[Compression of the ulnar nerve at Guyon's canal caused by a pseudoaneurysm of the ulnar artery following trauma].

We repor there on a 16-year-old patient who presented with pain and swelling in the hypothenar eminence as well as loss of sensibility in the fingers of the region innervated by the ulnar nerve; this happened 2-3 weeks after an injury by a glass splinter in his proximal palm. A pseudoaneurysm could be verified by duplex sonography. The patient wished to avoid any graft for arterial bridging for religious reasons. On the basis of an the Allen test preoperatively and the intraoperative findings, an adequate blood supply of the finger by the radial artery was expected. Thus, in respect to the pat

A Vascular Malformation in the Hand with Compromised Neurologic Status.

BACKGROUND: Vascular malformations of the hand are rare vascular malformations that are challenging to treat. METHODS: We present a case of a large vascular malformation with left hand pain and decreased sensation of the small and ring fingers. The lesion was treated operatively with surgical excision. RESULTS: The malformation was successfully removed surgically, and pain resolved and numbness recovered by 2 weeks after surgery. CONCLUSIONS: This is a rare case of large vascular malformation in the hand with compromised neurologic status. Surgical treatment provided complete relief of the

[Endoscopically assisted nerve decompression of nerve nerve compression syndromes at the upper extremity].

Besides carpal tunnel and cubital tunnel syndrome, other nerve compression or constriction syndromes exist at the upper extremity. Using the technique of endoscopically assisted decompression such rare nerve compression syndromes of the upper extremity can be treated. The technique of endoscopical decompression is presented in six patients with rare compression or hour-glass-like constriction syndromes at the upper extremity. According to the classification of Roles and Maudsley in 5 of 6 cases excellent results were recorded. All but one patient considered the results excellent. The poorest r

Ulnar nerve transposition using a mini-invasive approach: case series of 30 patients.

The treatment of ulnar nerve compression at the elbow remains controversial. No single technique has yet proven its superiority. We describe a technique combining the advantages of the mini-invasive approach with those of transposition. We present the results of 30 patients, of mean age 52 years, who underwent anterior subcutaneous transposition of the ulnar nerve using a mini-invasive approach with a follow-up of more than six months. The incision measures 3 cm. The results were evaluated by measuring pain intensity, quick disabilities of the arm shoulder and hand (DASH), grip strength and pi

The influence of glove and hand position on pressure over the ulnar nerve during cycling.

BACKGROUND: Chronic ulnar nerve compression is believed to be the primary cause of sensory and motor impairments of the hand in cyclists, a condition termed Cyclist's Palsy. The purpose of this study was to quantitatively evaluate the effects that hand position and glove type can have on pressure over the ulnar nerve, specifically in the hypothenar region of the hand. METHODS: Thirty-six experienced cyclists participated. Subjects rode at a constant cadence and power output on a stationary bicycle with their hands in the tops, drops and hoods of a standard drop handlebar. A high resolution pr

Strain on the ulnar nerve at the elbow and wrist during throwing motion.

BACKGROUND: It is well known that cubital tunnel syndrome frequently occurs in throwing athletes. The cause of cubital tunnel syndrome is considered to be mechanical stimuli on the ulnar nerve in the cubital tunnel. The hypothesis of the present cadaveric study was that the ulnar nerve is subjected to longitudinal strain in the cubital tunnel during the throwing motion. METHODS: Four phases of throwing (stance, wind-up, middle cock-up, and early acceleration) were passively simulated in seven fresh-frozen transthoracic cadaveric specimens that were fixed in an upright position to allow free a

Occurrence of nerve entrapment lesion in chronic inflammatory demyelinating polyneuropathy.

OBJECTIVE: To evaluate the occurrence of nerve entrapment syndrome in chronic inflammatory demyelinating polyneuropathy (CIDP). METHODS: We retrospectively evaluated neurophysiologic results of 41 (25 male and 16 female, mean age 49.8, range 11-87) patients with CIDP. We evaluated the frequency of focal neurophysiologic lesion at entrapment site distinguishing two kinds of lesion: (a) true entrapment; and (b) false entrapment on the basis of nerve conduction results. RESULTS: Occurrence of focal aggression within the entrapment site is similar to that out of the entrapment site in all examin

Risk factors and outcomes in 385 cases of ulnar nerve submuscular transposition.

Submuscular transposition (SMT) for treatment of ulnar nerve entrapment is commonly performed, however published comparisons of surgical techniques exclude a high proportion of the at-risk population encountered in real world practice. To examine the influence of risk factors on the clinical outcome following SMT we performed a retrospective review of all patients who underwent SMT, including patient self-reported outcome and Louisiana State University Medical Centre ulnar nerve grading scale. A total of 403 ulnar nerves were operated, with follow-up data available for 385 cases (359 patients)

Combined manifestation of a neurofibroma and a nerve sheath ganglion in the ulnar nerve after radiotherapy in early childhood.

Occurrence of clinically symptomatic benign neurofibromas of peripheral nerves after radiotherapy is a rarity. We saw a 55-year-old female who developed progressive failure of the ulnar nerve 55 years after 20Gy (226)Ra brachytherapy of a haemangioma of the left elbow at the age of 3 months. Nerve compression at the sulcus segment was caused by the intraneural growth of a neurofibroma and the formation of a nerve sheath ganglion. The rapidly progressive symptoms required operative treatment. Due to the infiltrating tumour growth we decided to resect this segment of the ulnar nerve and reconstr

Neurophysiological classification of ulnar entrapment across the elbow.

Ulnar nerve entrapment across the elbow (UAE), a common entrapment, requires neurophysiological evaluation for a diagnosis, but a standardised neurophysiological classification is not available. The aim of our study was to evaluate the validity of a neurophysiological classification of UAE, developed by us. To this end, we examined whether sensorimotor deficits, as observed by the physician and as referred by the patients, increased with the neurophysiological severity according to the classification. We performed a multiperspective assessment of 63 consecutive arms from 52 patients with a cli

Analysis of tardy ulnar nerve palsy associated with cubitus varus deformity after a supracondylar fracture of the humerus: a report of four cases.

Four cases of tardy ulnar nerve palsy associated with a cubitus varus deformity of the elbow secondary to a supracondylar fracture of the humerus are presented. All patients had surgical management of their ulnar nerve palsy. In two patients, the ulnar nerve was entrapped by scar tissue at the abnormal position and the nerve developed a sharp V-shaped kink when the elbow was flexed. In one patient, the ulnar nerve displaced anteriorly with elbow flexion and spontaneously reduced into the ulnar nerve groove with elbow extension. In one patient, the ulnar nerve remained in the ulnar nerve groove

Nerve entrapment complicating neurogenic thoracic outlet syndrome surgery: A 10-year retrospective study.

Neurogenic thoracic outlet syndrome results from compression of the brachial plexus. The symptoms are mainly pain, upper-limb weakness and paresthesia. Management always starts with a rehabilitation program, but failure of rehabilitation may necessitate surgery. In practice, we observed that several patients developed secondary distal nerve entrapment in the months following surgery, with no preoperative compression. We aimed to assess the occurrence of distal nerve entrapment after surgery for neurogenic thoracic outlet syndrome in a retrospective cohort study. Seventy-four patients were incl

Endoscopic release of the median nerve in the proximal third of the forearm. A new approach.

INTRODUCTION: Pronator syndrome has classically required open surgery that leaves a large scar; initial endoscopic techniques required approaches of an average 4cm without achieving release of all structures. The purpose of this study was to describe a new endoscopic approach that allows the median nerve to be safely and completely decompressed, leaving a smaller and less visible scar. METHODS: Description of a new approach for decompression of the median nerve in the proximal third of the forearm with minimal incision and endoscopic technique in cryopreserved cadaveric specimens, describing

Biphasic motion of the median nerve in the normal Asian population.

BACKGROUND: The biomechanical interaction between the median nerve and the flexor tendons is an important consideration in Carpal tunnel syndrome (CTS). We aim to quantify the displacement and compressive deformation pattern of the median nerve in various stages of finger flexion in the normal population at the inlet of the carpal tunnel. METHODS: Transverse ultrasounds images were taken at the carpal tunnel inlet during full-extension, mid-flexion and full flexion. The displacement, distance, Feret's diameter, and perimeter of the median nerve were calculated and compared between each positi

Single tendon--two insertion versus two tendon transfers for reablement of ulnar-median palsied thumb in leprosy.

Twenty-three opponensplasties for ulnar median paralysis in 20 leprosy patients were performed by two different procedures i.e. extensor indicis proprius and flexor digitorum superficialis transfers using standard techniques. The results were evaluated using various objective and subjective, anatomical and functional parameters. Two tendon transfers appeared to be superior to single tendon two insertion transfers. It was concluded that if situation permits, two tendon transfers should be performed for combined ulnar median paralysis.
