A model-informed clinical trial simulation tool with a graphical user interface for Duchenne muscular dystrophy.

Quantitative model-based clinical trial simulation tools play a critical role in informing study designs through simulation before actual execution. These tools help drug developers explore various trial scenarios in silico to select a clinical trial design to detect therapeutic effects more efficiently, therefore reducing time, expense, and participants' burden. To increase the usability of the tools, user-friendly and interactive platforms should be developed to navigate various simulation scenarios. However, developing such tools challenges researchers, requiring expertise in modeling and i

Validation of a novel virtual reality simulation system with the focus on training for surgical dissection during laparoscopic sigmoid colectomy.

BACKGROUND: Mastery of technical skills is one of the fundamental goals of surgical training for novices. Meanwhile, performing laparoscopic procedures requires exceptional surgical skills compared to open surgery. However, it is often difficult for trainees to learn through observation and practice only. Virtual reality (VR)-based surgical simulation is expanding and rapidly advancing. A major obstacle for laparoscopic trainees is the difficulty of well-performed dissection. Therefore, we developed a new VR simulation system, Lap-PASS LP-100, which focuses on training to create proper tension

Preliminary study on mechanical characteristics of maxillofacial soft and hard tissues for virtual surgery.

PURPOSE: Virtual surgery system can provide us a realistic and immersive training environment, in which haptic force-feedback gives operators 'touching feeling.' Appropriate deformation models of soft and hard tissues are required for the achievement of real-time haptic feedback. To improve accuracy of modeling and haptic feedback simulation for maxillofacial virtual surgery, mechanical characteristics of soft and hard tissues should be explored. METHODS: Craniofacial soft tissues from one male and female cadavers were divided into two layers: skin and muscle. Maxillofacial tissues were divid

Virtual Reality in Pediatric Psychology.

Virtual reality (VR) technologies allow for controlled simulations of affectively engaging background narratives. These virtual environments offer promise for enhancing emotionally relevant experiences and social interactions. Within this context, VR can allow instructors, therapists, neuropsychologists, and service providers to offer safe, repeatable, and diversifiable interventions that can benefit assessments and learning in both typically developing children and children with disabilities. Research has also pointed to VR's capacity to reduce children's experience of aversive stimuli and re

Identification of novel anthrax toxin countermeasures using in silico methods.

Anthrax is an acute infectious disease caused by the spore-forming, gram-positive, rod-shaped bacterium Bacillus anthracis. The anthrax toxin lethal factor (LF) is the primary anthrax toxin component responsible for cytotoxicity and host death and has been a heavily researched target for design of postexposure therapeutics in the event of a bioterror attack. Various computer-aided drug design methodologies have proven useful for pinpointing new antianthrax drug scaffolds, optimizing existing leads and probes, and elucidating key mechanisms of action. We present a selection of in silico virtual

Combination of brain-computer interface training and goal-directed physical therapy in chronic stroke: a case report.

BACKGROUND: There is no accepted and efficient rehabilitation strategy to reduce focal impairments for patients with chronic stroke who lack residual movements. METHODS: A 67-year-old hemiplegic patient with no active finger extension was trained with a brain-computer interface (BCI) combined with a specific daily life-oriented physiotherapy. The BCI used electrical brain activity (EEG) and magnetic brain activity (MEG) to drive an orthosis and a robot affixed to the patient's affected upper extremity, which enabled him to move the paralyzed arm and hand driven by voluntary modulation of micr

Prosthetic interfaces with the visual system: biological issues.

The design of effective visual prostheses for the blind represents a challenge for biomedical engineers and neuroscientists. Significant progress has been made in the miniaturization and processing power of prosthesis electronics; however development lags in the design and construction of effective machine-brain interfaces with visual system neurons. This review summarizes what has been learned about stimulating neurons in the human and primate retina, lateral geniculate nucleus and visual cortex. Each level of the visual system presents unique challenges for neural interface design. Blind pat

Evolution of surgical skills training.

Surgical training is changing: one hundred years of tradition is being challenged by legal and ethical concerns for patient safety, work hours restrictions, the cost of operating room time, and complications. Surgical simulation and skills training offers an opportunity to teach and practice advanced skills outside of the operating room environment before attempting them on living patients. Simulation training can be as straight forward as using real instruments and video equipment to manipulate simulated "tissue" in a box trainer. More advanced, virtual reality simulators are now available an

Urethral diverticulum: diagnosis with virtual CT urethroscopy.

OBJECTIVE: A 26-year-old woman presented with urinary frequency. Findings were negative on voiding cystourethrography. Cystourethroscopy failed to reveal a urethral diverticulum. A new method of virtual CT urethroscopy was performed using a 16-MDCT scanner. The orifice of the diverticulum was identified clearly on 3D virtual urethroscopy. Intraoperatively, the diverticulum was identified, with the orifice location compatible with that seen on virtual urethroscopy. CONCLUSION: Virtual CT urethroscopy provides more information than conventional urethral examinations and is less invasive than co

"Conflicting" motion cues to the visual and vestibular self-motion systems around 0.06 Hz evoke simulator sickness.

The basic question this research addressed was, how does simulator sickness vary with simulated motion frequency? Participants were 11 women and 19 men, 20 to 63 years of age. A visual self-motion frequency response curve was determined using a Chattecx posture platform with a VR4 head-mounted display (HMD) or a back-projected dome. That curve and one for vestibular self-motion specify a frequency range in which vestibular and visual motion stimuli could produce conflicting self-motion cues. Using a rotating chair and the HMD, a third experiment supported (p < .01) the hypothesis that conflict

Virtual reality modeling and computer-aided design in pediatric surgery: applications in laparoscopic pyloromyotomy.

As a test of the applicability of modern bioengineering tools to pediatric surgery, we developed by computer-aided design (CAD) methods and evaluated a virtual reality (VR) model of laparoscopic pyloromyotomy (LP) and created a set of retractable hook instruments, called Pylorohooks. Our VR model of LP incorporated the infant's body wall, stomach, and hypertrophied pylorus as well as laparoscopic cannulas and two Pylorohooks. CAD software was used to design the 3-mm-diameter, retractable, double-pronged, minimally-traumatic hook instrument. A rapid prototype processor was employed to create a

The development of the virtual reality system for the treatment of the fears of public speaking.

The fear of public speaking is a kind of social phobias. The patients having the fear of public speaking show some symptoms like shame and timidity in the daily personal relationship. They are afraid that the other person would be puzzled, feel insulted, and they also fear that they should be underestimated for their mistakes. For the treatment of the fear of public speaking, the cognitive-behavioral therapy has been generally used. The cognitive-behavioral therapy is the method that makes the patients gradually experience some situations inducing the fears and overcome those at last. Recently

Brain-muscle-computer interface: mobile-phone prototype development and testing.

We report prototype development and testing of a new mobile-phone-based brain-muscle-computer interface for severely paralyzed persons, based on previous results from our group showing that humans may actively create specified power levels in two separate frequency bands of a single surface electromyography (sEMG) signal. EMG activity on the surface of a single face muscle site (auricularis superior) is recorded with a standard electrode. This analog electrical signal is imported into an Android-based mobile phone and digitized via an internal A/D converter. The digital signal is split, and th

TransKinect: a computer vision and machine learning clinical decision support system for automatic independent wheelchair transfer technique assessment.

BACKGROUND: Physical and occupational therapists provide routine care for manual wheelchair users and are responsible for training and assessing the quality of transfers. These transfers can produce large loads on the upper extremity joints if improper sitting-pivot-technique is used. Methods to assess quality of transfers include the Transfer Assessment Instrument, a clinically validated tool derived from quantitative biomechanical features; however, adoption of this tool is low due to the complex usage requirements and speed of typical transfers. OBJECTIVE: The objective of this study is to
