Despite the decline in the number of operations for intractable duodenal ulcer disease, there are still many patients who suffer from the side effects of vagotomy. Incomplete vagotomy may be a significant cause of surgical treatment failure. Diagnosis of incomplete vagotomy is not easy since there is no reliable, safe, single test available for this purpose. Proximal gastric vagotomy for bleeding duodenal ulcer was performed in 52 low-risk patients between 1973 and 1986. Duodenotomy without violation of the pylorus was done in all patients to allow inspection and control of the bleeding site. The median duration of operation was 3 hours and 20 minutes, although 25% of the procedures required 4 or more hours. There was no postoperative mortality and no early reoperations. The article analyses the results of truncal and selective proximal vagotomy with gastric draining operations in 101 patients with bleeding duodenal ulcer. The work is based on the results of analysis of clinical material concerning patients who were treated by operation for duodenal ulcer 3-5 years earlier. Truncal vagotomy and pyloroplasty (TV + P) was performed in 166 and selective proximal vagotomy with pyloroplasty (SPV + P) in 195 patients. The authors found the long-term results to be similar in both groups and did not reveal any advantages of SPV + P over TV + P, in view of which they do not find it necessary in principle to decline from conducting TV + P at the present time.

The authors in the experiment on 24 rabbits have developed a method for determination of the completeness of vagotomy at operation based on the potentiometric measurement of total redox potential of the serosa of the anterior and posterior walls of the body, fundus and cardia. A method was used in 40 patients with perforative duodenal ulcer. The suggested method has the advantages over the existing ones, and can be used in the surgical practice.

Twenty-three selected patients presenting with massive bleeding from peptic ulcers underwent emergency parietal cell vagotomy (PCV). Nineteen patients had a duodenal ulcer, two a prepyloric ulcer, and one a gastric ulcer. Truncal vagotomy and pyloroplasty for perforated duodenal ulcer. A retrospective study of 220 patients operated upon between 1959 and 1982 for perforated duodenal ulcer by truncal vagotomy and pyloroplasty shows that this operation can be performed as safely as suture plication, even in the critically ill patient. The advantages of this procedure as the routine treatment of perforated duodenal ulcer are discussed.
