California's Recovery Incentives Program: Implementation strategies.

INTRODUCTION: Due to the increasing role of psychostimulants in the US drug poisoning crisis, there is an increasing need to effectively implement evidence-based treatment for individuals with stimulant use disorder (StimUD). Contingency management is a behavioral strategy with robust evidence of support for the treatment of StimUD. In 2023, California initiated a large-scale effort to implement CM as a treatment for individuals with a stimulant use disorder (cocaine, methamphetamine, amphetamine) called the Recovery Incentives Program: California's Contingency Management Benefit. METHODS: Th

Telehealth and Hybrid Service Use for Major Depression and Substance Use Disorders Among U.S. Adults, 2022-2023.

INTRODUCTION: This study examined patterns of telehealth and hybrid service use (telehealth combined with outpatient or inpatient care) for major depressive episodes and substance use disorders among U.S. adults. METHODS: Pooled data from the 2022 and 2023 National Survey on Drug Use and Health (N=92,233) were analyzed to examine trends and patterns in overall telehealth use and hybrid service use among adults with past-year major depressive episodes and those with past-year substance use disorders. Bivariate analyses and survey-weighted multivariable logistic regression were conducted betwee

Substance use and use disorders during pregnancy and the postpartum period.

Substance use and substance use disorders among pregnant and postpartum individuals have risen dramatically, necessitating enhanced clinician education and intervention strategies. This expert review offers obstetricians and gynecologists a comprehensive overview of substance use during the prenatal and postpartum periods. We discuss the epidemiology, maternal and fetal adverse effects, as well as treatment approaches for major substances: nicotine, cannabis, alcohol, benzodiazepines, stimulants, and opioids. Additionally, we address the ethical and legal implications of substance use during p

The relationship of frequency of cocaine use to substance and psychiatric disorders in the U.S. general population.

BACKGROUND: In clinical trials of pharmacotherapy for substance use, abstinence is the primary endpoint accepted by regulatory agencies. However, this endpoint could be overly restrictive, impeding efforts to identify effective medications for cocaine use disorder. To examine non-abstinent gradations in cocaine use as potential indicators of improvement, we investigated the relationship of frequency of cocaine use to clinical correlates in national survey data. METHODS: Lifetime cocaine users (n = 2501) were interviewed in the National Epidemiological Survey on Alcohol and Related Conditions

Beyond mood: alcohol and substance use older adults with bipolar disorder.

PURPOSE OF REVIEW: Substance use and substance use disorders (SUD) are common comorbidities in bipolar disorder (BD) and remain clinically consequential in older-age bipolar disorder (OABD), where medical vulnerability increases and detection may be more challenging. This review summarizes epidemiologic and clinical findings on substance use in OABD and highlights implications for assessment and management. RECENT FINDINGS: Recent longitudinal studies suggest that fluctuations in alcohol consumption, even when modest, are associated with poorer mood stability and functioning in BD, while wors

Psychiatric comorbidity in substance use disorders, a systematic review of neuro-imaging findings.

Substance use disorder (SUD) have negative consequences for affected individuals and society. Current treatments are moderately effective, partly due to the large heterogeneity in SUDs, including co-occurring psychopathology. A better understanding of the mechanisms underlying these frequently co-occurring psychiatric conditions is required to develop individualized treatments to increase treatment success rates. We systematically reviewed case-control studies investigating neurobiological differences measured using neuroimaging between participants with SUD only and participants with SUD and

Neuropsychological Instruments and Tasks for Dependence Behaviors in Medication-Overuse Headache.

PURPOSE OF REVIEW: This review aims to discuss about the potential roles of neuropsychological instruments and tasks in the evaluation of dependence behaviors shared by medication-overuse headache (MOH) and substance use disorders (SUDs). RECENT FINDINGS: Recent studies utilizing criteria from the Diagnostic and Statistical Manual of Mental Disorders (DSM) for SUDs have revealed that MOH patients often exhibit impaired control over medication use, along with tolerance and withdrawal symptoms. In addition, dependence questionnaires such as the Leeds Dependence Questionnaire and the Severity of

Novel potential pharmacological approaches in treating eating disorders comorbid with substance use disorders.

Eating disorders (EDs), including anorexia nervosa (AN), bulimia nervosa (BN), and binge eating disorder (BED), frequently co-occur with substance use disorders (SUDs), affecting 20-30 % of individuals with either condition. This significant overlap is driven by shared neurobiological mechanisms, such as disruptions in dopamine, opioid, and cannabinoid systems, as well as by psychological traits including impulsivity, anxiety, and stress. While AN is often characterized by restrictive behaviors, BN and BED involve compulsive binge episodes, mirroring addiction patterns observed in SUDs. The cu

Clinical pharmacology of ayahuasca: potential applications and future considerations.

INTRODUCTION: In the last few years, interest is growing around the topic of possible therapeutic properties of substances classified as psychedelic, especially for the treatment of mental disorders. Ayahuasca is a classic psychedelic with a complex composition, which has been used in traditional contexts for centuries. AREAS COVERED: In this review, we explore the current evidence and limitations around ayahuasca use for the treatment of depression, anxiety, posttraumatic stress and substance use disorders, with a focus on clinical and observation studies. EXPERT OPINION: Similar to what ha

What is known about persons with co-occurring problems' experiences with supported housing, recovery, and health promotion? A scoping review.

BACKGROUND: Having a home is the foundation of most people's lives. People with co-occurring substance use and mental health problems may experience challenges in acquiring and keeping housing. Many also have major health challenges. Supported housing is the subject of increasing interest, but there seems to be a lack of studies exploring supported housing's potential for facilitating recovery and health promotion. Therefore, a scoping review was performed to answer our review question: What is known in the literature about the experiences of persons with co-occurring substance use and mental

Subjective effects as predictors of substance use disorders in a clinical sample: A longitudinal study.

BACKGROUND: The literature on the association between subjective effects (SEs; i.e., how an individual perceives their physiological and psychological reactions to a drug) and substance use disorders (SUDs) is largely limited to community samples. The present study addressed the following aims in a clinical sample: whether SEs predict general versus substance-specific SUD in adolescence and adulthood after controlling for conduct disorder symptoms (CDsymp); whether SEs predict SUDs across drug classes; whether SEs predict change in SUD from adolescence to adulthood; and whether there are racia

Incidence of inpatient cases with mental disorders due to use of cannabinoids in Germany: a nationwide evaluation.

BACKGROUND: Quantitative (e.g. increasing recreational cannabinoid use) and qualitative (e.g. increasing availability and use of synthetic cannabinoids and cannabis preparations with increased tetrahydrocannabinol content) changes in cannabinoid use may be associated with changes in the prevalence of cannabinoid-related mental and behavioural disorders and, accordingly, changes in the need for medical care. We aimed to investigate if there are changes in the number of inpatient cases (ICs) due to cannabinoid-related disorders in Germany. METHODS: Data were obtained from the Federal Statistica

Management of Pregabalin Use Disorder: A Case Series.

Pregabalin is indicated for the treatment of partial epilepsy, generalized anxiety disorder, and neuropathic pain. The first reports on pregabalin use disorder have been published in Europe in 2010 and notified to the French Addictovigilance Network (FAN) in 2011. The management of pregabalin use disorder is challenging due to the risks associated with the abrupt withdrawal and lack of guidelines. In this retrospective observational study, the management of pregabalin use disorder was analyzed in eight cases reported to the addictovigilance center of Montpellier, France, between 2019 and 2020.

Substance use in psychiatric crisis: relationship to violence.

BACKGROUND: Substance use and psychiatric illness, particularly psychotic disorders, contribute to violence in emergency healthcare settings. However, there is limited research regarding the relationship between specific substances, psychotic symptoms and violent behaviour in such settings. We investigated the interaction between recent cannabinoid and stimulant use, and acute psychotic symptoms, in relation to violent behaviour in a British emergency healthcare setting. METHODS: We used electronic medical records from detentions of 1089 individuals under Section 136 of the UK Mental Health A

Polysubstance use by psychiatry inpatients with co-occurring mental health and substance use disorders.

BACKGROUND: Polysubstance use, the consumption of more than one substance over a defined period, is common and associated with psychiatric problems and poor treatment adherence and outcomes. This study examined past-month polysubstance use at intake among psychiatry inpatients with co-occurring mental health and substance use disorders, and outcomes 3 months later. METHODS: Participants (n=406 psychiatry inpatients with documented mental health and substance use disorders) completed a baseline and a 3-month follow-up (84%) interview. With baseline data, a latent class analysis was conducted o
