Efficacy of acupuncture versus local methylprednisolone acetate injection in De Quervain's tenosynovitis: a randomized controlled trial.

There is no consensus on the management of De Quervain's tenosynovitis, but local corticosteroid injection is considered the mainstay of treatment. However, some patients are reluctant to take steroid injections. This study was performed to compare the efficacy of acupuncture versus corticosteroid injection for the treatment of this disease. Thirty patients were consequently treated in two groups. The acupuncture group received five acupuncture sessions of 30 minutes duration on classic points of LI-5, LU-7, and LU-9 and on ahshi points. The injection group received one methylprednisolone acet…

Predictors of Prolonged Hospitalization in Pyogenic Tenosynovitis of the Hand: A Retrospective Cohort Study Focused on Length of Stay.

Background and Objectives: Septic tenosynovitis of the hand remains a surgical emergency associated with significant morbidity and healthcare resource utilization. While prior studies have focused primarily on diagnostic features, microbiology, and functional outcomes, factors influencing prolonged hospitalization remain insufficiently characterized. Length of stay (LOS) represents a pragmatic, patient- and system-centered outcome that may reflect disease burden and treatment complexity. To identify clinical, demographic, and treatment-related factors associated with prolonged hospitalization …

Beyond Overuse: A Cross-Sectional Analysis of Insulin Resistance in De Quervain's and Stenosing Flexor Tenosynovitis.

INTRODUCTION: Insulin resistance (IR) or metabolic syndrome (MetS) may escalate the propensity for tenosynovitis from accrual of advanced glycation end products from chronic hyperglycaemia. This study characterised IR/MetS among patients with de Quervain's tenosynovitis (DQT) and trigger finger (TF) or stenosing flexor tenosynovitis in a clinical practice setting. MATERIALS AND METHODS: A prospective, non-interventional, cross-sectional study enrolled 118 patients with DQT or TF at a general orthopaedics-hand surgery practice. IR/MetS was defined using established criteria, including HbA1c le…

The Use of Platelet-Rich Plasma for De Quervain's Tenosynovitis: A Systematic Review With Limited Meta-Analysis.

INTRODUCTION: De Quervain's tenosynovitis (DQT) is a painful wrist condition that impairs daily activities. Corticosteroid (CS) injections are commonly used for symptom relief, but their effects may diminish over time. Platelet-rich plasma (PRP) has emerged as a regenerative alternative that promotes tissue healing rather than merely suppressing inflammation. This systematic review and limited meta-analysis compare the efficacy and safety of PRP versus CS injections in DQT. A limited meta-analysis was chosen due to the high heterogeneity between studies (I2 = 98.7%). METHODOLOGY: A comprehens…

Collagen-Povidone Injection as Treatment for Stenosing Tenosynovitis in Older Adults With Type 2 Diabetes Mellitus.

BACKGROUND: Diabetes, affecting 415 million adults globally, leads to collagen-related hand issues. Stenosing tenosynovitis, common in patients with diabetes, is often managed nonoperatively. Collagen-povidone, with safety demonstrated in rheumatoid arthritis, emerges as a potential intervention. METHODS: A randomized clinical trial conducted between January 2021 and January 2023 compared a povidone-collagen intervention with a placebo in adults aged 60 and older diagnosed with stenosing tenosynovitis. Participants were randomized into case (collagen-povidone) and control (physiological solut…

Fascial Lengthening at the Time of Surgical Release for de Quervain's Tenosynovitis to Prevent Postsurgical Tendon Subluxation.

BACKGROUND: De Quervain's tenosynovitis is a relatively common, painful condition. Although commonly managed nonoperatively, some patients have recalcitrant disease, necessitating surgical release. Traditional surgical release for de Quervain's tenosynovitis with simple retinacular release can leave patients susceptible to first dorsal extensor compartment tendon subluxation. We present a stair-step flap technique that releases the first dorsal compartment while simultaneously preventing postoperative tendon subluxation via fascial lengthening. METHODS: All patients over the past decade who u…

Genetic characterization of newly emerging avian reovirus variants in chickens with viral arthritis/tenosynovitis in Israel.

In recent years, new avian reovirus (ARV) variants caused a variety of symptoms in chickens worldwide, the most important of which was Viral arthritis/tenosynovitis which caused substantial economic losses and has become a concern to the worldwide chicken industry. In this study, we characterized emerging ARV variants in Israel and analyzed their genetic relationship with reference strains. One hundred thirty-four ARV variants were isolated from tendons and synovial fluids of commercial broiler chickens with signs of arthritis/tenosynovitis. Phylogenetic analysis of the partial segment of the …

Pyogenic Extensor Tenosynovitis of the Wrist-A Cause of Spontaneous Extensor Tendon Rupture: A Report of 2 Cases.

CASES: We present 2 cases of spontaneous extensor tendon rupture in the setting of pyogenic extensor tenosynovitis of the wrist caused by penetrating wounds-a cat bite and injection drug use in patients 1 and 2, respectively. Patient 1 underwent reconstruction of the extensor digitorum communis tendons with palmaris longus autograft. Patient 2 declined surgical reconstruction. CONCLUSIONS: These cases highlight a rare but severe complication of dorsal hand/wrist infection. Early surgical intervention should be considered to prevent this complication, which may have permanent functional conseq…

A Systematic Review of the Efficacy of Corticosteroid Injections of Tendon Sheaths, Excluding Stenosing Tenosynovitis of the Wrist and Hand.

OBJECTIVE: Corticosteroid injections into tendon sheaths are common in clinical practice for treatment of a variety of tendon disorders. Previous systematic reviews have been performed on injections for stenosing tenosynovitis (DeQuervain tenosynovitis and trigger finger), but little high-level evidence remains for other tendon sheath injections. This systematic review analyzes the available literature on improvements in pain and function after these injections. DESIGN: This systematic review of MEDLINE and Embase databases, as well as article bibliographies, examined studies of adult tendon …

Persistent Tenosynovitis, Steroid Dependency and a Hyperpigmented Scaly Macular Rash in a Child With Juvenile Idiopathic Arthritis.

Blau syndrome is a rare autoinflammatory disease, characterized by granulomatous symmetric arthritis, skin rash and uveitis. It is caused by mutations in the CARD15/NOD2 gene, which is a significant part of innate immunity. We describe the case of a patient with Blau syndrome, initially misdiagnosed as juvenile idiopathic arthritis. Genetic analysis showed R334Q mutation in the NOD2 gene that is known to be linked to Blau syndrome. Our patient was successfully treated with the IL-1β blocking agent canakinumab, with clinical and laboratory remission without any adverse effects. To our knowledge

Extensor digitorum longus tenosynovitis caused by talar head impingement in an ultramarathon runner: a case report.

Stenosing tenosynovitis of the extensor digitorum longus tendon is an injury related to ultramarathon running. A 32-year-old male ultramarathon runner developed chronic tenosynovitis of the ankle dorsiflexors. He was diagnosed with extensor digitorum longus tenosynovitis caused by talar head impingement associated with exostosis. He failed to respond to non-operative management and decided to undergo tenosynovectomy of the extensor digitorum longus tendon. The pain was relieved without functional disturbance of the foot and ankle, and the patient returned to running 3 weeks postoperatively. At…

Comparison of Outcomes in Surgical Treatment of Pyogenic Flexor Tenosynovitis: Hand Surgeons Versus Other Orthopedic Subspecialists.

BACKGROUND: Call patterns among hospital systems vary. At our institution, most pyogenic flexor tenosynovitis (FTS) patients receive irrigation and debridement (I&D) from non-hand-trained orthopedic specialists on call. Our hypothesis was that there would be no outcome differences among patients managed by hand surgeons versus non-hand surgeons. MATERIALS AND METHODS: This was a retrospective cohort study of patients who underwent I&D for FTS over a six-year period. Demographic data, mechanism of injury, past medical history, laboratory results indicating inflammation, time to the operating r…

Treatment of Chronic Extensor Carpi Ulnaris Stenosing Tenosynovitis Using Ultrasound-Guided Retinaculum Release: A Case Report.

CASE: A 65-year-old man with chronic extensor carpi ulnaris (ECU) stenosing tenosynovitis who had failed treatments for 3 years was successfully treated with an ultrasound-guided retinaculum release of the sixth dorsal compartment. CONCLUSION: There are limited options in the literature for treating chronic, recalcitrant ECU tenosynovitis. We describe a novel technique in which the retinaculum overlying the ECU tendon was successfully incised under ultrasound guidance to release the sixth dorsal compartment stenosis. There was no recurrence of symptoms in the following 2 years of follow-up.
