Treatment of necrotizing soft tissue infections. The need for a new approach.

Necrotizing soft tissue infections are classified by the type of infecting organism, the presenting clinical picture, and the treatment required. However, reliance on this schema is impractical since it often leads to an inordinate delay in appropriate surgical therapy with an unwarranted loss of a limb or life. Since 1958, 21 patients were treated at the UCLA Medical Center with necrotizing soft tissue infections. Unifocal ulcerations and nonspreading infections were excluded. Of the 21 patients, the initial classification of the infections included necrotizing fasciitis in 38 percent, clostr

The role of intramedullary nailing in treatment of open fractures.

The management of open fractures remains one of the greatest challenges to orthopedic trauma surgeons. Damage to the soft tissue envelope together with periosteal stripping are the most important factors making open fractures prone to complications such as nonunion and infection. Urgent and thorough soft tissue debridement, proper surgical fracture stabilization as well as the administration of intravenous and local antibiotics as adjunctive therapy are mandatory to reduce the risk of infection. Intramedullary nail osteosynthesis has become an accepted treatment method of open long bone fractu

How to perform local soft-tissue glucocorticoid injections?

Inflammation of periarticular soft-tissue structures such as tendons, tendon sheaths, entheses, bursae, ligaments and fasciae is the hallmark of many inflammatory rheumatic diseases, but inflammation or rather irritation of these structures also occurs in the absence of an underlying rheumatic disease. In both these primary and secondary soft-tissue lesions, local glucocorticoid injection often is beneficial, although evidence in the literature is limited. This chapter reviews local injection therapy for these lesions and for nerve compression syndromes.

Compliance to antibiotic guidelines leads to more appropriate use of antibiotics in skin and soft tissue infections in injecting drug users.

Background: Knowledge about the treatment of skin and soft tissue infections in injecting drug users in countries with a low prevalence of antibiotic resistance is limited. We investigated bacterial antibiotic resistance and treatment of skin and soft tissue infections in Norwegian drug users. Methods: We performed a two year clinical cross-sectional observational study in a Norwegian hospital. Data were collected retrospectively from hospital records. We examined bacteriological findings and antibiotic resistance, and evaluated compliance to treatment guidelines and appropriateness of empiric

Mycobacterium haemophilum skin and soft tissue infection in a kidney transplant recipient: A case report and summary of the literature.

Non-tuberculous mycobacteria are ubiquitous pathogens causing infections in immunocompromised patients. Here, we describe a kidney transplant recipient who developed skin and soft tissue infection by Mycobacterium haemophilum, complicated by tenosynovitis and fluid collection, following an injury sustained to her right foot. Her immunosuppressant dose was reduced, and she underwent prolonged antimicrobial therapy followed by surgical debridement with a favorable outcome. Non-tuberculous mycobacteria should be considered as a potential etiology of subacute skin and soft tissue infections.

Surgical vacuum-assisted closure for treatment of dramatic case of stonefish envenomation.

Skin necrosis of the foot, oedema and lymphangitis from stonefish (Synanceia verrucosa) sting are complications well known for a long time and with potential long-term sequelae. Literature reports of stonefish envenomation give no specific reference on soft tissue management and surgical reconstruction. This is the first report describing a case of foot stonefish envenomation treated by vacuum-assisted closure therapy as an easy to use, accessible and simple adjuvant tool for management of large soft tissue necrosis.

Recent developments in salvage chemotherapy for patients with metastatic soft tissue sarcoma.

The number of effective cytotoxic agents for the treatment of patients with metastatic adult soft tissue sarcoma is limited, especially when patients have failed anthracycline- and ifosfamide-based chemotherapy. For the subgroup of patients with inoperable gastrointestinal stromal tumour (GIST), progress has been made via the rapid development and approval of the targeted therapy imatinib. Small round cell tumours (SRCTs), such as Ewing's sarcoma/primitive neuroectodermal tumour, desmoplastic SRCT and rhabdomyosarcoma, are chemotherapy-sensitive and potentially curable malignancies, which are

Treatment of large apical lesions with mucosal fenestration: a clinical study with long-term evaluation.

INTRODUCTION: Mucosal fenestration at the root apex may compromise the treatment results of periradicular surgery from exposing the surgical wound to the oral environment. The purpose of this study was to evaluate the long-term outcomes of periapical lesions with mucosal fenestrations treated by guided tissue regeneration (GTR) combined with the management of soft tissue defects. METHODS: Five patients with mucosal fenestration and large periapical lesions were treated by endodontic surgeries and periodontal regenerative procedures during 1999 to 2006. The barrier membranes and osseous grafts

Harvest graft substitute for soft tissue volume augmentation around existing implants: A randomized, controlled and blinded multicenter trial.

BACKGROUND: Using a single-blind, randomized, controlled, multicenter, practice-based clinical trial, a volume-stable collagen matrix (VCMX) was compared with connective tissue graft (CTG) for soft tissue augmentation around existing dental implants. METHODS: Sixty patients (31 VCMX and 31 CTG) were included. The primary outcome was a soft tissue thickness change 3 mm below the gingival margin (GM). Secondary outcomes included clinical measures, such as keratinized tissue widths (KTw), probing pocket depths, and pink esthetic scores, and patient-reported outcomes (PRO). RESULTS: There were n

Use of the cold test as a measure of pulpal anesthesia during endodontic therapy: a randomized, blinded, placebo-controlled clinical trial.

Soft tissue signs are inadequate to assess pulpal anesthesia during root canal therapy (RCT). The intention of this study was to determine if a negative response to cold testing is a more effective measure to assess pulpal anesthesia compared to soft tissues signs. Using a randomized, blinded, placebo-controlled design, subjects with baseline responsiveness to cold testing were enrolled. Test subjects achieved a negative response to cold testing prior to RCT. Control subjects were given a sham cold test after demonstrating soft tissue signs. Visual analog scale determined pain level. Unadjuste

The anterolateral thigh flap for complicated abdominal wall reconstruction after giant incisional hernia repair.

In the management of giant incisional hernias with loss of domain several surgical obstacles have to be addressed. Adequate coverage of the defect using mesh, sufficient local tissue advancement and prevention of wound and mesh infections are prerequisites for success. We present a case of a complicated giant incisional hernia repair after oncologic surgery, in which we chose for an intraabdominal mesh repair using a composite mesh. The patient developed a wound dehiscence and mesh infection, successfully treated with negative pressure therapy followed by a free ALT perforator flap. Several su

Human Extracellular Matrix-Like Collagen-Based Hydrogels for Soft Tissue Regeneration and Mandibular Retrognathia Treatment.

Skin aging, trauma, and congenital diseases lead to tissue defects and functional loss. Effective regeneration remains challenging due to limited material bioactivity and inadequate consideration of mechanical cues. This study develops a hydrogel integrating human extracellular matrix-like collagen (hCol) derived from induced human adipose mesenchymal stem cells (hASCs) with hyaluronic acid (HA) and polyethylene glycol diamine (PEGDA), engineered to achieve a modulus (≈1 kPa) representative of subcutaneous soft tissue. The hCol, produced at scale, provides essential biochemical signals, which,

EWSR1::ATF1 fusions characterize a group of extra-abdominal epithelioid and round cell mesenchymal neoplasms, phenotypically overlapping with sclerosing epithelioid fibrosarcomas, and intra-abdominal FET::CREB fusion neoplasms.

With the increasing use of next generation sequencing in soft tissue pathology, particularly in neoplasms not fitting any World Health Organization (WHO) category, the spectrum of EWSR1 fusion-associated soft tissue neoplasms has been expanding significantly. Although recurrent EWSR1::ATF1 fusions were initially limited to a triad of mesenchymal neoplasms including clear cell sarcoma of soft tissue, angiomatoid fibrous histiocytoma and malignant gastrointestinal neuroectodermal tumor (MGNET), this family has been expanding. We herein describe 4 unclassified extra-abdominal soft tissue (n = 3)

Masticatory function and three-dimensional facial morphology of soft tissues: One year after orthognathic surgery.

OBJECTIVE: Dentofacial deformities (DFD) require orthodontic treatment, orthognathic surgery, and speech therapy for aesthetic and functional problems. This longitudinal study analyzed changes in masticatory function and three-dimensional (3D) facial soft tissue in patients with Class II and Class III DFD after orthognathic surgery. In addition, the study investigated the relationship between facial measurements, maximum bite force (MBF), and orofacial myofunctional status (OMS). DESIGN: The sample consisted of 46 participants, including 10 patients with Class II DFD, and 11 patients with Cla

Soft tissue sarcoma at a dialysis access site in a transplant recipient.

Soft tissue sarcomas typically present as soft, painless masses on an extremity. Here, we present a patient with metastatic soft tissue sarcomas at his dialysis access site. This association with dialysis access has not been documented previously. A 62-year-old man presented with a nonhealing wound on his left upper extremity after excision of a pseudoaneurysmal arteriovenous fistula. The patient had received a second kidney transplant that was functioning well. Immunosuppression included tacrolimus, mycophenolate mofetil, and prednisone. He was induced with thymoglobulin twice. A biopsy was p

Current status and perspectives of mucogingival soft tissue measurement methods.

UNLABELLED: The morphologic and metric assessment of mucogingival soft tissue dimensions is of great multidisciplinary clinical and academic interest, in order to quantify and monitor gingival changes while in treatment, e.g., during periodontal, restorative, prosthetic, orthodontic, or implant therapy. Pink esthetics play an increasingly important role in the overall treatment success, and therefore have to be monitored throughout therapy. The purpose of this article was to identify and summarize methods, which aim at quantifying gingival dimensions in terms of morphology, thickness, and volu

Cutaneous and subcutaneous complications of calcium infusions.

Five infants with hypocalcemia experienced complications after treatment with calcium gluconate intravenously. Inadvertent soft tissue extravasation resulted in erythema, subcutaneous calcification, tissue necrosis, skin slough, and transient radial nerve damage with wrist drop, the latter previously unreported. The soft tissue lesions may be mistaken for cellulitis, abscess, calcified hematoma, or osteomyelitis, resulting in unnecessary antibiotic therapy or surgical intervention. Initially, no clinical abnormality may be apparent. The lesions appear from days to weeks following extravasation

Management considerations of ameloblastic carcinoma in soft-tissue: case report and literature review.

BACKGROUND: Ameloblastoma is a benign yet locally aggressive odontogenic neoplasm characterized by a significant propensity for local recurrence. Soft-tissue recurrence following mandibular resection and reconstruction represents a distinct clinical entity, often posing diagnostic and therapeutic challenges. In contrast, ameloblastic carcinoma may arise de novo or through malignant transformation of a pre-existing ameloblastoma, reflecting a spectrum of biological behaviour. We report a case of post-resection soft-tissue recurrence with subsequent malignant transformation, underscoring the unp
