A systematic review of vertigo in primary care.

The symptom of vertigo is usually managed in primary care without further referral. This review examines the evidence on which general practitioners can base clinical diagnosis and management of this relatively common complaint. Research in this area has in the main been from secondary and tertiary centres and has been of variable quality. Indications are that the conditions that present in general practice are most likely to be benign positional vertigo, acute vestibular neuronitis, and Ménière's disease; however, vascular incidents and neurological causes, such as multiple sclerosis, must be…

Vertigo and the enlarged vestibular aqueduct syndrome.

An enlarged vestibular aqueduct (EVA) is one of the most commonly identified inner ear bony malformations in children with sensorineural hearing loss of unknown cause. Most previous reports have focused on hearing loss, but individuals with EVA may also experience paroxysmal vertigo lasting minutes to hours. We report the clinical vestibular features and vestibular function testing of two children and one adult with EVA who had a history of sensorineural hearing loss and presented to our Neurotology Clinic for the evaluation of episodic vertigo. All the patients had an antecedent history of pr…

A combined approach for the treatment of cervical vertigo.

BACKGROUND: Cervical vertigo is a diagnosis commonly made at both otorhinolaryngologist and chiropractic offices. Hypothesized non-vascular mechanisms are reviewed. Therapeutic approaches have been suggested in the literature, ranging from cervical immobilization to vertebral manipulation. OBJECTIVE: To characterize the patient population with cervical vertigo and observe therapeutic results of a treatment protocol by using distinct conservative modalities. METHODS: Fifteen subjects with cervical vertigo were selected from patients presenting with dizziness at an otorhinolaryngology medical …

Distinguishing and treating causes of central vertigo.

A patient's dizziness can be caused by a peripheral vestibular disorder, VIIIth nerve compression, brain stem ischemia, or cerebellar stroke. Clues from the history and physical examination are mentioned, and diagnostic entities, such as demyelination, cerebrovascular disease, migraine, Arnold-Chiari malformation, cerebellar degeneration, and neoplastic disease are discussed. Treatment options are outlined so that therapeutic and diagnostic trials can be initiated. Guidelines are offered for when to image the brain or posterior circulation vasculature and when a patient with acute vertigo shou…

[The effect of pharmacological treatment in the compensation of vertigo].

From the age of sixty, vertigo is mainly due to vertebro-basilar insufficiency. It has been described that the association of Dihydroergocristine-Piracetam (D-P) is a useful treatment for vertebro-basilar insufficiency. That is why we have designed a comparative study between D-P an a Placebo, so that to prove if this association can be usefull in the treatment of vertigo occasioned by cerebrovascular insufficiency. Fifty patients complaining of vertigo were included in the study after an untreated term. 19 received a daily capsule of Placebo, and the other 31, treated with D-P, were divided i…

Paroxysmal vertigo in children--an epidemiological study.

BACKGROUND: Little is currently known about the prevalence of vertigo in children. METHODS: In a questionnaire designed to examine the prevalence of migraine and migraine equivalents in children of school age, we included an item on 'attacks of dizziness in the past year'. The questionnaire was applied to 2165 children (10% of the school population in the city of Aberdeen, Scotland). RESULTS: 314 children had experienced at least one episode of dizziness in the previous year, unexplained in 44% of cases. A total of 57 children with three attacks, either unexplained or attributed to migraine,…

Correlates of vertigo attacks in Ménière's disease.

BACKGROUND: Ménière's disease is a medical condition that involves hearing loss, tinnitus and attacks of vertigo. The attacks can be severely disabling with nausea, dizziness, and aural sensations. METHOD: Three scales assessing the correlates of vertigo attacks in Ménière's disease were developed and completed by 514 subjects diagnosed with the disease. The three scales measured were somatic sensations (SOM), psychological state (PSYCHOL), and situational characteristics (SIT) associated with an attack. RESULTS: Psychometric properties of the three scales were investigated showing Cronbach'…

Lyme borreliosis--an unusual cause of vertigo.

A total of 2055 consecutive vertigo patients were examined in a prospective study in an area endemic for Lyme borreliosis for clinical signs of Lyme borreliosis or serum antibodies against Borrelia burgdorferi. Of these, 41 patients (2%) had positive levels of serum antibodies against B. burgdorferi. The incidence of seropositivity against B. burgdorferi among the vertigo patients did not differ from the incidence of the normal Finnish population. In addition to seropositivity the criteria used for Lyme borreliosis included previous erythema migrans, a positive polymerase chain reaction (PCR) …

Course of illness in phobic postural vertigo.

Forty-two patients with phobic postural vertigo took part in a neurological and psychiatric follow-up study. During the follow-up time of about 2.5 years the neurological diagnosis remained stable (41 of 42 patients). PPV can be assigned to various psychiatric categories according to DSM-III-R. Although an association of PPV with anxiety disorders is evident, not all patients present with symptoms of anxiety or panic during attacks of vertigo. However most patients develop a disabling "phobic-avoidance pattern" with recurrent attacks. Important psychosocial stressors can be identified at the o…

SPECT of Brain and Vertigo - A Case Report.

Vertigo has been identified as a complaint associated with the symptom of tinnitus; as well as a specific clinical type of tinnitus i.e., vestibular tinnitus. Classically, the clinical diagnosis of Ménière's Disease includes a quadrad of complaints, highlighted by episodic vertigo, ear blockage, a gradual progressive sensorineural hearing loss, and an associated complaint of a "low frequency tinnitus". Diagnostic and etiologic difficulties for Ménière's Disease have resulted in multiple diagnostic entities, e.g., Ménière's Syndrome, Cochlear Ménière's Disease; Vestibular Ménière's Disease, etc…

[Autonomic failure as the cause of vertigo and syncope].

BACKGROUND: Vertigo and syncope often occur during orthostatic conditions. A possible cause is asympathicotonic dysregulation with a fall of blood pressure and no increase in heart rate. After the exclusion of secondary disturbances of autonomic innervation. g. in diabetes mellitus, and pharmacological side effects, e.g. tricyclic antidepressants, primary reasons must be considered. We present 2 cases of primary autonomic failure. PATIENTS: Patient 1 had Parkinson-syndrome, impaired bladder function and orthostatic dysregulation with a maximal drop of blood pressure to 60/40 mm Hg. Patient 2 …

Tympanic membrane temperature in a patient with vertigo.

Tympanic membrane temperature (TMT) is greatly influenced by the internal carotid artery and the jugular vein. Average human brain temperature is influenced by the differences between the temperature of the internal carotid artery and of the jugular vein. TMT has been used extensively as an indicator of brain temperature. Tympanic membrane temperature was measured in six healthy men (control group) and one patient with vertigo. In the control group, TMT showed considerable circadian changes parallel to the core temperature. In the patient with vertigo, TMT varied widely after the attack of ver…

Bilateral vestibular neurectomy for treatment of vertigo.

The effects of bilateral vestibular neurectomy on equilibrium and vestibular function were clinically evaluated in two patients more than 15 years after surgery. Both patients had bilateral Menière's disease and their vertiginous spells were permanently resolved after the second vestibular neurectomy. Symptoms of disequilibrium were absent in one patient and mild in the other. Reflexive horizontal eye movements on whole body rotation in darkness were absent on low angular accelerations (2 degrees/s2), but could be elicited with angular accelerations of 20 degrees/s2 or higher. Extravestibular

Tinnitus and vertigo in patients with temporomandibular disorder.

The association of tinnitus and vertigo with temporomandibular disorder (TMD) has been debated for many years. The observation that patients with TMD have otologic symptoms is confounded because tinnitus and vertigo are common symptoms in the normal population. The present study was conducted to determine if tinnitus and vertigo are actually more prevalent in patients with TMD than in appropriate age-matched controls. One control group was recruited from patients seeking care for health maintenance and the other from patients seeking routine dental care. We surveyed 1032 patients: 338 had TMD …

[Vertigo in patients with otosclerosis and post stapedectomy].

The aim of the paper was evaluation of behaviour of the balance system in patients with clinical otospongiosis and after stapedectomy. The investigation was carried out in 131 otosclerotic patients. Before and post operation vertigo was noticed and the spontaneous nystagmus, positional nystagmus and directional preponderance in kinetic test were examined in ENG. Vertigo and objective symptoms of disturbance of the vestibular function were found in clinical otospongiosis, but postoperative subjective and objective symptoms were more frequently.

Benign paroxysmal vertigo of the horizontal canal.

Over a period of 4 years we observed 15 cases of benign positional vertigo (BPV) probably caused by deposition of otoliths in the horizontal semicircular canal. Rapid rolling onto one side in recumbent position provokes a paroxysmal, purely horizontal and geotropic nystagmus which nearly always spontaneously inverts direction. Rolling the patient onto the other side provokes a left intense geotropic nystagmus. BPV is more violent but resolves more rapidly than that of the better known posterior canal positional vertigo. Sometimes both syndromes are present together.

Calcium entry blockers in the treatment of vertigo.

Few calcium entry blockers have been studied in the treatment of vertigo. Flunarizine and cinnarizine are the most extensively studied substances. They have been shown to be valuable drugs in the therapeutic approach to vertigo. A few pilot studies have been performed with nimodipine but controlled data are lacking. Flunarizine has a direct long-lasting vestibulodepressant effect in both animals and humans. In patients with labyrinthine vertigo, it produced a marked symptomatic relief of symptoms significantly exceeding the effects of placebo or nicergoline. A number of studies dealing with ve…

Vertigo in postoperative follow-up of otosclerosis.

Causes of vertigo after otosclerosis surgery were studied postoperatively and in long-term follow-up examinations. Pressure and mobility changes in the posterior labyrinth fluids, enzymatic process, and decrease in blood supply at the time of operation appear to be the major causes. Methods of detection, avoiding, and managing vertigo are presented.
