On the basis of varying morphology and pathogenesis, two types of vulvar intraepithelial neoplasias (VIN) have been defined: the common type (approximately 98%), classic VIN, is characterised by strong association to high-risk HPV infection (up to 90%), occurrence at younger age (median age 30-40 years) and multifocality. The differentiated (or simplex) type is rare (1%-2%) and is associated with older age (median age 65 years) and p53 alterations. It is usually diagnosed in combination with vulvar (keratinizing) squamous cell carcinoma. The classification currently preferred by the WHO in whi

The authors report the case of a degenerative Buschke-Löwenstein tumor located on the vulva. The authors stress the unusual nature of this disorder, recall its etiopathogenic factors and insist on the necessity for surgery to avoid a transition to malignancy.

Vulval squamous cell carcinoma is relatively rare; however, up to 20% of women have significant vulval symptoms during their lifetime. Formal screening programmes for vulval disease have not been established. The evidence for the use of vulval cytology and vulvoscopy is reviewed. No randomised-controlled trials have compared follow-up regimens, and although a few consensus documents have been published, formal guidelines are lacking in Grade A evidence. With increasing pressure on healthcare resources, the possibility of identifying high-risk groups to optimise the use of follow up in speciali

Vulvar squamous cell carcinoma (SCC) develops following two different pathways, which have their own premalignant lesions. In the absence of human papilloma virus (HPV), vulvar SCC can develop in a background of lichen sclerosus (LS), differentiated vulvar intraepithelial neoplasia (VIN) or both. The other pathway leading to vulvar SCC is associated with HPV and the HPV-associated premalignancy is usual VIN. In this review we will discuss the history, epidemiology, aetiology, histology, clinical characteristics, treatment options, malignant potential and prevention strategies of the three squa

Kaposi sarcoma is a vascular neoplastic disorder that is associated with the acquired immunodeficiency syndrome (AIDS). The causative factor in Kaposi sarcoma is human herpes virus-8. This complication of AIDS has a predilection for homosexual males and is rarely associated with the female AIDS population. However, we present a case of Kaposi sarcoma mimicking the benign and common Bartholin gland abscess. A search of the literature including the MEDLINE database revealed a single report of Kaposi sarcoma presenting as a vulvar mass, but not as a suspected Bartholin abscess. The history, prese

We developed a simple menu driven database on a personal computer for classification and documentation of vulvar lesions (IBM-PC, dBaseII). Vulvar lesions were classified according to morphological appearance: red, white, dark, ulcer, small tumor (less than 1 cm), large tumor. The data for each category were entered in multiple choice form or as free text and stored in a separate database file. Overlap between categories was observed in vulvar intraepithelial neoplasia, invasive carcinoma and between large and small tumors. For each category an automated table of the frequencies of the differe

Fifty women whose clinical vulvar appearance was compatible with that of hyperplastic or mixed vulvar dystrophy were evaluated in the Vulvovaginal Referral Unit, University of Florida, from 1980 through 1986. Histologic material from biopsies performed on these patients was reviewed. The histologic picture was consistent with the clinical diagnosis in 33 cases. Fifteen patients had lichen sclerosus with various degrees of hyperkeratosis, while one had human papillomavirus-associated vulvar intraepithelial neoplasia and another had only mild chronic inflammation. Three patients in the mixed dys

Vulvovaginal melanomas are rare cancers of the vulva and vagina that are often treated similarly despite important differences. Furthermore, although these melanomas are generally considered mucosal, due to their rarity and limited clinical trial data and level I evidence they are often treated with cutaneous melanoma strategies. Resection of the tumor, with or without lymph node assessment, is the preferred primary treatment approach for these cancers. Breslow thickness is a main determinant of surgical approach, and a negative excision margin should be the goal, if possible, without pelvic e
