BACKGROUND: Tissue expanders are used in breast reconstruction after mastectomy to stretch the remaining tissue to create space for placement of permanent breast implants. The AeroForm™ Tissue Expander, developed by AirXpanders™ Inc., contains electronic components designed to activate the release of carbon dioxide from an internal reservoir to inflate the expander. Breast cancer patients who undergo mastectomy and tissue expander/implant-based breast reconstruction may require radiation therapy at doses up to 50-60 Gy while the expander is in place. The ionizing radiation used in postmastecto

OBJECTIVE: To explore the possibility of para-orbital soft-tissue expansion, before orbital osteotomy and medial translocation procedures with a combined intracranial-extracranial approach.
METHODS: Tissue expansion in the region of the zygomatic and temporal has been undergone for 3 weeks before the traditional intracranial-extracranial approach for orbital osteotomy and medial translocation in two patients. The healing between the orbital bone was studied with measurement of interorbital distance and three-dimensional CT.
RESULTS: The inter-orbital distance of the two patients decreased fr…

BACKGROUND: The rising popularity of prepectoral tissue expander placement with acellular dermal matrices in immediate breast reconstruction has prompted many studies on the safety of this technique. However, a comprehensive propensity-matched, historically controlled trial comparing perioperative outcomes following prepectoral versus partial subpectoral (dual-plane) placement of tissue expanders is lacking.
METHODS: Retrospective propensity-matched cohort analysis was performed on all patients of two senior reconstructive surgeons who underwent bilateral tissue expander placement following a

BACKGROUND: Capsular contracture is a well-recognized complication following prosthetic breast reconstruction. It has been the authors' observation that some patients undergoing breast reconstruction experience contracture specifically of the acellular dermal matrix placed at the time of their tissue expander insertion. The goal of the authors' study was to identify clinical and histologic findings associated with the development of acellular dermal matrix-associated contracture.
METHODS: The authors performed a retrospective cohort study of all patients undergoing bilateral implant-based bre

The purpose of this study is to describe our experience using expandable spherical hydrogel implants and injectable hydrogel pellets for orbital volume augmentation in cases of post-enucleation socket syndrome after acquired anophthalmia or phthisis bulbi. We retrospectively reviewed the clinical records of all adult patients who received an expandable hydrogel implant for orbital volume loss following enucleation or phthisis bulbi at the Emory Eye Center between 2004 and January 2007 and the Yale Eye Center between 2009 and 2011. The study included 9 women and 5 men with a mean age of 51.2 ye

BACKGROUND: Two-stage tissue expander/implant breast reconstruction has become increasingly popular, but reports have varied widely in the incidence of complications associated with this approach. A large consecutive patient experience using low-height tissue expanders in immediate breast reconstruction is presented and discussed in conjunction with other recent reports using this approach following mastectomy.
METHODS: A total of 172 consecutive patients underwent immediate breast reconstruction following mastectomy, with placement of a total of 246 tissue expanders. All expanders used were

Adipose tissue (AT) is comprised of a diverse number of cell types, including adipocytes, stromal cells, endothelial cells, and infiltrating leukocytes. Adipose stromal cells (ASCs) are a mixed population containing adipose progenitor cells (APCs) as well as fibro-inflammatory precursors and cells supporting the vasculature. There is growing evidence that the ability of ASCs to renew and undergo adipogenesis into new, healthy adipocytes is a hallmark of healthy fat, preventing disease-inducing adipocyte hypertrophy and the spillover of lipids into other organs, such as the liver and muscles. Scar and defect in the lower face always lead to serious facial deformity and asymmetry. Conventional method such as skin graft or free flap is hard to obtain a satisfactory outcome. In this article, we present a skin expanding technique. An expanded flap was designed to repair the lower face scar or defect on submental and submandibular regions. A tissue expander was implanted into the region and inflated for 2 to 3 months. After removing the expander, we harvested the flap as a rotation flap to repair the lower facial defect. The longest follow-up period was 3 years. All of the flaps were we

BACKGROUND: Secondary facial defects remain a primary focus in plastic surgery, particularly large defects involving the midface, forehead, and scalp. While tissue expansion of the fronto-scalp flap offers a viable reconstructive option, the lengthy expansion period and the issue of post-expansion flap retraction necessitate further solutions.
OBJECTIVE: We conducted a prospective, comparative clinical study to investigate the role of Botulinum toxin in the expansion of fronto-scalp flaps.
METHOD: Between December 2019 and December 2024, 40 patients with large upper facial or scalp lesions w…

BACKGROUND: Implant-based reconstruction represents the most common method of breast reconstruction today. Many surgeons approach implant-based breast reconstruction in 2 stages, with the first stage being placement of a tissue expander at the time of mastectomy. Neoadjuvant chemotherapy can have systemic effects that may place patients at risk for TE/implant loss, infection, wound, and other complications.
METHODS: A retrospective review of all cases of TE breast reconstruction at a single institution between 2017 and 2024 was performed. A total of 210 patients, representing 319 total breast

BACKGROUND: Ideal timing for autologous reconstruction remains debated. The authors developed a novel flap after short tissue expansion (FAST) protocol that combines the benefits of both immediate and staged reconstruction by involving a very short tissue expander phase to delay the mastectomy pocket while still achieving expedient definitive reconstruction. The authors investigated outcomes of the FAST protocol in comparison with immediate, delayed, and more traditional staged autologous reconstruction, with a longer period between expander and flap reconstruction.
METHODS: A retrospective i…

BACKGROUND: The majority of postmastectomy breast reconstructions (PMBRs) are currently performed in two stages using a tissue expander (TE). However, complications during the expansion phase occur regularly, leading to unplanned reoperations and/or reconstruction failure. This study aimed to identify risk factors for unplanned reoperation after TE placement, assessed the time until unplanned and planned reoperation, and investigated indications for unplanned reoperation.
METHODS: Patient- and surgery-related characteristics of patients who underwent two-stage PMBR between 2017 and 2021 were

PURPOSE: To evaluate the anatomical effects and tissue biocompatibility in a feline model of an integrated orbital tissue expander (OTE) designed to stimulate bone growth in an anophthalmic socket.
DESIGN: An animal study was performed in cats to assess orbital bone growth with and without an OTE.
METHODS: The OTE is an inflatable (0.5 to >6.0 cm(3)) polymeric globe sliding on a titanium T plate secured to the lateral orbital rim with screws. Eight cats had left eye enucleation at age two weeks, with five orbits receiving an OTE and the remaining three serving as nonimplanted controls. Seria

An expander capsule is a fibrous membrane that forms around an expander after tissue expansion. However, whether the capsule should be removed after the expander is removed remains controversial. The authors aimed to investigate the efficacy and safety of cervical capsulectomy for improving neck mobility and appearance after expanded flap transfer. We retrospectively analyzed patients with cervical scar contractures who underwent expanded flap transfer at our hospital between September 2018 and December 2023. Depending on whether capsulectomy was performed simultaneously with expander removal,…

BACKGROUND: Infection is a common complication following tissue expander (TE)-based breast reconstruction. Few studies have examined risk factors specifically in the unique populations encountered at safety-net hospitals. The purpose of this study was to identify predictors of TE infection at Harris Health safety-net hospitals, which serve the third most populous county in the United States.
METHODS: A retrospective review was performed to evaluate women over the age of 18 years who underwent TE placement at two safety-net hospitals from October 2015 to November 2022. Demographic information,

BACKGROUND: The use of postmastectomy radiation therapy (PMRT) in the setting of immediate 2-stage breast reconstruction is becoming increasingly widespread. However, the timeframe of tissue expander exchange for permanent implant placement following PMRT is not well defined, and it remains unclear what time interval optimizes surgical outcomes and patient satisfaction.
METHODS: A systematic review conducted in accordance with Preferred Reporting Items for Systematic Reviews and Meta-Analyses 2020 was completed. The PubMed, Embase, Scopus, and Cochrane databases were searched under keywords p

BACKGROUND: Large congenital melanocytic nevi entail significant medical and cosmetic ramifications for patients and families. Reconstruction is a challenging endeavor, especially when the lesion is present on the limbs. The literature describes various methods by which a plastic surgeon can address reconstruction; yet, to date, there has been no series describing a method that provides consistent results with low complication rates. In this study, we describe our surgical technique for reconstruction of the upper extremity after excision of large circumferential CNM with a pre-expanded bi-ped
