Index extensor digitorum communis tendon entrapment in a growth plate injury of distal radius.

BACKGROUND: We report a rare case in which an epiphyseal injury of the distal radius caused entrapment and rupture of the index extensor digitorum communis tendon during conservative treatment. CASE PRESENTATION: A 16-year-old girl suffered epiphyseal injury of the right distal radius with palmar displacement during playing judo. Closed reduction and application of a short arm cast were performed. 4 weeks after injury, movement of her index finger was limited. Extensor tendons adhesion at the fracture site was suspected. It was found that the index extensor digitorum communis tendon had becom

Murine supraspinatus tendon injury model to identify the cellular origins of rotator cuff healing.

UNLABELLED: Purpose of this study: To elucidate the origin of cell populations that contribute to rotator cuff healing, we developed a mouse surgical model where a full-thickness, central detachment is created in the supraspinatus. MATERIALS AND METHODS: Three different inducible Cre transgenic mice with Ai9-tdTomato reporter expression (PRG4-9, αSMA-9, and AGC-9) were used to label different cell populations in the shoulder. The defect was created surgically in the supraspinatus. The mice were injected with tamoxifen at surgery to label the cells and sacrificed at 1, 2, and 5 weeks postopera

Construct validity of the canadian occupational performance measure in participants with tendon injury and Dupuytren disease.

BACKGROUND: In patient-centered practice, instruments need to assess outcomes that are meaningful to patients with hand conditions. It is unclear which assessment tools address these subjective perspectives best. OBJECTIVE: The aim of this study was to establish the construct validity of the Canadian Occupational Performance Measure (COPM) in relation to the Disabilities of Arm, Shoulder, and Hand (DASH) questionnaire and the Michigan Hand Outcomes Questionnaire (MHQ) in people with hand conditions. It was hypothesized that COPM scores would correlate with DASH and MHQ total scores only to a

Outlet biceps tenodesis: a new technique for treatment of biceps long head tendon injury.

Degeneration and tearing of the long head of the biceps brachii tendon (LHBT) are common intra-articular findings, and surgical intervention including tenodesis or tenotomy is beneficial. A new arthroscopic shoulder technique may be performed through an anterior portal while one is viewing from a posterior portal: (1) Visualize the intra-articular biceps tendon. (2) Identify the segment of the LHBT to be enlarged. (3) Use a tissue modulation wand to enlarge the tendon. (4) Evaluate the diameter of the enlarged segment. It should be twice the original diameter. (5) Cut the biceps tendon at the

Tibialis anterior tendon entrapment after ankle tightrope insertion for acute syndesmosis injury.

Ankle syndesmotic injuries often require anatomic reduction and fixation. Syndesmosis screw fixation is commonly used; however, this prevents physiological motion at the joint, often requires a second operation for screw removal, and may limit postoperative weight bearing. The Ankle Tightrope (Arthrex, Naples, FL) was introduced to allow physiological tibiofibular micromotion, early ankle range of motion, and weight bearing, and did not require a second operation for removal. For Maisonneuve injuries, or more severe syndesmosis instability, a second tightrope is required. This is advised to be

Lateral talar process fracture and peroneal tendon dislocation: a previously unrecognized injury complex.

BACKGROUND: Lateral talar process fractures and peroneal tendon dislocations are frequently unrecognized at the time of injury. Lateral process fractures were initially classified by Hawkins as three types. Type II injuries are comminuted fractures involving both the talofibular and talocalcaneal articular surfaces. The purpose of this retrospective chart review was to describe an injury complex of Type II lateral talar process fracture with peroneal tendon dislocation. MATERIALS AND METHODS: Between January of 1995 and December 2006, 13 patients were seen for a lateral talar process fracture

Achilles tendon reconstruction after electrical injury with a neurotized cross-leg fillet flap.

The authors present a case where a cross-leg fillet flap from an amputated limb allowed reconstruction of the contralateral disrupted Achilles tendon and provided neurotized soft tissue. Achilles tendon reconstruction of the left leg was achieved by utilizing vascularized extensor tendons from the dorsum of the right foot based on the dorsalis pedis circulation. Neurotization was accomplished end to side between a cutaneous nerve from the dorsum of the foot and the recipient sural nerve. Healing was complete and without complication. Cross-leg flaps including tendon and nerve may be used to re

The significance of extensor tendon tethering and dorsal joint capsule tightening after injury to the hand.

This paper examines the clinical problem of extensor tendon tethering and/or dorsal joint capsule tightening secondary to hand injury. One hundred and forty-one patients were examined 13 to 51 months after hand injuries of varying severity. Fifty-six patients (40%) had suffered simple and eighty-five patients (60%) complex injuries. Seventy-four (52%) of the 141 patients had no extensor tendon tethering and/or dorsal joint capsule tightening. In 30 (21%), the extensor tendon tethering and/or dorsal joint capsule tightening was considered to be "obvious" in that it was easily seen on examinatio

Decrease in adhesion formation by a single application of 5-fluorouracil after flexor tendon injury.

Using an animal model, the effect of a single intraoperative application of 5-fluorouracil on digital flexor tendon adhesions was assessed. After a standard partial division of the tendon and immobilization with a stitch, the synovial sheath in 30 rabbit tendons was treated with 5-fluorouracil solution (50 mg/ml)-soaked sponge pledgets for 5 minutes. Buffered saline was substituted for 5-fluorouracil in 30 control tendons. The tendons were harvested 1 week postoperatively, and histologic sections were assessed with a light microscope. There was a significant reduction in synovial sheath thicke

A prospective evaluation of upper extremity tendon transfers in children with cervical spinal cord injury.

Three children (five hands) between 6 and 11 years of age with cervical level spinal cord injuries underwent tendon transfers to restore voluntary lateral pinch. Repeated measures of pinch force and the Grasp and Release Test (GRT) were obtained before surgery and at regular intervals after tendon transfers. The Functional Independence Measure (FIM) was administered before surgery and at 12 months after surgery. Responses to open-ended questions were used to supplement the FIM data at 1 year after surgery. Before surgery, no measurable force was obtained in any hand; after tendon transfers, pi

Tendon transfers and functional electrical stimulation for restoration of hand function in spinal cord injury.

Spinal cord injury at the C5 and C6 level results in loss of hand function. Electrical stimulation of paralyzed muscles is one approach that has demonstrated significant capacity for restoring grasp and release function. One potential limitation of this approach is that key muscles for stimulation may have lower motor neuron damage, rendering the muscles unexcitable. We have used surgical modification of the biomechanics of the hand to overcome this limitation. Tendon transfer of paralyzed but lower motor neuron intact muscles can compensate for potential function lost owing to muscles with lo

Fibroblast Activation Protein-Targeted PET/CT With Al18F-NODA-FAPI-04 for In Vivo Imaging of Tendon Healing in Rat Achilles Tendon Injury Models.

BACKGROUND: Fibroblast activation protein (FAP) has shown high expression in inflammatory responses and fibrosis. HYPOTHESIS: We speculated that FAP could serve as a diagnostic and monitoring target in the tendon healing process. STUDY DESIGN: Controlled laboratory study. METHODS: A total of 72 Sprague-Dawley rats were randomly divided into a tendon crush group and a half-partial tendon laceration group. Four rats in each group were injected with radiotracers weekly for 4 weeks after surgery, with aluminum fluoride-labeled 1,4,7-triazacyclononane-N,N',N″-triacetic acid-conjugated FAP inhibi

Testosterone Therapy and Tendon Injury: Clinical Evidence, Biological Mechanisms, and Sports Medicine Implications.

Testosterone therapy increases lean mass and can improve muscle strength in men with hypogonadism. This review examines the relationship between testosterone therapy, tendon adaptation, and tendon injury, including the possibility of a mismatch between changes in muscle strength and tendon capacity. This narrative review critically evaluates the clinical and experimental evidence relating testosterone exposure to tendon injury. The present review focuses specifically on tendon-related safety and is not intended to assess the overall benefit-risk profile of testosterone therapy. Single-cell transcriptomics reveals that tenocyte-derived MIF contributes to tendon injury pathogenesis.

Tendon injury is accompanied by inflammation and pathological tissue remodeling, yet the underlying cellular and molecular mechanisms remain incompletely understood. Reanalysis of single-cell transcriptomic data across tendon-healing time points revealed dynamic cellular heterogeneity, including eight transcriptionally distinct mesenchymal subpopulations and eight immune-cell clusters. CellChat predicted relatively strong outgoing signaling from mesenchymal populations and incoming signaling to myeloid populations during repair. MIF-associated communication between selected tenocyte and myeloi

Magnetic resonance imaging features of sacrotuberous ligament partial injury with normal hamstring tendons.

The sacrotuberous ligament has an oblique course, from a broad fan-like origin in the posterior superior iliac spine, transverse tubercles of the sacrum and the upper part of the coccyx, descending laterally to attach to the medial surface of the ischial tuberosity. It provides stability to the pelvic girdle, including the sacroiliac joint, counterbalancing its rotation and limiting its movement. Magnetic resonance imaging (MRI) can identify this ligament, depicting its origin, course and continuity with the conjoined biceps femoris and semitendinosus tendon. However, there is no previous repo

Aged tendons have impaired mechanosensitivity and lower thresholds for injury under dynamic compression.

Rotator cuff tendinopathy is highly prevalent in aging populations, yet the mechanisms leading to age-dependent tendon degeneration are not well understood. In addition to tensile loading, tendons are subjected to compressive forces at certain anatomical sites (e.g., Achilles, rotator cuff), where altered adaptive responses may contribute to degenerative remodeling. The objective of this study was to investigate age-related differences in tendon responses to dynamic compressive loading using an ex vivo model. Murine flexor tendon explants from young and aged animals were cultured in a biaxial

Split Latissimus Dorsi Tendon Transfer for External Rotation in Obstetrical Brachial Plexus Injury.

The intramuscular neurovascular branching pattern of the latissimus dorsi muscle provides 2 parts with independent neurovascular supply. The medial and lateral branches arising from the thoracodorsal pedicle run parallel to the muscle fibers and, therefore, 2 independent motor flaps can be harvested. We propose a modification to the latissimus dorsi tendon transfer for external rotation based on these 2 muscular parts. The medial (transverse) part is transferred to the rotator cuff insertion to dynamically enhance external rotation, and the lateral part acts as a stabilizer of the humeral head

Capitellum Fracture with Distal Triceps Tendon Avulsion Injury: A Rare Presentation with Literature Review.

INTRODUCTION: Capitellum fractures are rare, representing only 1% of all elbow injuries. McKee Type 4 capitellum fractures encompass osteochondral shearing fractures of the capitellum, which involve a significant lateral portion of the trochlear ridge and the trochlea. These fractures may be linked with soft tissue and bone injuries surrounding the elbow, such as distal triceps tendon avulsion (DTTA) injuries. To date, only three case reports in the English literature have documented the correlation between Type 4 capitellum and trochlear fractures with DTTA injuries. Orthopedic surgeons shoul

Examining prior injury relative to Achilles tendon ruptures in National Basketball Association players.

The purpose of this study is to examine Achilles Tendon (AT) rupture and prior injury in the National Basketball Association (NBA). Publicly available data was sourced via two online databases and filtered to identify those reported as having sustained AT rupture. A full injury history was then generated for those players. A total of 51 AT ruptures were identified. Thirty-five per cent (18) had previously missed at least one game for "Achilles tendinitis" or "Achilles tendon injury" on the side of the affected limb. A further 18% were noted to have suffered plantar fasciitis or foot inflammati
