Preparation of silibinin- dendrimer-stabilized Au nanoparticles for decreasing vancomycin resistance in S. aureus clinical isolates.

INTRODUCTION: Silibinin (SB), a bioactive flavonolignan derived from milk thistle seeds (Silybum marianum L.), exhibits well-documented antioxidant, antibacterial, antifungal, antiviral, and hepatoprotective activities. This study aimed to conjugate SB with fourth-generation poly(amidoamine) dendrimer-stabilized gold nanoparticles (DSA) to enhance its solubility and evaluate its antibacterial efficacy against clinical isolates of Staphylococcus aureus (S. aureus). METHODS: The morphology and structural features of silibinin-dendrimer-stabilized gold nanoparticle complex (SB-DSA) were characte

Development of an ultrafast PCR to detect clinically relevant acquired vancomycin-resistance genes from cultured enterococci.

BACKGROUND: VRE are increasingly described worldwide. Screening of hospitalized patients at risk for VRE carriage is mandatory to control their dissemination. Here, we have developed the Bfast [VRE Panel] PCR kit, a rapid and reliable quantitative PCR assay for detection of vanA, vanB, vanD and vanM genes, from solid and liquid cultures adaptable to classical and ultrafast real-time PCR platforms. METHODS: Validation was carried out on 133 well characterized bacterial strains, including 108 enterococci of which 64 were VRE. Analytical performances were determined on the CFX96 Touch (Bio-Rad)

Structural and Functional Analysis of Urease Accessory Protein E from Vancomycin-Resistance Staphylococcus aureus MU50 Strain.

BACKGROUND: An increasing prevalence of biofilm forming strains by vancomycinresistance Staphylococcus aureus (VRSA) is one of the most important causes of antimicrobial resistance. VRSA possesses various regulatory factors to form and sustain biofilm in biotic or abiotic conditions. Among them, ureolytic activity is an important factor in the stabilization of biofilms by neutralizing the acidic environment. Various urease accessory proteins are required to activate the urease enzyme inside the biofilm. OBJECTIVE: To optimize the cloning, expression and purification of urease accessory protei

When less is more: shortening the Lpp protein leads to increased vancomycin resistance in Escherichia coli.

Vancomycin is a naturally occurring cell-wall-targeting glycopeptide antibiotic. Due to the low potency of this antibiotic against Gram-negative pathogens, such as Escherichia coli, there is a limited knowledge about interactions between vancomycin and this group of bacteria. Here, we show that an in-frame 63 bp deletion of the lpp gene caused a fourfold increase in vancomycin resistance in E. coli. The resulting protein, LppΔ21, is 21 amino acids shorter than the wild-type Lpp, a helical structural lipoprotein that controls the width of the periplasmic space through its length. The mutant rem…

Distinguishing Clinical Enterococcus faecium Strains and Resistance to Vancomycin Using a Simple In-House Screening Test.

Vancomycin-resistant enterococci (VRE) are a major concern as microorganisms with antimicrobial resistance and as a public health threat contributing significantly to morbidity, mortality, and socio-economic costs. Among VREs, vancomycin-resistant Enterococcus faecium (VREfm) is frequently isolated and is resistant to many antibiotics used to treat patients with hospital-acquired infection. Accurate and rapid detection of VREfm results in effective antimicrobial therapy, immediate patient isolation, dissemination control, and appropriate disinfection measures. An in-house VREfm screening broth

Emergence of Linezolid Resistance Associated with 23S rRNA G2576T Mutations in Vancomycin-Resistant Enterococcus faecium during Oxazolidinone Therapy.

Linezolid is a key option for vancomycin-resistant enterococci (VRE), yet resistance can emerge during therapy. We aimed to characterize the emergence of linezolid resistance in vanA-positive Enterococcus faecium using serial isolates and genomic analysis. Serial VRE isolates obtained during persistent bacteremia after allogeneic hematopoietic stem cell transplantation underwent broth microdilution MIC testing and short- and long-read whole-genome sequencing. Mutations in 23S rRNA domain V and ribosomal protein genes (rplC/rplD) and acquired oxazolidinone resistance genes (cfr, optrA, poxtA) w

Decolonisation of a multi-drug-resistant Escherichia coli and a vancomycin-resistant Enterococcus faecium after intervention with faecal microbiota transplantation.

In this study, we report a case of decolonisation of a multi-drug-resistant extended spectrum beta-lactamase-producing Escherichia coli, and a vancomycin-resistant Enterococcus faecium after intervention with capsulated faecal microbiota transplantation (FMT). Following eradication, our patient acquired an extended-spectrum beta-lactamase-producing Klebsiella pneumoniae, which was subsequently eliminated after a second round of FMT capsules. Additionally, we report the detection of six distinct carbapenemase-producing organisms in this patient, who was evacuated from the war in Ukraine.

Development of a One-Step Multiplex qPCR Assay for Detection of Methicillin and Vancomycin Drug Resistance Genes in Antibiotic-Resistant Bacteria.

The most common antibiotic-resistant bacteria in Korea are methicillin-resistant Staphylococcus aureus (MRSA) and vancomycin-resistant enterococci (VRE). Pathogen identification in clinical laboratories can be divided into traditional phenotype- and genotype-based methods, both of which are complementary to each other. The genotype-based method using multiplex real-time polymerase chain reaction (PCR) is a rapid and accurate technique that analyzes material at the genetic level by targeting genes simultaneously. Accordingly, we aimed to develop a rapid method for studying the genetic character

Quantification and Multidrug Resistance Profiles of Vancomycin-Resistant Enterococci Isolated from Two Wastewater Treatment Plants in the Same Municipality.

Wastewater treatment plants (WWTPs) are points of control for the environmental dissemination of antimicrobial resistant bacteria. Vancomycin-resistant enterococci (VRE) were used as indicators of antimicrobial resistance (AMR) in two WWTPs (biologically aerated filter (BAF) and conventional activated sludge (CAS)) in the same municipality. The removal and abundance of enterococci and VRE as well as the species and antimicrobial resistance profiles of VRE were assessed. Enterococci and VRE from the primary and final effluents were enumerated. Results were assessed from an ecological context. V

Genomic insights on heterogeneous resistance to vancomycin and teicoplanin in Methicillin-resistant Staphylococcus aureus: A first report from South India.

Methicillin-resistant Staphylococcus aureus (MRSA) infection is an important clinical concern in patients, and is often associated with significant disease burden and metastatic infections. There is an increasing evidence of heterogeneous vancomycin-intermediate S. aureus (hVISA) associated treatment failure. In this study, we aim to understand the molecular mechanism of teicoplanin resistant MRSA (TR-MRSA) and hVISA. A total of 482 MRSA isolates were investigated for these phenotypes. Of the tested isolates, 1% were identified as TR-MRSA, and 12% identified as hVISA. A highly diverse amino ac
