The study aimed to describe a novel port design and simple surgical strategy for uniportal thoracoscopic right middle lobectomy. In this approach, a 3- to 4-cm incision was created at the sixth intercostal space posterior to the scapular line. The surgery was performed by serial division of the anterior oblique fissure, vein, bronchus, artery, and horizontal fissure. Based on our preliminary experience, this approach could provide an appropriate direction and angle for dissection and stapling, solving the challenge of conventional uniportal right middle lobectomy.

OBJECTIVES: Minimally invasive thoracic procedures have been increasingly used; however, only a small number of lobectomies are performed by videothoracoscopy, showing the need for training on this technique. The objective of this study is to demonstrate the technique of lobectomy using videothoracoscopy in swine, highlighting the steps to be taken during its use in surgical experimentation.
METHODS: During the advanced course on video-assisted and thoracoscopic procedures carried out at IRCAD Latin America, Barretos, Brazil, 40 swine were used for the hands-on course on video-assisted upper

Myasthenia gravis is an autoimmune disorder of neuromuscular transmission affecting 2 out of every 100,000 people. Neurologists and surgeons still debate what role surgery should play in its management. Many patients who might benefit from thymectomy are denied the opportunity because of misconceptions, ignorance, or trepidation. By offering effective methods of less invasive thymectomy to these patients, a significant number of patients and treating neurologists previously unwilling to consider surgery may realize the benefits of this established, proven treatment alternative. The surgical ap

Pulmonary hydatid cysts (PHCs), caused by Echinococcus granulosus, remain endemic in livestock-raising regions worldwide. Surgical removal of cysts is the mainstay of treatment, with the aim of eliminating the parasite while preserving the lung parenchyma and minimizing postoperative complications and recurrences. This article synthesizes the latest evidence on the optimal surgical management of PHCs, emphasizing practical recommendations for operating surgeons based on current best practices and the highest level of available evidence.

Pleural tuberculosis (PLTB) is a significant form of extrapulmonary tuberculosis. It contributes substantially to exudative pleural effusions in high-burden regions and it often appears as postprimary disease in low-incidence areas. Immunocompromised patients, particularly those with Human immunodeficiency virus, exhibit higher mycobacterial loads and altered diagnostic performance. Management combines standard antituberculosis therapy with procedural or surgical interventions for complicated cases. Timely therapy ensures favorable outcomes, but residual pleural thickening or fibrothorax may p

Metastasis to the pleural and peritoneal cavities is a common terminal pathway for a wide variety of cancers. This article explores how these unique environments both promote aggressive tumor behavior and suppresses anti-tumor immunity, and ways in which local delivery of protein therapeutics can leverage the contained nature of these spaces to a therapeutic advantage, achieving high intra-cavital concentrations while minimizing systemic toxicity.

BACKGROUND: Pleural biopsy using either video-assisted thoracoscopic surgery or medical pleuroscopy is the current diagnostic criterion standard for pleural pathology with a high, yet imperfect, diagnostic yield. Cryobiopsy may provide greater tissue, increase depth of sampled tissue, and/or reduce crush artifact. However, its impact on diagnostic yield remains uncertain, and there are potential concerns regarding its safety too. We performed a systematic review and meta-analysis to investigate the same.
METHODS: We performed a systematic search of MEDLINE, Embase, and Google Scholar for stud

In recent years, robot-assisted thoracic surgery is gaining more and widespread interest in Europe. Due to the narrow space and the complexity of anatomical structures, conventional minimally invasive mediastinal surgery may be challenging for the thoracic surgeon. Robot-assisted mediastinal surgery opens up new possibilities for minimally invasive surgery, as it permits greater dexterity, a three-dimensional view, and tremor adjustment, which allows the surgeon to perform complex procedures in small thoracic spaces. As robotic platforms continue to evolve, more complex mediastinal thoracic su

BACKGROUND: Chest surgery is associated with significant pain, and potent opioid medications are the primary medications used for pain relief. Opioid-free anaesthesia (OFA) combined with regional anaesthesia is promoted as an alternative in patients with an opioid contraindication.
METHODS: Objective: To assess the efficacy of OFA combined with a paravertebral block in pain treatment during video-assisted thoracic surgery.
DESIGN: A randomized, open-label study.
SETTING: A single university hospital between December 2015 and March 2018.
PARTICIPANTS: Sixty-six patients scheduled for electi

The surgical approach to affections of the chest wall and pleura, still the predominant indications for thoracic surgery, has greatly changed since the advent of thoracoscopic procedures, and is emphasized in this second part of a two-part review, together with other indications for mediastinal tumours. Indicated after lung exeresis or emergency chest surgery, protective chest wall reconstruction with muscular flaps is no longer an exceptional operation. Inversely, thoracic surgery for infectious complications have become less frequent, unusually limited to well established procedures for tube

We report the use of video-assisted thoracic surgery to plicate the diaphragm after phrenic nerve injury associated with an operation for congenital heart disease. Right diaphragm paresis developed in a cyanotic newborn girl with pulmonary atresia and intact ventricular septum after a right modified Blalock-Taussig shunt. Diaphragm plication was performed endoscopically and the patient recovered. Refinement of technique and instrumentation may allow wider application of video-assisted thoracoscopic plication of the diaphragm in neonatal and pediatric patients.

INTRODUCTION: Robot-assisted surgery emerged and evolved in order to increase the surgical precision and due to the need to overcome the drawbacks of conventional minimally invasive surgery. In thoracic surgery the first reported use of a robotic device was in a series of 12 patients with different lung pathologies with the assistance of the DaVinci Robotic Surgical System in 2002. The DaVinci system has been used for various procedures in the field of thoracic surgery since then. While its advantages for the resection of early stage thymoma have been well documented, its role in the treatment

BACKGROUND: We compared the clinical outcomes and cost-efficiency of surgical approaches (sternotomy-open, video assisted thoracoscopic surgery, and robotic assisted thoracic surgery) for thymectomy.
METHODS: This is a retrospective review of 220 consecutive patients who underwent thymectomy between January 1, 2007, and January 31, 2017. Surgical approach was determined by the surgeon, but we only included cases that could be resected using any of the 3 approaches.
RESULTS: Open approach was used in 69 patients, whereas minimally invasive technique was used in 151 (97, video assisted thoraco

STUDY OBJECTIVE: To demonstrate the advantages of a combined robotic-assisted laparoscopic and thoracic approach in the management of extensive diaphragmatic, pleural, and pericardial endometriosis.
DESIGN: A video article demonstrating excision of endometriosis from pericardium, diaphragm, and pleura.
SETTING: Thoracic endometriosis is the most common site of extrapelvic endometriosis [1]. Surgical treatment aims to excise all visible disease to relief symptoms and prevent recurrence [2-4].
INTERVENTIONS: A 41-year-old lady with cyclical shoulder tip and chest pain and known extensive diap
