Comparative Study Between the Auditory and Vestibular Functions in Ménière's Disease.

OBJECTIVES: To evaluate the relationship between the loss of the cochleovestibular functions in Ménière's disease (MD). METHODS: Forty-three patients with definite MD underwent pure-tone audiometry (PTA) and caloric test. Canal paresis (CP%), dPTA (interaural difference), and average PTA results were contrasted. IBM SPSS V24 was used for statistical analysis. RESULTS: According to PTA, most patients were in stage C, and caloric weakness was found in 29 patients. Linear (R2 = 0.06) and nonlinear correlation tests (rho = 0.245, P = .113) between canal paresis (CP%) and dPTA showed no correlati

Cochlear morphology in the developing inner ear of the porcine model of spontaneous deafness.

BACKGROUND: Auditory function and cochlear morphology have previously been described in a porcine model with spontaneous WS2-like phenotype. In the present study, cochlear histopathology was further investigated in the inner ear of the developing spontaneous deafness pig. RESULTS: We found that the stria vascularis transformed into a complex tri-laminar tissue at embryonic 85 days (E85) in normal pigs, but not in the MITF-/- pigs. As the neural crest (NC) of cochlea was derived by melanocytes. MITF mutation caused failure of development of melanocytes which caused a subsequent collapse of coc

Growth rate of vestibular schwannoma.

Vestibular schwannoma (VS) is the most common tumor in the extra-axial posterior fossa compartment in adults. Growth rate is paramount to decision making regarding treatment and follow up of these tumors. We conducted a comprehensive review of the literature to answer four questions: What percentage of newly diagnosed VS will grow on follow-up? What factors correlate to tumor growth? What is the "normal" growth rate for sporadic VS? What factors characterize VS with rapid growth? Thirty-seven reports, with more than 4000 patients, fit our review criteria. One third of newly diagnosed VS will g

Use of betahistine in the treatment of peripheral vertigo.

CONCLUSION: Clinical studies and meta-analyses demonstrated that betahistine is effective and safe in the treatment of Ménière's disease, BPPV (benign paroxysmal positional vertigo), vestibular neuronitis, and other types of peripheral vertigo. OBJECTIVES: The goal of this paper is to review the pharmacological profile of betahistine and the evidence for its effectiveness and safety in the treatment of peripheral vertigo. METHODS: Selection criteria for the publications on betahistine included randomized clinical trials that evaluated the effectiveness and safety of betahistine vs placebo or

[Features of the psychophysiological status of men with different levels of vestibuloautonomic resistance and their interrelation with motion sickness' etiology and pathogenesis].

The results of more than 600 observations of 200 healthy male-volunteers of 23-45 years old including parameters of electroencephalogram, sensorimotor reactions, and questionnaires of Cattell, Strelau and Taylor were analyzed. Division into groups with high, moderate and low vestibuloautonomic resistance (VAR) was done on base of endurance of rotary test with interrupted cumulation of Coriolis accelerations. It is shown, that VAR degree along with distinctions in sensitivity of peripheral compartment of a vestibular analyzer and other sensory systems is appreciably defined by the distinctions

Vestibular "PREHAB".

A sudden unilateral loss or impairment of vestibular function causes vertigo, dizziness, and impaired postural function. In most occasions, everyday activities supported or not by vestibular rehabilitation programs will promote a compensation and the symptoms subside. As the compensatory process requires sensory input, matching performed motor activity, both motor learning of exercises and matching to sensory input are required. If there is a simultaneous cerebellar lesion caused by the tumor or the surgery of the posterior cranial fossa, there may be a risk of a combined vestibulocerebellar l

Vestibulotrigeminal pathways in the frog, Rana esculenta.

The aim of this study was to investigate whether primary vestibular afferent fibers establish direct connections with the motor and sensory trigeminal system in the brainstem of the frog. The experiments were carried out on Rana esculenta. In anaesthetized animals the trigeminal and vestibular nerves were prepared, and their proximal stumps were labeled either with fluorescein binding dextran amine (trigeminal nerve) or tetramethylrhodamine dextran amine (vestibulocochlear nerve). With a confocal laser scanning microscope we could detect close connections between the vestibular fibers and bran

Idiopathic paroxysmal kinesigenic choreoathetosis: precipitation of attacks by vestibular stimulation.

A 24-year-old woman presented with a 3.5-year history of paroxysmal dystonia that was precipitated by sudden movement, especially when she started to walk. It was characterised by shrugging of shoulders, flexion of the neck and thoracic spine, and stiffness of the right leg followed by falls. Each attack lasted for less than 5min. Inadequate sleep and stress were exacerbating factors. There was no similar family history. Physical examination and investigations were normal. The following manoeuvres that caused vestibular stimulation precipitated attacks: turning her head from side to side while

The pathophysiology of primary dystonia.

Co-contraction and overflow of EMG activity of inappropriate muscles are typical features of all dystonic movements whether voluntary or involuntary. Voluntary movements are slow and more variable than normal, and there is particular difficulty switching between component movements of a complex task. Reduced spinal cord and brainstem inhibition is common to many reflex studies (long-latency reflexes, cranial reflexes and reciprocal inhibition). These reflex abnormalities may contribute to the difficulties in voluntary movements but cannot be causal as they can occur outside the clinically invo

[Selective vestibular neurectomy: a comparison of techniques].

The surgical treatment of patients suffering from incapacitating peripheral vertigo is still controversial. In the ENT Department of the Civil Hospital of Venice 43 patients with disabling menieric vertigo spells underwent a selective vestibular nerve section employing either a retrosigmoid (25 cases) or a retrolabyrinthine approach (18 cases) between 1991 and 1993. The two approaches are compared with regard to surgical technique, pre- and postoperative complications and i.e. functional results, i.e. vertigo control and hearing preservation. Functional results, classified according to the gui

Alteration in alkaline phosphatase activity of duodenal tissue following vestibulo-cerebellar modulation.

Effect of electrolytic lesion in vestibulo-cerebellar area of male Wistar rats was investigated on duodenal alkaline phosphatase (AP) activity. Histomorphological observations of brain indicated, that extensive lesion restricted to the wide area of nodule including ventral uvula, resulted in significant fall of AP activity at the mucosal brush border within 20 days. A comparative behavioural study in experimental and control groups by openfield test indicated that, sympathoexcitation resulting from vestibulo-cerebellar lesion may contribute to significant alteration of AP activity in duodenum

Depersonalization, vertigo and Ménière's disease.

Ménière's disease is generally accepted to be a consequence of distention of the endolymphatic sac of the inner ear. Although the exact etiology is unclear, there is a body of research suggesting that Ménière's disease is of psychosomatic origin. While we do not intend to review the literature exhaustively, we briefly review several frequently cited studies. Even though this literature is so severely flawed that no solid conclusions may be drawn from it, certain questions about the psychological aspect of this disorder continue to surface. Our limited access to patients with vertigo has not al

[Quantifying the stimulus parameters in the neck torsion test].

Cervically induced eye movements can be observed when the trunk is rotated and the head fixed. In most test situations, rotating chair motion and head fixation are carried out manually. As a result, specific and non-specific sources of error are involved in these procedures that hamper the reproducibility of test conditions. A system is described that provides quantifiable and reproducible, electronically controlled movements of the rotating chair. Ramp and rectangular stimulus profiles are achieved by infrared optical gate devices on the chair axis. Various processor-controlled sequences of a

Drop attacks in Menière's disease.

An inner ear etiology for drop attacks is easily overlooked and seldom mentioned in the literature. Cardiovascular pathology is what generally is looked for, e.g. transitory ischemic attacks. Twenty-nine patients with frequent and severe Meniere attacks were treated with gentamicin intratympanally, when they had reached a stage of incapacitation due to vertigo and dread of further attacks and all medical treatments had failed. All but one patient were suffering from more or less frequent drop attacks. The attacks of vertigo were successfully cured in all patients except 2, who had recurrence o

Vertigo. Current concepts in management.

Vertigo is not a disease, but a symptom. The management of vertigo requires more than the treatment of the symptom alone, as it concerns the whole physical and psychological condition of the patient. In addition to causal treatment, symptomatic treatment is needed in many cases. This specific treatment consists of vestibular exercises and drug therapy. The efficacy and use of the different drugs available are discussed.
