Treatment of Ménière's disease by low-dosage intratympanic gentamicin application: effect on otolith function.

OBJECTIVES: The intratympanic application of a low dosage of gentamicin is increasingly favored as treatment for Ménière's disease. While posttreatment observations have confirmed a long-term success of the therapy of vertigo attacks, clear differences in the posttreatment recovery interval can be observed. In addition to differences in central-vestibular compensation, the degree of peripheral vestibular damage, i.e., to the saccule, utricle, and semicircular canal ampullae, varies among patients. This study provides comprehensive pre- and posttreatment results from unilateral functional tests

Verticality perception in patients with active multiple sclerosis: The applicability of subjective visual vertical test and its modifications.

BACKGROUND: Dizziness and balance disturbances are common in patients with MS. Subjective visual vertical (SVV) is a test of vestibular perception that allows clinicians to evaluate the integration of multiple sensory inputs for spatial orientation in the CNS. We hypothesize that central vestibular impairment caused by active MS lesions may be reflected in the modified SVV testing. OBJECTIVE: To evaluate the applicability of dynamic and head-tilt SVV for detecting spatial orientation disturbances in MS patients and compare it with conventional SVV testing. METHODS: The SVV test was conducted

Application of supervised machine learning algorithms for the evaluation of utricular function on patients with Meniere's disease: utilizing subjective visual vertical and ocular-vestibular-evoked myogenic potentials.

BACKGROUND: Research on the otolith organs remains inconclusive. OBJECTIVES: This study seeks to further elucidate utricular function in patients with Meniere's disease (MD) in three ways: (1) We aimed to disambiguate the role of the Subjective Visual Vertical (SVV) and Ocular Vestibular Evoked Myogenic Potential (o-VEMP) tests regarding which utricular subsystem each is measuring. (2) We sought to characterize the acute and chronic state of MD by identifying differences in the relationship of SVV and o-VEMP results across patients with acute and chronic MD. (3) We attempted to find a machine

Evaluation of the Video Ocular Counter-Roll (vOCR) as a New Clinical Test of Otolith Function in Peripheral Vestibulopathy.

IMPORTANCE: Video-oculography (VOG) goggles have been integrated into the assessment of semicircular canal function in patients with vestibular disorders. However, a similar bedside VOG method for testing otolith function is lacking. OBJECTIVE: To evaluate the use of VOG-based measurement of ocular counter-roll (vOCR) as a clinical test of otolith function. DESIGN, SETTING, AND PARTICIPANTS: A case-control study was conducted to compare vOCR measurement among patients at various stages of unilateral loss of vestibular function with healthy controls. The receiver operating characteristic curv

The perception of body verticality (subjective postural vertical) in peripheral and central vestibular disorders.

The perception of body verticality (subjective postural vertical, SPV) was assessed in normal subjects and in patients with peripheral and central vestibular lesions and the data were compared with conventional neuro-otological assessments. Subjects were seated with eyes closed in a motorized gimbal which executed cycles of tilt at low constant speed (1.5 degrees s-1), both in the frontal (roll) and sagittal (pitch) planes. Subjects indicated with a joystick when they entered and left verticality, thus defining a sector of subjective uprightness in each plane. The mean angle of tilt (identifyi

Dyscalculia in patients with vertigo.

This paper reports a hitherto undescribed relationship between vertigo of central origin and dyscalculia. Subjects with vertigo skipped and displaced decades when counting backwards by two. The error is not recognized when presented visually. The subjects also display decrements in ability to do mental arithmetic and in central auditory processing. The results are discussed in light of the relationship between the central vestibular/auditory system and structures involved in higher cognitive function. The relationship between balance disorders and children with learning disabilities is also ex

Changes in patient-perceived balance and vestibular function after Gamma-knife stereotactic radiosurgery for vestibular schwannoma: 12-month outcomes in a single-centre pilot study.

BACKGROUND: Hearing outcomes are well-documented for patients with vestibular schwannoma (VS) treated with Gamma Knife stereotactic radiosurgery (GK-SRS). However, how GK-SRS affects patient-perceived balance and vestibular function remains unclear. This study therefore evaluated changes in these parameters one-year post-treatment. METHODS: A prospective, observational, before-and-after, pilot study was conducted on patients with unilateral VS treated with GK-SRS between June 2021 and July 2022. Balance-related handicap was assessed using the Dizziness Handicap Inventory (DHI). Objective vest

Changes in Vestibular Function Following Pediatric Cochlear Implantation: a Prospective Study.

OBJECTIVES: Given the close interconnection between the auditory and vestibular end organs, the increasingly broad application of (bilateral) cochlear implantation (CI) in children raises concern about its impact on the vestibular function. Unfortunately, literature on this matter is inconclusive and subject to several limitations. Therefore, this study aimed to elucidate the impact of pediatric CI on the vestibular function in a large sample of children, representative for the current CI population. DESIGN: Fifty hearing-impaired children followed in the Ghent University Hospital were includ

Selective vestibular neurolabyrinthitis of the lateral and superior semicircular canal ampulla and ampullary nerves.

OBJECTIVES: The diagnosis of vestibular neurolabyrinthitis is based on the sudden appearance of vertigo that lasts for hours or days without associated cochlear or central nervous system signs or symptoms. The advent of the video head impulse test, cervical vestibular evoked myogenic potential testing, and ocular vestibular evoked myogenic potential testing has provided interesting clinical evidence for evaluating and monitoring the damage to specific compartments of the vestibular apparatus. These various methods of testing individual end-organ functions may have a clinical impact on the vest

Dynamic visual acuity testing for screening patients with vestibular impairments.

Dynamic visual acuity (DVA) may be a useful indicator of the function of the vestibulo-ocular reflex (VOR) but most DVA tests involve active head motion in the yaw plane. During gait the passive, vertical VOR may be more relevant and passive testing would be less likely to elicit compensatory strategies. The goal of this study was to determine if testing dynamic visual acuity during passive vertical motion of the subject would differentiate normal subjects from patients with known vestibular disorders. Subjects, normals and patients who had been diagnosed with either unilateral vestibular weak

Comorbid intracranial vertebral artery asymmetry as a risk factor for severe vestibular neuronitis.

BACKGROUND: Our previous study had demonstrated that verterbral artery hypoplasia (VAH) contribute to ipsilateral vestibular dysfunction. The aim of this study was to test if VAH contributes to prognosis of vestibular neuronitis (VN) through presumed regional malperfusion. METHODS: We performed a prospective magnetic resonance angiographic registry in patients with acute vestibular neuritis in which were then assigned to VAH (n=29) and control group (n=40). Vestibular function was determined by caloric irrigation, with the use of the vestibular paresis formula (to measure the extent of unilat

Vestibular evaluation in Behçet's disease. Personal experience.

Few reports have appeared in the literature concerning vestibular findings in Behçet's disease. In the present study, extensive vestibular testing, performed in 14 patients (8 male, 6 female; mean age: 32 years; range: 12-51) presenting definite Behçet's disease, revealed a high prevalence of central vestibular dysfunctions (78%). Data reported here suggest that an otoneurological evaluation of Behçet's disease patients may be helpful in identifying unexpected vestibular dysfunctions and central nervous system involvement different from the classical manifestations of the neuro-Behçet's syndro

[The diagnostic value of the Stepping Test of Unterberger (author's transl)].

Of the vestibulo-spinal reactions, the "stepping test" is considered to be the most important, especially for unilateral peripheral vestibular disturbances. Electronystagmography was performed on seven patients after transtemporal neurectomy of the vestibular nerve, 17 patients with a nearly compensated vesitbular defect after vestibular neuronitis, and seven patients after suboccipital removal of acoustic neuromas. These results were then compared to the spontaneous stepping test. The test could not be performed on six of the seven patients with the operated acoustic neuroma, and on three oth
