[Vasopressin receptor antagonists and heart failure].

Vasopressin plays a physiological role in regulation of blood pressure, fluid volume, and serum osmolality. In heart failure inadequate release of vasopressin may result in excess fluid retention and hyponatremia. Vasopressin receptor antagonists are a new class of orally active drugs targeted to inhibit one or more of three distinct vasopressin receptors, namely V1a- (-->vasoconstriction), V1b- (-->release of ACTH) und V2-receptors (-->inhibition of free water reabsorption in the kidney). In cardiac decompensation with fluid overload selective V2- (Lixivaptan, satavaptan and tolvaptan) and no

Effects of Long-term Blockade of Vasopressin Receptor Types 1a and 2 on Cardiac and Renal Damage in a Rat Model of Hypertensive Heart Failure.

The effects of chronic blockade of vasopressin type 1a receptors (V1aR) and the additive effects of a type 2 receptor (V2R) antagonist on the treatment of hypertension-induced heart failure and renal injury remain to be unknown. In this study, Dahl salt-sensitive hypertensive rats were chronically treated with a vehicle (CONT), a V1aR antagonist (OPC21268; OPC), a V2R antagonist (tolvaptan; TOLV), or a combination of OPC21268 and tolvaptan (OPC/TOLV) from the pre-hypertrophic stage (6 weeks). No treatment altered blood pressure during the study. Significant improvements were seen in median sur

Pharmacological management of polycystic kidney disease.

INTRODUCTION: Autosomal-dominant polycystic kidney disease (ADPKD) represents a therapeutic challenge as effective treatment to retard the growth of cysts in the kidneys and the liver has not been available despite decades of intense basic and clinical research. AREAS COVERED: Several clinical trials have been performed in recent years to study the effect of diverse drugs on the growth of renal and hepatic cysts, and on functional deterioration of the glomerular filtration rate. The drug classes that have been tested in randomized clinical trials include the mammalian target of rapamycin (mTO

Factors predicting postoperative hyponatremia and efficacy of hyponatremia management strategies after more than 1000 pituitary operations.

OBJECT: Syndrome of inappropriate antidiuretic hormone secretion-induced hyponatremia is a common morbidity after pituitary surgery that can be profoundly symptomatic and cause costly readmissions. The authors calculated the frequency of postoperative hyponatremia after 1045 consecutive operations and determined the efficacy of interventions correcting hyponatremia. METHODS: The authors performed a retrospective review of 1045 consecutive pituitary surgeries in the first 946 patients treated since forming a dedicated pituitary center 5 years ago. Patients underwent preoperative and daily inpa

Syndrome of inappropriate antidiuretic hormone secretion following immune effector cell-associated neurotoxicity syndrome during blinatumomab therapy: a case report.

BACKGROUND: Blinatumomab (BLINA) causes immune-related toxicities, such as cytokine release syndrome and immune effector cell-associated neurotoxicity syndrome (ICANS). Although hyponatremia associated with interleukin (IL)-6-mediated vasopressin secretion has been reported with chimeric antigen receptor T-cell therapy, similar reports with bispecific antibodies are scarce. CASE PRESENTATION: A woman in her 60s with B-cell acute lymphoblastic leukemia developed grade 3 ICANS on day 3 of the second BLINA cycle. On day 13, the patient experienced grade 4 hyponatremia (serum sodium, 106 mmol/L).

Autosomal dominant polycystic kidney disease: an overview of recent genetic and clinical advances.

Autosomal Dominant Polycystic Kidney Disease (ADPKD) is the most common inherited kidney disease, characterized by the progressive development of multiple kidney cysts, leading to a gradual decline in kidney function. ADPKD is also the fourth leading cause of kidney failure (KF) in adults. In addition to kidney manifestations, ADPKD is associated with various extrarenal features, including liver cysts, cardiovascular abnormalities, intracranial aneurysms, and chronic pain with significant impact on patients' quality of life. While several disease-modifying agents have been tested in ADPKD, tol

Case Report: One case of pregnancy complicated by large cell neuroendocrine carcinoma of the cervix with syndrome of inappropriate secretion of antidiuretic hormone.

This article reports an extremely rare case of a 31-year-old pregnant woman diagnosed with large cell neuroendocrine carcinoma (LCNEC) of the cervix complicated by syndrome of inappropriate antidiuretic hormone secretion (SIADH). Admitted at 35+¹ weeks gestation due to vaginal bleeding, she was diagnosed with cervical LCNEC and pelvic lymph node metastasis. Following cesarean delivery, she developed severe hyponatremia (as low as 92 mmol/L) leading to coma during chemotherapy, meeting the criteria for SIADH. The hyponatremia was successfully corrected with the selective vasopressin V2 receptor

[What is evidence-based in the treatment of autosomal dominant polycystic kidney disease?].

The cystic transformation of the kidneys and liver are the most common symptoms of autosomal dominant polycystic kidney disease (prevalence 1:400-1:1000). A set of other manifestations can be observed less frequently, such as intracranial aneurysms. End-stage renal disease affects 50% of patients by the age of 70 years. To date, a targeted treatment is only available for patients at risk of rapidly progressive kidney failure. In 2015, the vasopressin receptor antagonist tolvaptan was approved in Germany for slowing down the decline of renal function in autosomal dominant polycystic kidney dise

New therapies for the treatment of heart failure: a summary of recent accomplishments.

Despite continuous efforts to prevent cardiovascular diseases (CVDs), heart failure prevails as the number one cause of death in developed countries. To properly treat CVDs, scientists had to take a closer look at the factors that contribute to their pathogenesis and either modernize current pharmaceuticals or develop brand new treatments. Enhancement of current drugs, such as tolvaptan and omecamtiv mecarbil, sheds new light on already-known therapies. Tolvaptan, a vasopressin antagonist, could be adopted in heart failure therapy as it reduces pre- and afterload by decreasing systolic blood p

Exploring urinary biomarkers in autosomal dominant polycystic kidney disease.

BACKGROUND: Autosomal dominant polycystic kidney disease (ADPKD), the most common inherited kidney disease, is a progressive disease characterized by a bilateral proliferation and enlargement of renal cysts. Recent reports have shown that tolvaptan, a vasopressin V2 receptor antagonist, has been effective in inhibiting renal cyst proliferation and enlargement in ADPKD patients, although no biomarker has identified to predict the effects of tolvaptan. We explored the effective urinary biomarkers in ADPKD in human and in an animal model. METHODS: We measured 28 biomarkers in urine taken from AD

Diuretic therapy for critically ill patients: a systematic review and network meta-analysis.

BACKGROUND: Fluid overload is common in critically ill patients and is associated with worse outcomes. Diuretics are the mainstay of active fluid removal in patients with preserved renal function. The optimal diuretic strategy for fluid removal remains uncertain. METHODS: We conducted a systematic review and Bayesian random-effects network meta-analysis of randomized controlled trials comparing two or more diuretic strategies for fluid removal in critically ill adults. We searched MEDLINE, Embase, and the Cochrane Central Register of Controlled Trials via Ovid, as well as trial registries, fr

Targeted analysis of salicylate binding partners in renal epithelial cells reveals major AMPK dependency.

Autosomal dominant polycystic kidney disease (ADPKD) is the most prevalent genetic kidney disease and is characterized by the formation and growth of fluid-filled renal cysts. With the current treatment tolvaptan not suited for the majority of patients, an urgent need remains for novel therapeutics. We have shown before that activation of adenosine monophosphate activated protein kinase (AMPK) with salsalate ameliorates disease progression in vivo to a similar extent as tolvaptan. Salsalate achieves this by affecting multiple disease mechanisms known to be involved in ADPKD pathogenesis, such

MEDA-PKD: a dynamic, real-time analytics platform for precision care in autosomal dominant polycystic kidney disease.

The use of medical real-world data is constrained by fragmented data infrastructures and the absence of tools integrating longitudinal clinical with molecular data at scale. Autosomal dominant polycystic kidney disease (ADPKD), the most common monogenic cause of kidney failure, exemplifies this gap given its phenotypic complexity and the need for individualized treatment decisions. We present MEDA-PKD (MEdical DAta translation service of Polycystic Kidney Disease), an interactive, near-real-time analytics platform that integrates clinical routine with research data from the German AD(H)PKD Tol

Urinary extracellular vesicles as reservoirs of altered proteins during the pathogenesis of polycystic kidney disease.

PURPOSE: Recent findings indicate that urinary extracellular vesicles (EVs) might reflect the pathophysiological state of urinary system; and that EVs-induced ciliary signaling is a possible mechanism of intercellular communication within the tract. Here, we aimed to analyze the protein expression of urinary EVs during autosomal dominant polycystic kidney disease (ADPKD). EXPERIMENTAL DESIGN: EVs were isolated from pooled urine samples of healthy control and ADPKD patients at two different stages of the disease and under tolvaptan treatment using the double-cushion ultracentrifugation method.

Autosomal dominant polycystic kidney disease.

Autosomal dominant polycystic kidney disease (ADPKD) is the most common genetic cause of chronic kidney disease, resulting in substantial morbidity and mortality globally. Advances in molecular genetics and deep-phenotype imaging techniques have refined existing diagnostic and prognostic tools. The strong evidence-base for tolvaptan as a disease-modifying treatment supports its early use in groups at high risk of kidney failure. Screening and management of potentially serious complications including cyst infection, intracranial aneurysms, and polycystic liver disease are important components o

Can MQ232 guide to novel options for treatment of ADPKD?

Wang et al. showcase a green mamba-derived compound, MQ232, which interferes with cyst growth in a mouse model of autosomal dominant polycystic kidney disease, similar to the only approved drug for autosomal dominant polycystic kidney disease treatment, tolvaptan. However, the authors reveal cyclic adenosine monophosphate-dependent and -independent components in MQ232's mode of action and gender differences in efficacy. Therefore, MQ232 may pave the way to novel treatment options for autosomal dominant polycystic kidney disease.

Therapeutic opportunities in polycystic kidney and liver disease through extracellular matrix dynamics.

Autosomal Dominant and Autosomal Recessive Polycystic Kidney Disease (ADPKD and ARPKD) are, respectively, common and rare forms of polycystic disorders, characterized by the formation and progressive growth of cysts from tubules in the kidneys and bile ducts in the liver. Alterations in the extracellular matrix (ECM) and in the activity of matrix metalloproteases (MMPs), both associated with fibrosis, have been shown to be important factors in cystic growth and progression of these diseases. We used tandem mass spectrometry (LC-MS/MS) to identify the most enriched proteins and pathways in an o

The inhibition of MDM2 slows cell proliferation and activates apoptosis in ADPKD cell lines.

INTRODUCTION: Autosomal dominant polycystic kidney disease (ADPKD) is characterised by progressive cysts formation and renal enlargement that in most of cases leads to end stage of renal disease (ESRD). This pathology is caused by mutations of either PKD1 or PKD2 genes that encode for polycystin-1 (PC1) and polycystin-2 (PC2), respectively. These proteins function as receptor-channel complex able to regulate calcium homeostasis. PKD1/2 loss of function impairs different signalling pathways including cAMP and mTOR that are considered therapeutic targets for this disease. In fact, Tolvaptan, a v
