Clonal heterogeneity of synovial fluid T lymphocytes in inflammatory synovitis.

Several studies have examined patients with rheumatoid arthritis for the presence of oligoclonal populations of synovial T lymphocytes. The results of these studies have been conflicting. In this study one patient with rheumatoid arthritis and two with other forms of inflammatory synovitis were examined by Southern blot analysis of T cell clones generated from synovial fluid by primary limiting dilution. Evidence of oligoclonality was documented only in a patient with psoriatic arthritis. The distinguishing characteristics of this patient, in addition to the diagnosis, included the fact that o…

Remitting, seronegative, symmetrical synovitis with pitting edema--13 additional cases.

The case histories of 13 elderly patients (8 men/5 women) with a relatively acute onset of a severe symmetrical synovitis affecting the flexor digitorum tendon sheaths and wrist joints with pitting edema of the dorsum of both hands are described. All were persistently seronegative for IgM rheumatoid factors and all went into complete remission without relapse. Asymptomatic residual flexion contractures of the fingers and wrists were a constant feature during remission. HLA-B7 was present in 15 of a total 23 reported cases (relative risk = 4.4). This condition, with its excellent prognosis, is …

Ankle extensor tendon synovitis due to a date palm thorn.

Palm thorn injury to tendons and joints is characterized in most patients by an acute inflammatory stage followed by a relatively asymptomatic period and finally by chronic synovitis or tenosynovitis. Once the inflammatory process has begun, detection and localization of the thorn can be very difficult. We present an unusual case of palm thorn tenosynovitis of the ankle extensor tendon which was detected and localized preoperatively by ultrasound. Removal of the thorn and synovectomy were curative.

Tuberculous synovitis with "rice bodies" presenting as carpal tunnel syndrome.

A 41-year-old man had typical symptoms and signs of carpal tunnel syndrome. At operation there were multiple large rice bodies along the flexor tendons with a great deal of adherent synovitis involving the index finger. Widespread surgical debridement with excision of involved synovium was done. Mycobacterium tuberculosis was cultured from the tenosynovium excised.

Articular cartilage deformation following experimental synovitis in the rabbit hip.

The development of transient synovitis in the hips of young children occurs quite frequently. This experiment examined the effects of a model synovitis on the deformability of articular cartilage of the immature rabbit hip. At 1, 2, 3, and 4 weeks following synovitis, there was an increase of cartilage deformability on both the acetabular and femoral sides of the joint. This increased deformability may alter force transmission to the underlying bone and its contained vascular structures.

A pathogenetic model for erosive synovitis: lessons from animal arthritides.

Adjuvant arthritis, streptococcal cell wall-induced arthritis, and Erysipelothrix insidiosa arthritis are laboratory models of relapsing, erosive synovitis. A review of the experimental literature suggests that their pathogenesis is similar. The persistence in macrophages in the pannus of nonbiodegradable microbial cell wall components containing the peptidoglycan moeity is the central event. Based on this experimental literature, a model for the pathogenesis of these arthritides is developed. This model is testable and relevant to erosive synovitis in man.

Immune complexes in rheumatoid synovitis: a mixed staining immunofluorescence study.

Complexes of IgG—rheumatoid factor (RF) and IgG—β1C were demonstrated in the synovial membrane obtained from patients with rheumatoid arthritis (RA) by a mixed immunofluorescence method that shows simultaneously the two components of the complex. IgG—RF was found only in the synovia of cases with circulating RF, while IgG—β1C deposits could be detected in both seropositive and seronegative cases. These findings are discussed in connection with the pathogenesis of rheumatoid synovitis in the light of the immune complex pathogenetic mechanism.

LongTerm Outcomes of Suspected Transient Synovitis of the Hip in NonAspirated Patients.

BACKGROUND: Transient synovitis is a common cause of pediatric hip pain that must be distinguished from septic arthritis. Although the literature includes outcome data for aspirated hips, long-term outcomes in patients managed presumptively without aspiration remain limited. This study aimed to characterize the long-term clinical outcomes of children with presumed transient synovitis managed without hip aspiration. METHODS: A retrospective study of pediatric patients diagnosed with transient synovitis and managed presumptively without aspiration. Collected variables included demographics, cli…

Targeting synovitis by immunomodulation as a novel therapeutic approach for hemophilic arthropathy.

BACKGROUND: In severe hemophilia, recurrent hemarthroses lead to hemophilic arthropathy (HArt), characterized by chronic synovitis and cartilage destruction, reminiscent of rheumatoid arthritis. Unlike inflammatory synovitis, HArt lacks disease-modifying, joint-directed, targeted therapies. OBJECTIVE: To assess whether methotrexate (MTX), a reference treatment for inflammatory synovitis, can reduce joint damage when combined with extended half-life factor IX (EHL-FIX) prophylaxis in a murine model of hemophilia B. METHODS: Hemophilia B mice underwent 3 biweekly knee joint punctures to induce…

McMurray's test is influenced by perimeniscal synovitis in degenerative meniscus tears.

BACKGROUND: McMurray's test is a useful physical examination in determining meniscus tears, yet its sensitivity is only 38-62%. Furthermore, the relationship between degenerative meniscus tears (DMT) and mechanical symptoms during McMurray's test is not well defined. Perimeniscal synovitis occurs in osteoarthritic (OA) knees, inducing localized symptoms such as posterior knee pain in medial meniscus posterior horn DMTs. This study aimed to determine the relationship between McMurray's test with medial meniscus DMT and perimeniscal synovitis in patients with knee OA. METHODS: We retrospectivel…

PIGMENTED VILLONODULAR SYNOVITIS IN THE ANKLE OF A PEDIATRIC PATIENT: A CASE REPORT.

BACKGROUND: Pigmented villonodular synovitis (PVNS), also known as diffuse tenosynovial giant cell tumor, is a rare, benign but aggressive synovial proliferative disorder. It most commonly affects the knee; involvement of the foot and ankle is uncommon (incidence ~1.8 per million), and pediatric cases are especially rare. This rarity often leads to delayed diagnosis, as early symptoms can mimic benign causes (e.g. sprains or "growing pains") and initial radiographs may be unremarkable. CASE PRESENTATION: We report a case of a 12-year-old girl with a four-year history of chronic ankle pain and…

Foreign body-induced knee synovitis: importance of history by the radiologist.

We describe a case of a 7.5-year-old boy with a 7-week history of recurrent right knee swelling despite joint aspiration, persisting limping, and mild motion restriction, following a minor injury. Initial magnetic resonance imaging (MRI), ultrasound, and joint fluid analysis yielded non-specific synovitis. The child was investigated for juvenile idiopathic arthritis (JIA) and referred for a repeat MRI. Questioning by the attending radiologist disclosed falling onto wood sticks 10 weeks previously. Florid enhancing synovitis, prominent popliteal lymph nodes, and a linear foreign body were ident…

Molecular imaging for evaluation of synovitis associated with osteoarthritis: a narrative review.

Recent evidence highlights the role of low-grade synovial inflammation in the progression of osteoarthritis (OA). Inflamed synovium of OA joints detected by imaging modalities are associated with subsequent progression of OA. In this sense, detecting and quantifying synovitis of OA by imaging modalities may be valuable in predicting OA progressors as well as in improving our understanding of OA progression. Of the several imaging modalities, molecular imaging such as positron emission tomography (PET) and single-photon emission computed tomography (SPECT) has an advantage of visualizing the ce…

Pigmented Villonodular Synovitis of the Shoulder: A Case Report and Literature Review.

BACKGROUND Pigmented villonodular synovitis is a rare clinical entity, with 2-3% of all PVNS cases affecting the shoulder. Diagnosis is challenging and can elude clinicians for years, with definitive treatment involving arthroscopic or open synovectomy. CASE REPORT A 50-year-old woman presented with left shoulder pain persisting for 2 years. She was initially conservatively treated by a rheumatologist, with corticosteroid schemes intra-articularly injected and per os, but no improvement of her symptoms was noted. Two years later, she was referred to the Orthopedics Department of our hospital w…

Arthroscopic Management of Pigmented Villonodular Synovitis of the Hip: A Systematic Review.

Background/Objectives: Pigmented villonodular synovitis (PVNS) is a benign proliferation of synovial tissue that can cause joint damage. The hip, although less commonly affected than the knee, presents a challenging diagnosis and treatment, with magnetic resonance imaging (MRI) as the gold standard for detection. Surgical excision, arthroscopic or open, is the main treatment approach, but there is no consensus on the best strategy for the hip. The aim of this systematic review is to evaluate the clinical outcomes, complications, and revision rates associated with arthroscopic hip surgery for P…
