Klebsiella pneumoniae Lung Abscess Due to Aspiration of Seawater During Recreational Free Diving.

Cough persisting for 5 wk, low-grade fever, and thick, pinkish to reddish, foul-tasting sputum made the doctors seeing this patient think of tuberculosis at first. However, a history of aspirating seawater during recreational free diving gave a clearer picture of how this patient got sick. Investigations later revealed a cavitary lesion in the right lung, and sputum culture grew Klebsiella pneumoniae, resulting in a diagnosis of lung abscess. This case is being reported to show this peculiar way that a bacterial pathogen entered the respiratory tree and that recreational free diving poses a he

A 4-Day Exposure to High Altitude Prolongs QTc in Healthy Human Subjects.

IntroductionLong QTc syndrome can predispose patients to fatal ventricular arrhythmias. We studied the effects of a rapid ascent and a multiday stay at high altitude on QTc interval.MethodsWe recorded electrocardiograms (ECGs) on study subjects at sea level and then again after a 1-d ascent to 3800 m. Two sea-level ECGs were recorded at the initial screening visit (SL1) and 14 days later (SL2). Altitude ECGs were recorded at approximately 16 and 86 h after arrival. SpO2 was recorded prior to each ECG measurement. We defined prolonged QTc as a QTc interval >450 ms for men and >470 ms for women.

Review of Preexisting Medical Conditions in Persons Traveling to High Altitude, 2025.

As the global population grows and access to remote environments expands, travel to high altitude destinations has become increasingly common, now attracting ∼100 million visitors annually. This trend coincides with a population that is increasingly medically complex, emphasizing the importance of understanding how preexisting medical conditions are influenced by ascent to high altitude. This narrative review summarizes the current evidence describing the physiologic and clinical implications of altitude exposure among individuals with chronic medical conditions. A comprehensive literature sea

Simulated Avalanche vs Tree-Well Burial Effects on Human Physiology.

IntroductionAvalanches and tree-wells (TW) are causes of snow immersion death in skiers and snowboarders. TW burials may have different victim physiology profiles and rescue considerations than avalanche-related burials due to snow density, burial position, and compensatory physiologic responses. This study aimed to compare the physiological responses between avalanche and TW burials through a prospective controlled trial of simulated avalanche burial to simulated TW burial.MethodsEleven volunteers participated in two paired 60-min snow burials: Avalanche and TW simulations, controlling for po

Minor Decrease of Core Temperature in Shivering Volunteers Over a 3-Hour Exposure to Cold, Wet, and Windy Conditions.

ObjectiveHypothermia increases mortality among trauma patients. It is difficult to identify hypothermia in conscious patients due to the limited availability of diagnostic equipment yielding accurate measurements. Shivering is a symptom associated with hypothermia, and it serves as a parameter for clinical staging of hypothermia. Because shivering is part of the body's response to counteract hypothermia, the resulting thermogenic effect and expected cooling rates in shivering individuals warrant further investigation. This study aimed to describe core temperature trajectories in a setting of c

Inhaled Epoprostenol Augments Cold-Induced Vasodilation: A Double-Blind Crossover Trial.

IntroductionCold exposure initially induces peripheral vasoconstriction. After 5-10 min, distal blood vessels will transiently and cyclically vasodilate and vasoconstrict in a phenomenon known as cold-induced vasodilation (CIVD). Increased CIVD response is thought to improve dexterity and confer a reduced risk of frostbite injuries. Current guidelines recommend the use of an intravenous synthetic prostacyclin (PGI2) analog for the treatment of some cases of severe frostbite. This double-blind crossover study investigates the effects of the inhaled PGI2 analog epoprostenol on CIVD response thro

Case Report of a Traumatic Arthrotomy of the Knee Diagnosed by Ultrasound.

In this case report we describe evaluating a patient for a traumatic knee arthrotomy using ultrasound in a resource-limited medical clinic at the base of a ski area. A 23-y-old female presented with a laceration superior to the patella of the left leg. On examination, the wound tracked deep, and providers had concern for traumatic arthrotomy. Lacerations around the knee have the highest risk for traumatic arthrotomy of any joint. This risk is due to the joint capsule of the knee extending up to 12 cm proximally from the joint line of the knee, making suprapatellar lacerations a risk. Although

Extensor Mechanism Disruption with Multiligamentous Knee Injury from Bouldering.

Extensor mechanism injuries and multiligamentous knee injuries are rare and typically are caused by high-energy injury mechanisms. We present a case of an active-duty military service member who sustained a patellar tendon rupture with concomitant multiligamentous knee injury due to a fall while bouldering. This case report is the first to highlight this significant injury due to bouldering, in addition to the surgical steps taken to get the patient fit for full military duty. These injuries often go undiagnosed and are incredibly important to recognize given the importance of prompt evaluatio
