The significance of ante- and perinatal periods for formation of risk of sudden infant death syndrome.

The results of the research, which are carried out according to two international programs on studying and prevention of the sudden infant death syndrome (SIDS), are submitted in the article. The features of ante- and perinatal periods, characteristics of obstetric history of the mothers of deceased babies are analyzed, the relative risk of SIDS development is estimated at the influence of a complex of factors. High parity, young age of mother at the time of delivery, poor antenatal care, preterm delivery, intrauterine fetal growth retardation, fast delivery, alcohol and coffee abuse by a moth…

A review of the status of magnesium and related minerals in the sudden infant death syndrome (SIDS).

Sudden infant death syndrome (SIDS) is the sudden death of an infant under 1 year of age that remains unexplained after a thorough case investigation, including performance of a complete autopsy. Despite recent reductions in the SIDS rate attributed to placing the baby to sleep on his back, SIDS remains the most common cause of infant mortality between 1 month and 1 year of age in developed countries. This review concerns the current state of a hypothesis (1972) that magnesium deficiency, which causes sudden death in young mammals, is the major cause of SIDS. Numerous investigators have compar…

Observations made on severe apneic spells in two infants at risk for sudden death.

We have studied the sequence of events during prolonged apneic spells in two infants who, by virtue of multiple risk factors or ultimate death, were at risk for sudden death. Using techniques to quantitate airflow and pharyngeal airway pressures, we have reported many descriptive aspects of the apnea in these infants. Specifically, we have noted that severe apneic spells in these infants were accompanied by a wide spectrum of lengths and types of shorter apneas. In the case of infant A, the most severe spells occurred in clusters. Physiologic phenomena usually considered normal (sighs, swallow

Ventilatory pattern following diphtheria-tetanus-pertussis immunization in infants at risk for sudden infant death syndrome.

To evaluate the effects of diphtheria-tetanus-pertussis (DTP) immunization on the ventilatory pattern during sleep in infants at increased risk for sudden infant death syndrome (SIDS), we performed overnight pneumograms (recordings of ventilatory pattern and electrocardiogram) on 30 control infants, 46 infants with unexplained apnea, and 33 subsequent siblings of SIDS victims the night before and the night following a DTP immunization. Pneumograms were quantitated for total sleep time, longest apnea (in seconds), total duration of apneas longer than 6 s (in minutes), and total periodic breathi…

Incidence and significance of primary abnormalities of cardiac rhythm in infants at high risk for sudden infant death syndrome.

The exact relationship between cardiac arrhythmias and sudden infant death syndrome (SIDS) is uncertain. Several reports have implicated both ventricular and supraventricular arrhythmias in isolated cases, but there have been no studies of the incidence or type of arrhythmias that occur in populations at risk for SIDS. Of 1699 infants at high risk for SIDS, 60 (4%) were found to have a primary cardiac arrhythmia (i.e., not associated with disordered respiration or apnea). The incidence of atrial and ventricular premature beats, supraventricular tachycardia, and Wolff-Parkinson-White syndrome w…

Sudden unexplained death in infants and children: the role of undiagnosed inherited cardiac conditions.

Sudden unexplained death in childhood is a traumatic event for both the immediate family and medical professionals. This is termed sudden unexplained or arrhythmic death syndrome (SUDS/SADS) for children over 1 year of age while sudden unexplained death in infancy or sudden infant death syndrome (SUDI/SIDS) refers to unexplained deaths in the first year of life. There is increasing evidence for the role of undiagnosed inherited cardiac conditions, particularly channelopathies, as the cause of these deaths. This has far-reaching implications for the family regarding the potential risk to other …

Sudden twin infant death on the same day: a case report and review of the literature.

Sudden infant death syndrome (SIDS) is a major contributor to infant mortality. The cause of death is unknown: suggested possibilities include cardiovascular disease, anaphylactic shock, and suffocation. The occurrence of simultaneous sudden infant death syndrome is uncommon, such cases being extremely rare in forensic pathologic practice. We report two 10-week-old male twins who appeared well at the time of their evening feeding, yet died while sleeping on their backs. Both infants had petechial hemorrhages on the visceral pleura, epicardial surface of the heart, and thymus gland. Microscopic…

Changes in the classification of sudden unexpected infant deaths: United States, 1992-2001.

BACKGROUND: Sudden infant death syndrome (SIDS) makes up the largest component of sudden unexpected infant death in the United States. Since the first recommendations for supine placement of infants to prevent SIDS in 1992, SIDS postneonatal mortality rates declined 55% between 1992 and 2001. OBJECTIVE: The objective of this analysis was to examine changes in postneonatal mortality rates from 1992 to 2001 to determine if the decline in SIDS was due in part to a shift in certification of deaths from SIDS to other causes of sudden unexpected infant death. In addition, the analysis reviews the c…

Polysomnographic studies and home monitoring of siblings of SIDS victims and of infants with no family history of sudden infant death.

We report preliminary results of a prospective study conducted to prevent sudden death in asymptomatic infants. From 1977-1984, 3658 infants were studied polygraphically. There were 923 siblings of SIDS victims and 2735 infants with no personal of family history of SIDS. The infants were studied at 8 weeks of age. Polygraphic "risk factors" were defined by central apnoeas longer than 15 s; periodic breathing above 5% sleep time; or obstructive apnoeas above 3 s. In 937 infants "risk factors" were seen and a second study was requested 4 weeks later. Out of 891 infants re-studied at 12 weeks, 15

Comparative analysis of microRNA expression in serum-derived extracellular vesicles from sudden infant death syndrome cases.

Sudden infant death syndrome (SIDS) remains one of the most common and poorly understood diagnoses of death in infants. In this study, we searched for novel biomarkers to aid in elucidating the pathogenesis of SIDS through a bioinformatics analysis of serum-derived extracellular vesicle (EV) miRNAs using next-generation sequencing. Comparative analyses between infants who died of SIDS and those who died from known causes showed that 15 and 38 miRNAs were significantly up- or down-regulated more than twofold in SIDS, respectively. Myocardial-specific miRNAs, such as miR-1, miR-208, and miR-499,…

Characteristics Associated with Sudden Unexpected Infant Death in a Rural Hispanic Population: A Case-Control Study.

INTRODUCTION: Sudden unexpected infant death (SUID) is the leading cause of death among U.S. infants aged 28 days to 1 year. In Kansas, Hispanic infant mortality is nearly 50% higher than non-Hispanic White. Further, the SUID rate did not change between 2005-2018, while rates for non-Hispanic Black and White infants decreased significantly. This study sought to identify characteristics and behaviors of Hispanic birthing persons related to SUID. METHODS: Linked Kansas birth/death vital statistics data (2005-2018) identified Hispanic birthing persons with a singleton birth who experienced SUID.

Non-polyalanine repeat mutation in PHOX2B is detected in autopsy cases of sudden unexpected infant death.

BACKGROUND: Congenital central hypoventilation syndrome (CCHS), which is caused by PHOX2B with phenotypic variations, has a point of controversy: CCHS is putatively involved in autopsy cases of sudden unexpected infant death (SUID) including sudden infant death syndrome. OBJECTIVE: The relation of CCHS to SUID cases was investigated by extensive genotyping of PHOX2B. METHODS: We analyzed 93 DNA samples of less than one-year-old SUID cases that were autopsied in our department. Unrelated adult volunteers (n = 942) were used as the control. RESULTS: No polyalanine tract expansion was detected…

Medullary Serotonergic Binding Deficits and Hippocampal Abnormalities in Sudden Infant Death Syndrome: One or Two Entities?

Sudden infant death syndrome (SIDS) is understood as a syndrome that presents with the common phenotype of sudden death but involves heterogenous biological causes. Many pathological findings have been consistently reported in SIDS, notably in areas of the brain known to play a role in autonomic control and arousal. Our laboratory has reported abnormalities in SIDS cases in medullary serotonin (5-HT) receptor 1A and within the dentate gyrus of the hippocampus. Unknown, however, is whether the medullary and hippocampal abnormalities coexist in the same SIDS cases, supporting a biological relati…

The Mesencephalic Periaqueductal Gray, a Further Structure Involved in Breathing Failure Underlying Sudden Infant Death Syndrome.

The aim of this study was to investigate the involvement of the periaqueductal gray (PAG), an area of gray matter surrounding the cerebral aqueduct of Sylvius, in the pathogenetic mechanism of SIDS, a syndrome frequently ascribed to arousal failure from sleep. We reconsidered the same samples of brainstem, more precisely midbrain specimens, taken from a large series of sudden infant deaths, namely 46 cases aged from 1 to about 7 months, among which 26 SIDS and 20 controls, in which we already highlighted significant developmental alterations of the substantia nigra, another mesencephalic struc…

Ambient Air Pollution and Sudden Infant Death Syndrome in Korea: A Time-Stratified Case-Crossover Study.

Sudden infant death syndrome (SIDS) is an occasional cause of unexpected mortality in infancy. While various etiological factors have been hypothesized, air pollution has been consistently presented as an environmental factor. In this study, we aimed to estimate the risk of SIDS in relation to exposure to air pollution and the effects of its modifying factors. A mortality dataset with supplementary infant mortality survey data from Statistics Korea was used and combined the concentration of ambient air pollution data from AirKorea based on the date of death and residential addresses of the SID…

Functional Invalidation of Putative Sudden Infant Death Syndrome-Associated Variants in the KCNH2-Encoded Kv11.1 Channel.

BACKGROUND: Heterologous functional validation studies of putative long-QT syndrome subtype 2-associated variants clarify their pathological potential and identify disease mechanism(s) for most variants studied. The purpose of this study is to clarify the pathological potential for rare nonsynonymous KCNH2 variants seemingly associated with sudden infant death syndrome. METHODS: Genetic testing of 292 sudden infant death syndrome cases identified 9 KCNH2 variants: E90K, R181Q, A190T, G294V, R791W, P967L, R1005W, R1047L, and Q1068R.
