[Delirium in palliative care].

Delirium has an abrupt onset and represents an emergency. The symptoms of delirium include reduced consciousness, cognitive failure and altered psychomotor activity. In this paper, we present delirium in palliative care based on a literature review and clinical experience. Delirium is the second most common psychiatric condition in cancer patients in general. The exact prevalence in palliative care is unknown. Several factors can precipitate a delirium. The most common factors are probably opioids, dehydration and metabolic disturbances. The assessment must be balanced towards the patient's li

Evaluating the impact of a home palliative programme for terminally ill non-cancer patients with end-stage organ failure and severe frailty in Singapore.

OBJECTIVE: To evaluate Programme IMPACT, a home-based palliative care initiative for terminally ill non-cancer patients with end-stage organ failure and severe frailty at the end-of-life, in achieving palliative outcomes, honouring end-of-life care preferences, and reducing acute healthcare utilisation. METHODS: Patients with non-cancer diagnoses, prognosis of ≤ 6 months, palliative goals of care, and preference for home as a place of care or death were enrolled between October 2017 and December 2021. First, we employed a single-arm prospective cohort design to assess symptom control and conc

Palliative Sedation via Intraosseous Vascular Access: A Safe and Feasible Way to Obtain a Vascular Access End of Life.

Intraosseous (IO) access is normally reserved for emergencies and critical care conditions when venous cannulation is not possible. Nonetheless, we present a case of IO insertion to a 56-year-old man, tetraplegic for many years due to progressive spinal muscular atrophy and with refractory suffering. The IO access was used for palliative sedation with propofol in a home care setting. The patient died after 11 days of palliative care, of which the last 4 days were with palliative sedation using an IO cannula as a vascular access. No complications were noted from this route of administration. We

Differing management of people with advanced cancer and delirium by four sub-specialties.

INTRODUCTION: Delirium in advanced cancer is prevalent, with limited prospective data to guide management. The aim was to survey current practice of specialists using two contrasting cases of delirium in cancer. METHODS: A questionnaire was designed to identify investigations and treatment used, in relation to two cases. RESULTS: Overall response rate (n = 270) was 30%. Place of care: Only 35% of medical oncologists would consider care at home for a patient with reversible delirium compared with 66% of other specialists. INVESTIGATIONS: 85% specialists would order basic bloods, however, med

Experiences and perceptions of affected individuals, families, caregivers and healthcare professionals regarding end-of-life planning in Huntington's disease: A narrative synthesis.

IntroductionPeople with Huntington's disease (HD) are encouraged to consider end-of-life planning early on to mitigate future challenges. Initial evidence shows that undertaking planning discussions around this subject presents various barriers; however, a comprehensive evidence synthesis has not been completed to date. Accordingly, we here synthesise evidence regarding experiences of end-of-life planning discussions (EOLPD) among people with HD, their families and familial caregivers, and healthcare professionals supporting them.MethodsFive databases were systematically searched for peer-revi

"Scarcity," "Futility," and "Moral Distress": A Case Analysis of the Challenges of Sound Ethical Description.

Appropriate use of ethical terminology is critical to effective, goal-concordant care, particularly for patients who are critically ill or nearing the end of life. In this case analysis, we describe how the ethical terms "scarcity," "futility," and "moral distress" were used in ways that failed to accurately capture the clinical situation, and likely favored the viewpoints of the clinical team. While careful attention to accurate clinical and ethical terminology is challenging in cases like this, we argue it is essential to communicate honestly and effectively with patients and families grappl

Knowledge, attitudes, and representations of caregivers of individuals with major neurocognitive disorders regarding palliative and end-of-life care: a narrative literature review.

BACKGROUND: Caregivers of individuals with major neurocognitive disorders often bear the responsibility of participating in decisions related to the provision of palliative care for their loved ones due to the loss of capacity to consent as the disease progresses. This role is highly significant, as caregivers can influence both the quality of life and life expectancy of those they care for. Since they make decisions based on their knowledge, skills, preferences, and values, It is essential to develop a more comprehensive understanding of theses concepts regarding palliative and end-of-life ca

General practitioner care of residential aged care facility residents at end of life: a systematic literature review and narrative synthesis.

OBJECTIVES: In 2023, 21% of deaths occurred in residential aged care facilities (RACFs), a setting expected to play an increasing role in palliative and end-of-life care (PEoLC). General practitioners (GPs) oversee and deliver PEoLC in residential and nursing homes, yet little is known about their practice. We conducted a systematic review of the published evidence concerning how GPs provide this care: what they do and the quality, challenges and facilitators of that care. DESIGN: Systematic review and narrative synthesis using the Preferred Reporting Items for Systematic Reviews and Meta-Ana

[Translated article] Ethical approach to pain management in pediatric patients in a terminal situation: A systematic review.

OBJECTIVE: Pediatric palliative care aims to provide a professional, scientific, and compassionate response to the needs of terminally ill pediatric patients, with pain management being one of its fundamental pillars. This systematic review analyzes the ethical aspects involved in the management of pain in pediatric patients at the end of life, using the core principles of clinical bioethics: autonomy, beneficence, non-maleficence, and justice as a framework. MATERIALS AND METHODS: A systematic review was conducted following the guidelines of the PRISMA (Preferred Reporting Items for Systemat

[The healthcare professional's encounter with the seriously ill and dying patient in the hospital].

Seriously ill patients often fear not death but dying in pain and solitude. This review emphasises setting treatment ceilings and prioritising palliation over unnecessary interventions. Such discussions are best held in calm settings but can be challenging in acute situations. Effective care addresses the physical, psychological, social, and existential needs of individuals. Family members play a key role and require clear communication and empathy. Pharmacological relief primarily includes opioids and benzodiazepines, aiming to ensure a dignified end of life and support for loved ones.

End-of-Life Palliative Care: Role of the Family Physician.

To care for patients at the end of life, family physicians should be able to evaluate the causes of symptoms, differentiate between distressing symptoms and common end-of-life changes, and balance treatment effectiveness with potential adverse effects, while ensuring alignment with the patient's values and wishes. For severe pain and dyspnea, opioids are the mainstay of treatment. Palliation of pain with adjuvant medications and nonpharmacologic measures may delay or decrease the need for opioids. Nausea can be treated by reducing exacerbating factors and choosing agents that target the specif

Subcutaneous drainage of refractory lower limb oedema at the end of life.

Severe, refractory lower limb oedema can be a significant burden for patients with advanced metastatic malignancy. Despite this challenge, management strategies are often limited to analgesia, as well as pressure care and nursing care with positioning and dressings. The evidence for subcutaneous drainage in this setting largely consists of case reports, and it is infrequently practised in palliative care units (PCUs). In this case report, a patient with advanced metastatic ovarian cancer in her final weeks of life is discussed. Her severe lower limb oedema resulted in frequent pain and severe

Facilitators and Barriers in Palliative and End-of-Life Care in Prisons.

With an aging prison population facing various chronic diseases, the importance of palliative and end-of-life care (EOLC) in correctional facilities is heightened. This systematic review aimed to identify facilitators and barriers to palliative and EOLC in prisons. PubMed, Web of Science, Scopus, and Cochrane databases were searched for primary qualitative studies (January 2014 to December 2022) in English. Eligible studies focused on people in prison, health care professionals, administrators, and governors regarding palliative or EOLC provision within prison settings. Five studies were inclu

Factors Associated With the Place of Death in Huntington Disease: Analysis of Enroll-HD.

Background: Most people prefer to die at home. Hospice is the standard in end-of-life care for people with Huntington disease (HD), a neurodegenerative genetic disorder that affects people in middle adulthood. Yet, we have little knowledge regarding the place of death for people with HD. Therefore, the current state of knowledge limits HD clinicians' ability to conduct high-quality goals of care conversations. Objectives: We sought to determine the factors associated with the place of death in people with HD. Design: We obtained cross-sectional data from Enroll-HD and included participants wit

Experiences of Homeless Recipients of Social Model Hospice Care: A Photovoice Exploration.

Homelessness and caregiver insecurity are barriers to accessing hospice for end-of-life care. Some communities have implemented a community-based social model of hospice care, and reports of its characteristics and outcomes are growing in hospice and homeless literature. This case report explored the experiences of social model hospice recipients using photovoice, a community-based participatory method to photograph meaningful aspects of care. Participants (n = 3) took photos (n = 93), journaled, and participated in semistructured interviews as co-analyzers. Through deductive codes from the li
