbal communication, stuttering can have substantial psychosocial, occupational, and educational impacts on those who are affected. This article summarizes invited talks and breakout sessions that were held in June 2023 …

Psychedelics in developmental stuttering to modulate brain functioning: a new therapeutic perspective?

Developmental stuttering (DS) is a neurodevelopmental speech-motor disorder characterized by symptoms such as blocks, repetitions, and prolongations. Persistent DS often has a significant negative impact on quality of life, and interventions for it have limited efficacy. Herein, we briefly review existing research on the neurophysiological underpinnings of DS -specifically, brain metabolic and default mode/social-cognitive networks (DMN/SCN) anomalies- arguing that psychedelic compounds might be considered and investigated (e.g., in randomized clinical trials) for treatment of DS. The neural b…

Can Stoicism inspire stuttering intervention? The clinical usefulness of an ancient philosophy.

BACKGROUND: A range of psychotherapies are effective in managing an individual's personal reactions to stuttering and reducing the impact stuttering has on their lives. Many of these therapies, such as cognitive behavioural therapy, have their origins in Stoicism, an ancient Greek philosophy founded in 301 bce. Stoicism remains a relatively unexplored topic in the psychotherapy and speech and language therapy literatures. AIMS: This paper aims to highlight the potential benefits of integrating Stoic principles and techniques into stuttering intervention. METHODS: This aim is achieved through…

Phenome risk classification enables phenotypic imputation and gene discovery in developmental stuttering.

Developmental stuttering is a speech disorder characterized by disruption in the forward movement of speech. This disruption includes part-word and single-syllable repetitions, prolongations, and involuntary tension that blocks syllables and words, and the disorder has a life-time prevalence of 6-12%. Within Vanderbilt's electronic health record (EHR)-linked biorepository (BioVU), only 142 individuals out of 92,762 participants (0.15%) are identified with diagnostic ICD9/10 codes, suggesting a large portion of people who stutter do not have a record of diagnosis within the EHR. To identify ind…

Speech-Language Pathologists' Comfort Level With Use of Term "Stuttering" During Evaluations.

Purpose Speech-language pathologists (SLPs) anecdotally report concern that their interactions with a child who stutters, including even the use of the term "stuttering," might contribute to negative affective, behavioral, and cognitive consequences. This study investigated SLPs' comfort in providing a diagnosis of "stuttering" to children's parents/caregivers, as compared to other commonly diagnosed developmental communication disorders. Method One hundred forty-one school-based SLPs participated in this study. Participants were randomly assigned to one of two vignettes detailing an evaluatio…

Speech-activated Myoclonus Mimicking Stuttering in a Patient with Myoclonus-Dystonia Syndrome.

BACKGROUND: Acquired neurogenic stuttering has been considered a fairly uncommon clinical occurrence; speech-activated myoclonus is a rare entity that can mimic stuttering and is caused by a wide array of etiologies. CASE REPORT: Here we report a patient with myoclonus-dystonia syndrome (MDS), due to an identified disease-causing mutation, who displayed speech-activated myoclonus mimicking stuttering. DISCUSSION: In MDS, myoclonus has only infrequently been reported to affect speech. This case further expands the spectrum of conditions causing the rare clinical phenomenon of speech-activated…
