[Regularization of ventricular intervals in atrial fibrillation--electrophysiologic findings on the underlying mechanism].

In 13 patients with chronic atrial fibrillation, programmed right ventricular pacing was performed before and after intravenous administration of 4 mg gallopamil. Application of the Ca-antagonist resulted in a marked decrease in the ventricular response in all and in a regularization of the ventricular response (variation coefficient of the ventricular cycle length: less than 10%) in 7 of 13 cases, while atrial fibrillation persisted. During regularization right ventricular extrastimulus testing showed a constancy of the postextrasystolic interval irrespective of the changes in the coupling in

Left ventricular thrombus development during ventricular fibrillation and resolution during resuscitation in a swine model of sudden cardiac arrest.

BACKGROUND: Intracardiac thrombus is a well-known complication of low-flow cardiac states including acute myocardial infarction and atrial fibrillation. Little is known, however, about the formation of intracardiac (left ventricular [LV]) thrombus during the extreme low-flow state of cardiac arrest. OBJECTIVE: Using a swine model of sudden cardiac arrest, we examined the sonographic development of LV thrombus over time after induction of ventricular fibrillation (VF) and resolution of thrombus with cardiopulmonary resuscitation (CPR). METHODS: This observational study was IACUC approved. For…

Bleeding and Thrombosis in Patients With Out-of-Hospital Ventricular Tachycardia/Ventricular Fibrillation Arrest Treated With Extracorporeal Cardiopulmonary Resuscitation.

BACKGROUND: Extracorporeal cardiopulmonary resuscitation improves outcomes after out-of-hospital cardiac arrest. However, bleeding and thrombosis are common complications. We aimed to describe the incidence and predictors of bleeding and thrombosis and their association with in-hospital mortality. METHODS AND RESULTS: Consecutive patients presenting with refractory ventricular tachycardia/ventricular fibrillation out-of-hospital cardiac arrest between December 2015 and March 2022 who met the criteria for extracorporeal cardiopulmonary resuscitation initiation at our center were included. Majo…

Application of CRT-D in a Marfan syndrome patient with chronic heart failure accompanied by ventricular tachycardia and ventricular fibrillation.

Marfan syndrome is a systemic connective tissue disease that could affect the cardiovascular system and eventually lead to heart enlargement and heart failure with high mortality, mainly due to progressive heart failure and/or sudden cardiac death caused by malignant arrhythmia. Here we report that a patient received a cardiac resynchronization therapy-defibrillator (CRT-D) with a pre-monitor function for heart failure and experienced obvious improvements in his cardiac function. Postoperative follow-up showed that the patient had reduced morbidity and hospitalization for heart failure, and al

Electrophysiological characteristics of Purkinje potentials and the conduction system in premature ventricular contractions triggering ventricular fibrillation after myocardial infarction.

AIMS: Catheter ablation for premature ventricular contractions (PVCs) triggering ventricular fibrillation (VF) after acute myocardial infarction (AMI) has proven effective, with Purkinje potentials often serving as the target. However, the electrophysiological features of Purkinje potentials and their relationship with the conduction system in VF-triggering PVCs remain unclear. METHODS: This multicentre retrospective study analysed patients who underwent catheter ablation for VF-triggering PVCs during the acute or subacute phase of AMI. Characteristics of Purkinje potentials, including retrog…

MANAGEMENT OF ATRIAL FIBRILLATION, VENTRICULAR ARRHYTHMIA, LEFT VENTRICULAR DYSFUNCTION, AND SUBSEQUENT CARDIOPATHOLOGIC EVALUATION IN AN ORANGUTAN (PONGO PYGMAEUS × ABELII).

A 37-yr-old male vasectomized hybrid orangutan (Pongo pygmaeus × abelii) was diagnosed with left ventricular dysfunction during a preventative health care examination. Treatment was initiated with carvedilol. The following year, this orangutan was evaluated for intermittent lethargy. Following observation of an irregular cardiac rhythm during an echocardiogram, a lead II electrocardiogram revealed atrial fibrillation and ventricular arrhythmia. Additional treatment included amiodarone, furosemide, spironolactone, clopidogrel, and aspirin. An improved activity level was noted, and follow-up tes

Global Bi-ventricular endocardial distribution of activation rate during long duration ventricular fibrillation in normal and heart failure canines.

BACKGROUND: The objective of this study was to detect differences in the distribution of the left and right ventricle (LV & RV) activation rate (AR) during short-duration ventricular fibrillation (SDVF, <1 min) and long-duration ventricular fibrillation VF (LDVF, >1 min) in normal and heart failure (HF) canine hearts. METHODS: Ventricular fibrillation (VF) was electrically induced in six healthy dogs (control group) and six dogs with right ventricular pacing-induced congestive HF (HF group). Two 64-electrode basket catheters deployed in the LV and RV were used for global endocardium electrica…

Sudden death in mitral valve prolapse with Holter monitoring-documented ventricular fibrillation: evidence of coexisting arrhythmogenic right ventricular cardiomyopathy.

We report the case of a 42-year-old female, affected by mitral valve prolapse and ventricular arrhythmias, who died suddenly from ventricular fibrillation recorded during Holter monitoring. The lethal arrhythmia initiated with late diastolic couplets followed by a ventricular tachycardia that eventually degenerated into ventricular fibrillation. The patient had experienced four orthostatic and stress-related syncopal episodes, associated with nausea and diaphoresis, and a positive tilt test. Holter monitoring documented ventricular arrhythmias, consisting of both isolated monomorphic and spora…

Right ventricular dysfunction in patients with symptomatic atrial fibrillation: Prevalence and functional implications.

BACKGROUND: Assessment of left ventricular (LV) function is an essential component of guideline-recommended management of atrial fibrillation (AF). However, the role and implications of right ventricular (RV) dysfunction in AF are poorly understood. OBJECTIVE: This study aimed to determine the prevalence of RV dysfunction and evaluate its association with cardiorespiratory fitness (CRF) and symptom burden in patients with symptomatic AF. METHODS: Consecutive adults with symptomatic paroxysmal or persistent AF and preserved LV ejection fraction scheduled for catheter ablation underwent transt

Ventricular Fibrillation Without Hemodynamic Deterioration: An Electrocardiographic Paradox Explained by Heterotopic Heart Transplantation.

BACKGROUND: Heterotopic heart transplantation (HHT) is rarely performed in contemporary practice and poses distinct electrophysiological challenges due to the coexistence of the patient's native heart and the transplanted donor organ. CASE SUMMARY: We present the case of a 56-year-old man who experienced recurrent ventricular arrhythmias 6 years post-HHT. The patient initially presented with a syncope and ventricular tachyarrhythmia originating from the donor heart while the native heart maintained sinus rhythm. The arrhythmogenic substrate was attributed to transplant vasculopathy in the don

Refractory Ventricular Fibrillation Associated With a Coronary Vasospasm and Fixed Moderate Coronary Stenosis.

BACKGROUND: A coronary vasospasm is an important but under-recognized cause of ventricular fibrillation, particularly when angiography shows a nonocclusive fixed coronary lesion. CASE SUMMARY: A previously healthy 36-year-old man presented with recurrent early-morning chest pain followed by refractory ventricular fibrillation requiring venoarterial extracorporeal membrane oxygenation. Coronary angiography showed moderate proximal left anterior descending artery stenosis with functionally significant ischemia. However, optical coherence tomography revealed no plaque rupture or thrombus. Becaus…

AF-Triggered Bundle Branch Re-Entrant Ventricular Tachycardia Prevented by Atrial Fibrillation Ablation.

BACKGROUND: Bundle branch re-entrant ventricular tachycardia (BBR-VT) is a macroreentrant tachycardia involving the right and left bundle branches,1 with the right bundle branch typically targeted for ablation. CASE SUMMARY: A 68-year-old man with prior myocardial infarction presented with recurrent ventricular tachycardia (VT). Electrophysiological study suggested BBR-VT. VT was inducible only by atrial pacing and occurred clinically exclusively during atrial fibrillation (AF). Therefore, AF ablation alone was performed, resulting in complete suppression of VT. DISCUSSION: Previous reports

Intradialytic Atrial Fibrillation With Rapid Ventricular Response From Catheter-related Right Atrial Thrombus.

Right atrial thrombus is a rare complication of hemodialysis catheter with an incidence of <6 %. New-onset atrial fibrillation can be the first symptom of catheter-related right atrial thrombus (CRAT) in a patient with long-term dialysis catheter. Therefore, evaluation for CRAT is justified in such scenario. We highlight a case report where a new-onset atrial fibrillation led to the discovery of a right atrial thrombus in a patient with long-term dialysis catheter.
