Optimizing Transitions of Care: The Impact of Pharmacist-Led Discharge Interventions on Readmissions and Healthcare Costs.

PURPOSE: Medication discrepancies at hospital discharge contribute to readmissions and adverse outcomes. Although national groups endorse pharmacist-led discharge medication reconciliation, implementation varies. We examined the effect of pharmacist-led discharge medication reconciliation on 30-day hospital readmission odds and financial outcomes after launching a transitions-of-care discharge service at our quaternary medical center. METHODS: This retrospective observational study evaluated discharged in-patients who received pharmacist-led discharge medication reconciliation (study interven

Students' perceptions of preparedness for delivering palliative and end-of-life care as they transition to registered nurse.

BACKGROUND: Ensuring that nurses are prepared for the delivery of palliative care is essential to the provision of high-quality palliative and end-of-life care. Education plays a key role in the preparation but may encompass more than skills competence. AIM: To understand student nurses' perceptions of their preparation and experiences of delivering palliative and end-of-life care as they complete their BSc Adult Nursing pre-registration programme. METHODS: Interpretative phenomenological analysis was used to explore student nurses' perceptions of delivering palliative and end-of-life care.

Hospital-to-hospice transfers in Germany: analyzing the impact of age and gender at the care transition interface.

BACKGROUND: Hospice care models vary worldwide due to differences in healthcare systems, cultural attitudes, and resource availability. In Germany, inpatient hospice care plays a vital role in the treatment of critically ill patients, providing inpatient end-of-life care if home care is not possible. The transition between different care settings is a critical interface. This study analyzes aggregated 2023 hospital data to assess the frequency and characteristics of direct transfers from hospitals to inpatient hospices and evaluates the impact of regional infrastructure on these transitions.

22q11.2 Deletion Syndrome: A Comprehensive Review for Pediatricians From Diagnosis to Adult Care Transition.

Affecting ~1 in 4000 to 6000 live births, 22q11.2 deletion syndrome (22q11.2DS) is the most common chromosomal microdeletion syndrome. It results from a 1.5 to 3 Mb heterozygous deletion at chromosome 22q11.2 and is associated with a broad spectrum of clinical manifestations, including cardiac defects, palatal abnormalities, thymic hypoplasia, developmental delays, and neuropsychiatric symptoms. Multiple techniques such as chromosomal microarray and sequencing techniques such as fluorescence in situ hybridization help in the diagnosis of 22q11.2DS. A multidisciplinary approach to management is

Development and validation of the Caregiver-reported Outcome Measure for Emergency care Transitions (COMET) tool.

INTRODUCTION: Emergency department (ED) care transitions are particularly challenging for persons living with cognitive impairment (PLWCI) and their care partners. Existing measures overlook care partners' unique experiences. We developed and validated the Caregiver-reported Outcome Measure for Emergency care Transitions (COMET) tool to assess these transitions. METHODS: We enrolled 170 care partners from four EDs in a multiphase process including qualitative interviews, item development, member checking, cognitive debriefing, expert review, and psychometric testing. RESULTS: The final 15-it

Weaning as an emotional transition in maternal mental health: A call for greater sensitive and inclusive care.

Weaning is commonly understood as a practical transition, yet it carries significant emotional implications that remain largely absent from maternal mental health discussions. Drawing on existing literature and clinical observations, this editorial explores the psychological dimensions of weaning, including guilt, sadness, relief, confusion, and emotional disconnection. The lack of professional visibility of this transition may invalidate maternal emotional experiences, potentially impacting mental health. Recognizing weaning as a meaningful emotional and developmental moment can foster more e

Optimizing the Transition of Care for Postpartum Preeclampsia: A Scoping Review of Management Strategies and Missed Opportunities.

Hypertensive disorders of pregnancy remain a primary driver of preventable maternal morbidity, specifically during the high-risk transition from hospital discharge to long-term primary care. This scoping review maps the "transition gap" within the fourth trimester to determine why traditional management strategies often fail to protect patients at risk for late-onset postpartum preeclampsia. Following Preferred Reporting Items for Systematic reviews and Meta-Analyses extension for Scoping Reviews (PRISMA-ScR) guidelines, a systematic search of PubMed, the Cochrane Library, and Google Scholar w

Strategies to Support Care Transitions for Patients With Opioid Use Disorder: Pathways and Partnerships for Success.

BACKGROUND: The Screening, Brief Intervention, and Referral to Treatment (SBIRT) model has been adapted for hospitals to address opioid use disorder (OUD). While most hospitals conduct screening and brief interventions, many do not offer referrals to further treatment, often due to limited external partnerships. There is limited evidence on which types of partnerships hospitals should prioritize to support care transitions. METHODS: We conducted a survey of U.S.-based experts in addiction medicine and research to identify strategies for improving transitions and partnerships. Participants ran

Transition Readiness and Care Needs Among Youth With Neuroimmune Disorders: A Descriptive Survey Study.

Transition of care (TOC) poses challenges across medical subspecialties, each with distinct patient needs. Understanding patient and caregiver awareness and readiness within specific patient populations is essential for developing effective population-specific TOC programs. Limited data exist describing TOC readiness in patients with neuroimmune disorders. We conducted a cross-sectional survey on youth with neuroimmune disorders and their caregivers assessing TOC readiness, challenges, and support needs. Thirty-one participants completed the survey (39% Hispanic, 29% Caucasian, 19% African Ame

Impact of household size and co-resident multimorbidity on unplanned hospitalisation and transition to care home.

The ability to manage ill health and care needs might be affected by who a person lives with. This study examined how the risk of unplanned hospitalisation and transition to living in a care home varied according to household size and co-resident multimorbidity. Here we show results from a cohort study using  Welsh nationwide linked healthcare and census data, that employed multilevel multistate models to account for the competing risk of death and clustering within households. The highest rates of unplanned hospitalisation and care home transition were in those living alone. Event rates were

Transitioning From Curative to Palliative Care Goals in Pediatrics: Common Pitfalls and Clinical Pearls.

If recent medical advances have positively impacted children's lives, access to this new knowledge and technology raises ethical questions. Medical progress has also contributed to prolonging suffering in those afflicted with life-limiting conditions. Despite the growing tension between the need to discuss end-of-life issues, particularly advanced directives, and the widespread discomfort in engaging in such discussions, the question of death remains frequently evaded. The transition from curative to palliative goals is fraught with misunderstandings and moral distress. This transition of care

Understanding the individual's transition from acute to chronic disabling pain: Opportunities for improved care.

When acute pain persists, it is said to become chronic after 3 months. Considerable interest has focused on why acute pain appears to transition to chronic pain in some cases, but not all, especially when it becomes disabling. We examine our current understanding of the processes involved in the progression from an acute injury to disabling chronic pain. Rather than viewing this progression as a time dependent transition with specific static risk factors, we consider whether it might be more helpful to understand this evolution in terms of unique individual pathways. While brief self-report sc
