Diagnosis of Extrinsic Upper Esophageal Compression Utilizing Video Laryngoscopy in an Infant Following Failed Transesophageal Echocardiogram Probe Placement.

Anesthesiologists are frequently responsible for placement of transesophageal echocardiography probes prior to cardiac surgery in children. A number of potential complications are possible, including placement failure. This report documents one such failed attempt at probe placement in a 3-month-old patient with a history of ventricular septal defect, and the utilization of video laryngoscopy by the anesthesiologist to diagnose a previously unknown extrinsic esophageal compression likely caused by an aberrant right subclavian artery. This case highlights the multiple vascular anomalies that ma

A randomized controlled trial of an online educational video intervention to improve glaucoma eye drop technique.

OBJECTIVE: To determine the effectiveness of an online video intervention in improving self-efficacy and eye drop application technique in glaucoma patients. METHODS: We randomized ninety-two patients with primary open-angle glaucoma, all who self-administer their eye drops, to either watch the 4-minute Meducation® eye drop technique video in the intervention group, or a nutrition video in the control group. We assessed five eye drop technique steps using objective video recordings at baseline, immediately after watching the video, and 1 month later. We used linear regression models to determ

Impact of alcohol-related video sequences on functional MRI in abstinent alcoholics.

The object of this study was the identification of brain areas that were significantly more connected than other regions with a previously identified reference region, the posterior cingulate cortex, during the presentation of visual cues in alcoholics. Alcohol-related and neutral video sequences were presented to 30 alcoholics who had been abstinent for at least 4 days. Participants underwent a psychometric assessment before and after the presentation of the video sequences. Functional MRI data were acquired. Psychophysiological interaction analyses were carried out. Participants reported a s

Orgasm-associated incontinence (climacturia) after bladder neck-sparing radical prostatectomy: clinical and video-urodynamic evaluation.

INTRODUCTION: Orgasm-Associated Incontinence (OAI) or climacturia has been observed in male patients maintaining sexual potency after radical prostatectomy and cystectomy. AIM: We investigated the incidence and video-urodynamic aspects of this event in continent and potent patients after bladder neck-sparing (BNS) radical prostatectomy (RP). MAIN OUTCOME MEASURE: Comparing functional and morphological aspects between climacturic and non-climacturic patients to identify a possible explanation of this unusual kind of leakage that could seriously impact the sexual life after surgery. METHODS:

Supine head tremor: a clinical comparison of essential tremor and spasmodic torticollis patients.

BACKGROUND: Essential tremor (ET) is among the most misdiagnosed neurological diseases, and overdiagnosis is especially common. As many as 30-50% of supposed 'ET' cases have other diagnoses, with dystonia prominent among these. Therefore, the recognition of differences in tremor phenomenology has potential diagnostic value for practising clinicians. There is an anecdotal sense that head tremor in ET is positional rather than resting, a feature which could aid in the diagnosis, yet no published data exist. METHODS: In this cross-sectional study of ET and spasmodic torticollis (ST) patients (3:

Pragmatic and idiosyncratic acts in human everyday routines: the counterpart of compulsive rituals.

Our daily activities are comprised of motor routines, which are behavioral templates with specific goals, typically performed in an automatic fixed manner and without much conscious attention. Such routines can seem to resemble pathologic rituals that dominate the motor behavior of obsessive-compulsive disorder (OCD) and autistic patients. This resemblance raises the question of what differentiates and what is common in normal and pathologic motor behavior. Indeed, pathologic motor performance is often construed as an extended stereotyped version of normal everyday routines. In this study we a

The hazards of using floor mats as a fall protection device at the bedside.

OBJECTIVES: In this study, we evaluated the clinical safety of the floor mat, a device intended to reduce severity of injury in falls from the bed or at the bedside. METHODS: Fifteen participants with a range of gaits and fall risk were video-recorded as they approached and entered a hospital bed and then as they exited and walked away. For 10 participants, a 1-inch-thick floor mat with a beveled edge was used as a fall protection device at the bedside. The videos were coded and analyzed for the purpose of determining the mechanisms by which the floor mat affected balance and gait while parti

In their own words: the reasons why people will (not) sign an organ donor card.

This multisite, qualitative study of 78 family-pair dyads provides rich data on the reasons people cite for (not) wanting to sign an organ donor card in the context of family conversations. In this study, dyads were videotaped as they discussed 8 questions pertaining to their views on organ donation, beginning with the most general opinions and progressing to more detailed questions. Analysis of the transcribed data revealed that the most common reasons for wanting to donate organs were based on religion or a desire to help other people in need. The most common reasons cited for not wanting to

Paroxysmal tonic upgaze of childhood with co-existent absence epilepsy.

Paroxysmal tonic upgaze (PTU) is a childhood oculomotor syndrome of unclear etiology characterized by episodic tonic upward eye deviation with neck flexion. Neuroimaging findings are often normal and the electroencephalography during episodes is typically normal. We describe a 2-year-old boy who presented with macrocephaly, hypotonia, developmental delay and episodes of eye fluttering, head nodding and unresponsiveness. Video-EEG captured absence seizures and he was treated with valproate, which led to improvement of his seizures. However, two weeks after treatment, he developed paroxysmal epi

Functional MRI changes before and after onset of reported emotions.

The social nature of emotion is evident in the importance of facial and vocal displays in emotion-related behavior. This is the first brain-imaging study to use simulated face-to-face social interactions to evoke emotional responses and to compare valence-related activations before and after subjective onset of emotional response. Videotapes were prepared with actresses who described happy or unhappy experiences. Functional magnetic resonance imaging (fMRI) at 1.5 T was used to acquire BOLD images in 21 healthy young adults before, after, and during viewing of the happy and sad tapes. Subjects

Determining the etiology of mild vocal fold hypomobility.

The prevalence of mild vocal fold hypomobility is unknown. In a study by Heman-Ackah et al, vocal fold hypomobility in a population of singing teachers was found to be associated more frequently with vocal complaints than was the presence of vocal fold masses. The etiology of mild vocal fold hypomobility has not been previously explored. In the present study, a retrospective chart review was performed of 134 patients who presented to a tertiary laryngology referral center over a 6-month period for evaluation of vocal complaints. Of the 134 patients, 61 (46%) were found to have mild vocal refer

Concussion in professional football: reconstruction of game impacts and injuries.

OBJECTIVE: Concussion in professional football was studied with respect to impact types and injury biomechanics. A combination of video surveillance and laboratory reconstruction of game impacts was used to evaluate concussion biomechanics. METHODS: Between 1996 and 2001, videotapes of concussions and significant head impacts were collected from National Football League games. There were clear views of the direction and location of the helmet impact for 182 cases. In 31 cases, the speed of impact could be determined with analysis of multiple videos. Those cases were reconstructed in laborator

The effect of situational structure on the social performance of socially anxious and non-anxious participants.

Twenty-six socially anxious and 24 low-anxious female undergraduate students were observed in brief unstructured and structured hetero-social interactions with a confederate. The unstructured interaction was a naturalistic interaction in which participants were observed surreptitiously. The structured interaction was a role-play in which participants were instructed to try and get to know as much as possible about their partner. Videotapes of the interactions were subsequently rated on subjective and objective measures of social skill. The results showed that high socially anxious females perf

Spontaneous leg movements in infants with and without periventricular leukomalacia: effects of unilateral weighting.

The present study was designed to investigate the contribution of the corticospinal tracts in the regulation and coordination of interlimb couplings and the spatio-temporal organization of kicking movements in young infants. Both healthy infants and those with differing degrees of periventricular leukomalacia (PVL) were subjected to a unilateral weight manipulation at the (corrected) age of 26 weeks. Infants with PVL were grouped according to the amount of damage in the area in which the corticospinal tracts are located as shown by neonatal MRI and confirmed with MRI recordings at 18 months. T

Media-based behavioural treatments for behavioural disorders in children.

BACKGROUND: Prevalence studies show that behaviour problems in children are quite common (10-15% in preschoolers). These problems may manifest as, for example anxiety, sadness, over-activity and tantrums. Some studies have shown that these problems can be persistent, and that they lead to a range of problems in adolescence and adulthood. Many approaches are used to address behavioural problems such as medication, or more usually, psychological treatments either directly with the child and/or his/her family. Behavioural interventions have been shown to be highly effective but access to these tr
