Chronic tension-type headache: what is new?

PURPOSE OF REVIEW: This review discusses current data on nosological boundaries related to diagnosis, pathophysiology and therapeutic strategies in chronic tension-type headache (CTTH). RECENT FINDINGS: Diagnostic criteria of CTTH should be adapted to improve its sensitivity against migraine. It seems that mechanical pain sensitivity is a consequence and not a causative factor of CTTH. Recent evidence is modifying previous knowledge about relationships between muscle tissues and CTTH, suggesting a potential role of muscle trigger points in the genesis of pain. An updated pain model suggests t…

Preprocedural Screening Tool to Guide Nonpulmonary Vein Trigger Testing in First-Time Atrial Fibrillation Ablation.

BACKGROUND: Patients undergoing first-time atrial fibrillation (AF) ablation can benefit from targeting non-pulmonary vein (PV) triggers. Preprocedural identification of high-risk individuals can guide planning of ablation strategy. This study aimed to create a preprocedural screening tool to identify patients at risk of non-PV triggers during first-time AF ablation. METHODS: All patients who underwent first-time AF ablation at the Hospital of the University of Pennsylvania between 2018 and 2022 were identified. Those who underwent non-PV trigger provocative maneuvers or had spontaneous non-P

Clinical experience of the percutaneous release for trigger fingers.

BACKGROUND: Conservative management for the trigger fingers includes splinting, steroid injection and other adjuvant methods. If conservative treatment fails, a surgical release of the A1 pulley is offered. Although the success rate of the surgical intervention is high, the complications, for example, a digital nerve injury, bowstringing, infection and continued triggering, have been reported. Percutaneous release with an 18 guage needle has been reported as a safe and effective procedure for the trigger fingers. This study evaluates the safety and efficacy of the percutaneous release. METHOD

Low Inter-Rater Reliability and Reproducibility of Neck Reflex/"Adler-Langer" Points in Neural Therapy Diagnostics but Increased Pressure Pain Threshold after Therapy: Results of a Randomized Controlled Observer-Blind Trial.

BACKGROUND: Neck reflex points or Adler-Langer points are commonly used in neural therapy to detect so-called interference fields. Chronic irritations or inflammations in the sinuses, teeth, tonsils, or ears are supposed to induce tension and tenderness of the soft tissues and short muscles in the upper cervical spine. The individual treatment strategy is based on the results of diagnostic Adler-Langer point palpation. This study investigated the inter- and intra-rater reliability and explored treatment effects. METHODS: We performed a randomized controlled trial with 104 inpatients (80.8% fe

Pseudo-dental pain and sensitivity to percussion.

Two case reports examine a little-known cause of dental pain and sensitivity to percussion. Contrary to the traditional assumption that pain and sensitivity to percussion almost always are diagnostic of pulpal inflammation and/or necrosis, these symptoms actually may be referred to the sensitive tooth from trigger points in the masticatory muscles. Therefore, myofascial pain syndrome must be ruled out in patients who have dental pain and display sensitivity to percussion.

Myofascial pain and fibromyalgia: Diagnosis and treatment.

Myofascial pain syndrome (MPS) and fibromyalgia (FM) are common muscular pain syndromes. They are both characterized by tenderness, but MPS is further characterized by the myofascial trigger point that has a taut band and causes referred pain. FM can be either primary (idiopathic) or secondary. MPS is always secondary to some muscle stressor. The diagnosis of these conditions is generally made by physical examination. Algometry can quantify the tenderness. EMG of the trigger point and diagnostic ultrasound can provide objective evidence of the trigger point. Biochemical markers and other pheno

Breast Implant-Associated Anaplastic Large Cell Lymphoma (BIA-ALCL): An Open Wound.

Since 2017, there have been an increasing number of reported cases regarding breast implant-associated anaplastic large cell lymphoma (BIA-ALCL) in the literature. Although significant attention was dispensed for this relevant issue, there is no consensus on what the trigger points for disease development are. BIA-ALCL trigger point speculation includes textured breast implants, bacterial contamination, and genetic factors. However, little attention is given by the literature regarding gel bleeding and the toxicity of polydimethylsiloxane. This opinion-based article aims to report our experien…

[From fascial tension abnormality to myofascial chain: modern anatomical and biomechanical interpretation of "taking painful points as acupoints" in acupuncture and moxibustion].

"Taking painful points as acupoints" is an important principle of acupoint selection in acupuncture and moxibustion. But its traditional connotation and modern mechanism have long been controversial. In this paper, the principle of "taking painful points as acupoints" is reinterpreted in light of anatomical and biomechanical studies of fascial system. Fascia, as a tension network covering the whole body, plays a crucial role in the onset and persistence of pain. The abnormal fascial tension may form tender points and myofascial trigger points (MTrPs), which are highly consistent with the distr…

Antiviral innate immunity induces alpha synuclein phosphorylation at serine129 in neurons independent of aggregation.

Alpha synuclein (asyn) is expressed in neurons and is associated with the pathogenesis of synucleinopathies like Parkinson's Disease. Asyn aggregates are associated with phosphorylation at serine position 129 (pS129), which can be induced by environmental triggers, including viral infection, and the risk increases with aging. We show that brain tissue from West Nile Virus (WNV) infected patients exhibit increased expression of pS129 asyn in neurons of brain tissue. We found that WNV infection, Poly I:C treatment, and type 1 interferon (T1 IFN) treatment elicit the acute induction, followed by

Update on the efficacy of extracorporeal shockwave treatment for myofascial pain syndrome and fibromyalgia.

Chronic muscle pain syndrome is one of the main causes of musculoskeletal pathologies requiring treatment. Many terms have been used in the … ness of the muscle, caused by hyperirritable nodules in musculoskeletal fibers, known as myofascial trigger points (MTP), and fascial constrictions. Fibromyalgia (FM) is a chronic condition that involves both central and peripheral sensitization and for which no curative treatment is available at the present time. Fibromyalgia shares some of the features of MPS, …

A new dyslipidemia-based scoring model to predict transplant-free survival in patients with hepatitis E-triggered acute-on-chronic liver failure.

BACKGROUND/AIMS: Hepatitis E virus (HEV)-triggered acute-on-chronic liver failure (ACLF) has unacceptably high short-term mortality. However, it is unclear whether the existing predictive scoring models are applicable to evaluate the prognosis of HEV-triggered ACLF. METHODS: We screened datasets of patients with HEV-triggered ACLF from a regional tertiary hospital for infectious diseases in Shanghai, China, between January 2011 and January 2021. Clinical and laboratory parameters were recorded and compared to determine a variety of short-term mortality risk factors, which were used to develop
