Fatbike crashes and severity in a level II trauma center: a 3-month observational study.

OBJECTIVE: Fatbikes are rapidly gaining popularity, but data on the severity and characteristics of fatbike-related injuries are lacking. This study compares fatbike-related crashes with those involving city bikes, e-bikes and road bikes. METHODS: This is a retrospective single-center study conducted at a Dutch level II trauma center. Between July and September 2024, all Emergency Department visits due to bicycle crashes were included. The primary outcome was the Injury Severity Score (ISS), with secondary outcomes including demographic characteristics, specific injuries, helmet use and the m

Hypothermia on trauma center arrival has a much greater impact on outcomes at the extremes of age.

BACKGROUND: Hypothermia is a potentially modifiable risk factor for poor outcomes in trauma; differences in impact of hypothermia on different age groups are not well-defined. This study characterized the effects of varying levels of hypothermia on injured patients by age. We hypothesized that pediatric and geriatric trauma patients would be disproportionately impacted by hypothermia. METHODS: This retrospective cohort study included all patients with a valid arrival temperature from the 2020 to 2023 Trauma Quality Improvement Program database. Patients were categorized by arrival temperature

Impact of trauma center volume on treatment strategies and outcomes of blunt traumatic aortic injuries.

OBJECTIVE: Management of blunt traumatic aortic injuries (BTAIs) has changed significantly over time. There is now increasing support for me … ce in baseline patient demographics or presenting hemodynamics between those treated at high-volume trauma centers (HVCs) vs low-volume trauma centers (LVCs). The average age of the cohort was 43.5 years, and 75.9% were male. Patients evaluated at HVCs had more concomitant injuries, as determined by a higher overall Injury Severity Score (ISS; 35.1 vs 33.8; P < …

Contemporary Analysis of Diagnosis, Treatment, and Outcomes in Pisiform Fractures at a Level 1 Trauma Center.

BACKGROUND: Pisiform fractures are rarely occurring carpal injuries and often are undetected due to more obvious associated injuries and subtle radiographic findings. While nondisplaced fractures are typically treated conservatively, significantly displaced fractures can be treated operatively; however, there is a lack of consensus given the injury's rarity and limited research. This study offers an extensive analysis of pisiform fractures, associated fractures, treatment methods, and long-term outcomes. METHODS: A retrospective review of patients with diagnosed pisiform fractures from 2017 t

Characterization of Propranolol Use and Outcomes in Traumatic Brain Injury at a Level I Trauma Center.

OBJECTIVES: The Eastern Association for the Surgery of Trauma guidelines conditionally recommend beta blockers for adults with traumatic brain injury (TBI); however, real-world utilization varies widely. This study describes critically ill patients with TBI who received propranolol during their hospitalization. METHODS: This was a retrospective study evaluating patients 18 years or older admitted with TBI to the intensive care unit (ICU) at a level 1 trauma center between January 1, 2015, and April 30, 2023. Patients were identified using the institution's trauma registry and propranolol admi

Improving Massive Transfusion Protocol Response in the Rural Level III Trauma Center Setting.

BACKGROUND: Massive transfusion protocol (MTP) activations at rural Level III trauma centers often face blood delivery delays, limited support, and product waste. While MTPs are well described in higher-level centers, their effectiveness and staff perceptions in rural Level III settings are unclear. Process breakdowns at one such center, including staffing gaps, delivery delays, and product waste, prompted implementation of a targeted MTP response system. OBJECTIVE: This project aims to evaluate the effectiveness of an MTP activation response system in a rural Level III trauma center setting.…

Blood transfusions in the emergency department: A descriptive analysis and referral patterns at a non-trauma center.

OBJECTIVES: Anemia is a prevalent condition associated with significant healthcare burden, contributing to approximately 800,000 Emergency Department (ED) visits annually in the United States. While immediate management often relies on packed red blood cell (PRBC) transfusions, limited data exist on the populations receiving transfusions in the ED, especially outside traumatic indications. This study aims to characterize patients receiving PRBC transfusions in the ED, with particular focus on the role of outpatient referrals and potential differences among subgroups based on clinical presentat

Prehospital care: Lessons learned from 311 mortalities at level I trauma center.

PURPOSE: Trauma, a leading cause of disability and death globally, underscores the critical importance of timely medical intervention, particularly within the "golden hour" following injury. Yet, in low-middle-income countries, the absence or fledgling state of emergency medical systems (EMS) exacerbates mortality rates. This study aims to retrospectively analyze mortalities at a level 1 trauma center in India, comparing patients who received prehospital care with those who did not. METHODS: Conducted at a level 1 trauma center in India from April 2019 to April 2020, this retrospective observ

A Case Series Focusing on Blunt Traumatic Diaphragm Injury at a Level 1 Trauma Center.

Introduction: Detection of blunt traumatic diaphragm injury (TDI) can be challenging in the absence of surgical exploration. Our objective is to study the mechanisms of injury and detection modes for patients with blunt TDI. Methods: This is a single-center, retrospective review conducted in a level 1 trauma center from 2016 to 2023, inclusive. We identified seven patients with blunt TDI using the primary mechanisms and trauma type. Results: Out of seven patients, two were associated with motor vehicle collisions, four were pedestrians struck, and one fell down the stairs. The mean ISS was 48.

Management of Complete Cricotracheal Separation: A Case Report From a Level 1 Trauma Center.

Complete cricotracheal separation is a rare and severe form of laryngeal trauma that most clinicians encounter infrequently, resulting in limited experience with its management. This report discusses the intricate management of a patient with complete cricotracheal separation following a traumatic incident. We present this case to highlight the challenges in the diagnosis and airway management of complete cricotracheal separation. The patient was transferred to our level 1 trauma center with a secured airway via an endotracheal tube from the referring hospital, where they experienced respirato

A Retrospective Study of Facial Fractures Associated With the Amazon Region at a Trauma Center in Northern Brazil.

BACKGROUND/AIM: Facial trauma is a major public health challenge due to its aesthetic, functional, psychological, and socioeconomic impacts. In Northern Brazil, particularly in the Amazon region, subsistence activities such as plant extraction, animal handling, and river transportation are essential to daily life and may contribute to an increased risk of facial injuries. These environmental and cultural factors are often underrepresented in the literature. This study aimed to investigate the epidemiological profile of facial fractures associated with etiologies specific to the Amazon region.

Preventing Atrophic Long-Bone Nonunion: Retrospective Analysis at a Level I Trauma Center.

Background: Among the risk factors for nonunion are unchangeable patient factors such as the type of injury and comorbidities, and factors that can be influenced by the surgeon such as fracture treatment and the postoperative course. While there are numerous studies analyzing unchangeable factors, there is poor evidence for factors that can be affected by the physician. This raises the need to fill the existing knowledge gaps and lay the foundations for future prevention and in-depth treatment strategies. Therefore, the goal of this study was to illuminate knowledge about nonunion in general a
