Based on a series of 120 patients treated by trans-maxillary buccopharyngectomy for tumors (mainly T3, T4) of the tonsillar region, results are compared of primary and recovery surgery, the latter being instituted from 1981 because of the rapid strides made in new reconstructive procedures. This development markedly modified therapeutic indications, with substitution of radiotherapy as primary treatment. After primary surgery, the 3- and 5-year survival rates were globally 59 and 49% respectively. The usual prognostic factors emphasized, size of tumor and glandular involvement, were verified.

Kaposiform hemangioendothelioma (KHE) is an uncommon vascular tumor of intermediate malignant potential, usually occurs in the extremities and retroperitoneum of infants and is characterized by its association with lymphangiomatosis and Kasabach-Merritt phenomenenon (KMP) in certain cases. It has rarely been observed in the head and neck region and at times, can present without KMP. Herein, we present an extremely uncommon case of KHE occurring in tonsil of a child, associated with a neck swelling, but unassociated with KMP. A 2-year-old male child referred to us with history of sore throat, d

We present an F-18 FDG PET scan which demonstrates 3 synchronous primary malignancies. The patient is a 61-year-old man who presented with weight loss and dysphagia. He was initially diagnosed with squamous cell carcinoma of the midesophagus, and was then found to have an adenocarcinoma in the right lung. A staging PET scan additionally showed increased left tonsillar uptake. Subsequent biopsy confirmed squamous cell carcinoma of the left tonsil. The demonstration of 3 synchronous primaries by PET is probably rare.

A high proportion of extranodal lymphoid infiltrates are diffuse small lymphocytic proliferations [1, 2] and therefore pose a particularly challenging diagnostic problem [1-4]. Their benign or malignant nature cannot be determined using clinical and radiologic criteria. The application of traditional morphologic criteria (i.e., cytologic maturity and polymorphism favor benignancy, while cytologic atypia and monomorphism favor malignancy) [5, 6], has probably improved diagnostic accuracy. However, these criteria generally have not been helpful in evaluating the large number of lymphoid infiltra

Operations for intraoral carcinoma do produce deformities and loss of function. Historically, surgeons have constantly tried to correct or improve these deficiencies. The tendency on the part of the surgeon is to replace each bit of tissue removed, hoping that if the site appears normal the patient will be normal. The appearance of the patient has as a rule been given more attention than the functional result. Dysfunction is related to the alteration of certain anatomic and physiologic conditions identified as: (1) inability to obliterate the anterior oral cavity, (2) defects in the palate, (3

Here, we present a protocol for spatial proteomic profiling of the tumor microenvironment in tonsil cancer using multiplexed imaging-powered deep visual proteomics (mipDVP). We describe steps for automated 22-plex immunofluorescence staining and imaging on formalin-fixed paraffin-embedded (FFPE) tissue sections, automated single-cell laser microdissection, and single-cell-type mass spectrometry. This workflow enables the spatially resolved isolation of distinct cell populations for proteomic analysis. We optimized this protocol for studying tumor-immune interactions, where it facilitates the s

BACKGROUND: Synovial sarcoma is a rare soft tissue malignancy, occasionally found in the head and neck region. The diagnosis necessitates a multidisciplinary approach involving the clinical presentation, proper imaging studies and histological confirmation, with molecular testing for definitive identification. Treatment entails surgical resection with adjuvant therapies as needed.
CASE PRESENTATION: A 33-year-old male patient presented with globus sensation concomitant with right-sided neck swelling. He was clinically found to have right tonsil enlargement with posterior extension. Therefore,

Significant dysphagia, pain, and risk of bleeding occur after transoral robotic surgery (TORS) radical tonsillectomy. We present a novel surgical technique utilizing robotically assisted submandibular gland transposition (SMGT) to reconstruct the radical tonsillar defect. A 48-year-old male with p16+ tonsillar squamous cell carcinoma underwent deep TORS radical tonsillectomy, contralateral tonsillectomy, ipsilateral neck dissection, and TORS-assisted reconstruction of the radical defect with ipsilateral SMGT. Postoperatively, the patient experienced minimal pain and was discharged on postopera

: A 64-year-old man, whose medical history was significant only for locally advanced squamocellular carcinoma of the right palatine tonsil treated with extended neck radiotherapy 9 years before, was evaluated for traumatic cerebral haemorrhage secondary to syncope after a postural change. The selective angiographic study of cerebral vessels was negative. No heart arrhythmias were recorded at ECG monitoring. The 24-h ABPM revealed sudden pressor and depressor episodes during day-time and a reverse dipper pattern during night-time. Noninvasive autonomic nervous system function testing showed sup

Treatment of squamous cell carcinoma of the oropharynx is challenging because of its effects on speech and swallowing, which may affect quality of life. Transoral robotic surgery may be an effective alternative to open surgery. Robotic lateral oropharyngectomy is best suited for early stage oropharyngeal squamous cell carcinoma, with the goal of avoiding or reducing the use or dose of adjuvant therapies. Successful robotic lateral oropharyngectomy requires appropriate training, detailed preoperative planning, organized operating room setup to obtain exposure, an understanding of the pertinent

Follicular dendritic cell sarcoma (FDCS) is a rare neoplasm that occurs extranodally and nodally. The following case report describes a 24-year-old male patient who suffered from FDCS of the tonsil. He presented at the ENT Department of the University Hospital Magdeburg with throat pain that had lasted for 3 months. There were neither B symptoms nor abnormal fatigue. An extended tonsillectomy was performed. The morphological and immunohistochemical findings confirmed the diagnosis of FDCS. FDCS should be considered as an important differential diagnosis in spindle cell tumors of the tonsil.

BACKGROUND: Actinomyces is a bacterial rod found in the normal oral flora. It can gain entry to the submucosa via trivial wounds and ultimately lead to slow growing lesions which may mimic cancerous lesions.
METHODS AND RESULTS: We present the case of an elderly, immunosuppressed woman who presented with a herald bleed from a tonsillar lesion. Despite initial operative arrest of the hemorrhage, she died of a carotid blowout. At autopsy, no sign of cancer was found, but rather an invasive actinomycosis.
CONCLUSION: Although actinomycosis of the tonsil is well known and has even been described

BACKGROUND AND PURPOSE: To evaluate the feasibility and dosimetric optimization potential of a unilateral two-field intensity-modulated radiotherapy (IMRT) technique in the curative treatment of lateralized tonsil cancer.
MATERIALS AND METHODS: Six patients with lateralized tonsillar carcinoma were treated unilaterally with a two-field IMRT technique (oblique-anterior and oblique-posterior fields, with or without collimator and couch rotation). Alternative IMRT plans using seven non-opposed coplanar fields were compared with the two-field plans for each patient.
RESULTS: Planning target volu

INTRODUCTION: Unilateral tonsillar enlargement is often seen in the out-patient setting. Frequently, these patients are listed for tonsillectomy for the purpose of ruling out malignant histology. This study aims to determine the necessity for tonsillectomy.
METHOD: This retrospective case-note review looks at all the tonsillectomies performed for histological examination at our institution over a five year period, and analyses the histological findings in those with unilateral tonsillar enlargement (UTE) alone, and those with UTE with other clinical features (history of chronic pain, dysphagi

This study examined swallowing transit times and motility problems in three groups of patients following ablative surgery for oropharyngeal carcinoma and in a control group of 10 normal subjects. A total of 30 patients was studied: 10 after anterior floor of mouth resection, 12 after tonsil/base of tongue resection, and 8 after supraglottic laryngectomy. Videofluoroscopic studies of liquid, thin paste, thick paste, and thick paste plus liquid swallows were completed 1 week post-initiation of oral feeding following surgery. From the videotapes, oral and pharyngeal transit times were measured, a

We reported 18 F-FDG and 68 Ga-FAPI-46 PET/CT findings in a 55-year-old woman with metastatic squamous cell carcinoma of the left neck. 18 F-FDG PET/CT was negative for the detection of primary malignancy. On 68 Ga-FAPI PET/CT, increased radiotracer uptake was noted in the left palatine tonsil, whereas low background uptake was observed in the right palatine tonsil. Based on the PET/CT findings, the patient underwent clinical examination and biopsy of the left tonsil. Histology demonstrated the presence of a squamous cell carcinoma of the tonsil. The case demonstrated the added value of 68 Ga-
