The value of lateral glenohumeral offset in predicting construct failure in proximal humerus fractures following internal fixation.

BACKGROUND: Proximal humerus fractures are the third most common osteoporosis defining injury in the United States, yet operative fixation of these injuries remains technically challenging. Although several modifiable and nonmodifiable risk factors are correlated with failure of proximal humerus fixation, no study has investigated whether failure to restore glenohumeral offset plays a part in fixation failure. The goals of this study are: (1) to determine if lateral glenohumeral offset (LGHO) and humeral head diameter (HHD) can be measured radiographically with accuracy between observers, (2)

Fractures of the medial column of the distal humerus in adults.

PURPOSE: Medial column fractures of the distal humerus are uncommon in adults. Among 26 patients identified in the English language literature, only 2 had articular fragmentation. We reviewed the experience of 2 trauma centers with isolated medial column fractures to evaluate our impression that these fractures often involve complex articular fragmentation. METHODS: There were 10 men and 4 women with an average age of 61 years (range, 44-86 years). The incidence was 3% of all distal humerus fractures at one institution with a prospective fracture database. The mechanism of injury was a fall f

[Ultrasound of extremity lesions caused by birth trauma].

With regard to injuries of the extremities caused by birth trauma, ultrasound allows simultaneous direct evaluation of the surface of osseous elements and of cartilaginous and soft-tissue structures as well. Thus, not only fractures, but also concomitant articular fluid collections, respectively haematomas and/or dislocation of an epiphysis are demonstrated in their initial extent and in their course of healing. Additionally, closed repositions, for instance of a displaced epiphysis, can be exactly monitored by ultrasound. A reduced number of roentgenograms as well as renunciation of other ima

The comparison of bedside point-of-care ultrasound and computed tomography in elbow injuries.

OBJECTIVE: The objective of the study was to compare the efficacy of point-of-care ultrasound (POCUS) and computed tomography (CT) in the diagnosis of the fracture of the bones that form the elbow joint and the determination of treatment method in elbow injuries. METHODS: Forty-nine patients aged 5 to 65 years, who were admitted with low-energy elbow injuries, had at least 1 fracture of the elbow joint bones, and underwent CT scanning, were included in this study. Before the initiation of the study, orthopedic surgeons and emergency physicians determined a common treatment based on the fractu

Closed intramedullary nailing with percutaneous cement augmentation for long bone metastases.

AIMS: The purpose of the study was to investigate whether closed intramedullary (IM) nailing with percutaneous cement augmentation is better than conventional closed nailing at relieving pain and suppressing tumours in patients with metastases of the femur and humerus. PATIENTS AND METHODS: A total of 43 patients (27 men, 16 women, mean age 63.7 years, standard deviation (sd) 12.2; 21 to 84) underwent closed IM nailing with cement augmentation for long bone metastases. A further 27 patients, who underwent conventional closed IM nailing, served as controls. Pain was assessed using a visual ana

Floating elbow in a polytrauma patient: timing and surgical strategy.

A 22-year-old man was admitted to our hospital because of polytrauma following a car accident. The instrumental investigations detected parenchymatous and bone lesions, including a floating elbow (articular distal humerus fracture associated with ipsilateral forearm fracture). After initial temporary stabilization with external fixator and restoration of hemodynamics, we proceeded to the fixation of the upper limb. To reduce local and general risks of complications, a two-stage reconstruction technique has been applied.

Operative management of closed intra-articular fractures of distal end of humerus in adults.

This is a study of 60 patients with mean age of 30 years having intra-articular fracture of distal humerus of type C (AO classification). We treated these fractures by open reduction and internal fixation through transolecranon approach. Mean duration of operative time with that of injury was 10 days. In all the cases 1st the intracondylar fracture was fixed followed by reconstruction and stabilisation of medial and lateral pillar by 1/3 tubular plate and 3.5 mm dynamic compressent plate (DCP) or recon plate respectively. Minimum follow-up was of 1 year duration with average follow-up of 3 yea

Avulsion fractures of the lesser tuberosity of the humerus in adolescents: review of the literature and case report.

Isolated fracture of the lesser tuberosity is an unusual phenomenon in children and adolescents. These injuries are difficult to diagnose acutely and often present as chronic shoulder pain. In this study, we report on 1 case of a displaced lesser tuberosity apophysis avulsion fracture in an adolescent treated with open reduction and internal fixation, as well as a review of the literature. A 14-year-old adolescent male presented to the senior surgeon complaining of left shoulder pain and weakness 10 days after a wrestling injury. He was diagnosed with a displaced, isolated fracture of the less

Total elbow arthroplasty following complex fractures of the distal humerus: results in patients over 65 years of age.

BACKGROUND: The treatment of complex distal humerus fractures is challenging and is often associated with unsatisfactory results. Anatomic reduction and stable internal fixation is difficult to obtain, especially in elderly osteoporotic patients. For these reasons, total elbow arthroplasty (TEA) has recently evolved as a valid procedure in selected cases following these injuries. The aim of this study was to analyze outcomes of TEA for the treatment of complex distal humerus fractures in 20 low-demanding patients over 65 years of age. MATERIALS AND METHODS: All patients, at a mean follow-up o

Childhood obesity as a risk factor for lateral condyle fractures over supracondylar humerus fractures.

BACKGROUND: Obese children reportedly have an increased risk of sustaining musculoskeletal injuries compared with their normal-weight peers. Obese children are at greater risk for sustaining fractures of the forearm, particularly from low-energy mechanisms. Furthermore, obesity is a risk factor for sustaining an extremity fracture requiring surgery. However, it is unclear what role obesity plays in fractures about the distal humerus. QUESTIONS/PURPOSES: We therefore asked whether (1) children who sustain lateral condyle (LC) fractures have a higher body mass index (BMI) as compared with those

The value of ultrasonic diagnosis in the management of vascular complications of supracondylar fractures of the humerus in children.

Of 48 consecutive children with Gartland III supracondylar fractures, 11 (23%) had evidence of vascular injury, with an absent radial pulse. The hand was pink and warm in eight and white and cold in the other three patients. They underwent colour-coded duplex scanning (CCDS) and ultrasound velocimetry (UV) to investigate the patency of the brachial artery and arterial blood flow. In seven patients with a pink pulseless hand, CCDS showed a displaced, kinked and spastic brachial artery and a thrombosis was present in the other. In all cases UV showed reduced blood flow in the hand. In three pati

[Two different methods of supracondylar lateral wedge osteotomy in cubitus varus deformity in adults].

OBJECTIVES: We reviewed the results of a transverse osteotomy for cubitus varus deformity in adults stabilized by two different methods of fixation with either Kirschner wire (K-wire) or Y plate. PATIENTS AND METHODS: Sixteen soldiers were treated by a lateral closing wedge osteotomy of the distal humerus for cubitus varus deformity in the military infirmary. Two different methods of fixation were used for the osteotomy. The osteotomies were stabilized with K-wires in the initial five cases. Because of loosening of the pins and loss of correction in these five cases, Y plates were introduced

Apparent capitellar fractures.

Isolated capitellar fractures are rare but are identified as such, even when they are more complex, because the displaced capitellar fracture is usually the most obvious and identifiable radiographic finding and because teaching has traditionally underemphasized the involvement of the trochlea in such fractures. The author prefers the term 'apparent capitellar fractures' and draws on his experience to explain why he favors three-dimensional CT for depicting fracture detail. This article discusses treatment options, emphasizing open reduction and internal fixation to restore the native elbow. O

Treatment of pediatric supracondylar humerus fractures in the community hospital.

BACKGROUND: Supracondylar fractures of the humerus are among the most common elbow injuries in the pediatric population. Because of the significant morbidity associated with treating displaced pediatric supracondylar humerus fractures, most community-based orthopedic surgeons prefer to transfer these injuries to specialty children's hospitals. Our intention in writing this article was to document and evaluate the results we obtained using the lateral diverging pin technique to treat patients at our community hospital. In doing so, we set out to determine if our results were comparable to those

Upper extremity radionuclide bone imaging: shoulder, arm, elbow, and forearm.

Along with resurgence of physical activity during these fitness conscious times, there is increasing participation by people of all ages in sporting activities. Enrollment in organized and recreational sports among young adults and children has increased with as many as 30% of adolescents participating in competitive high school athletics. Despite improvements in equipment, the number of sports-related injuries presenting for medical attention has increased. Injuries to the upper extremities account for more than 25% of all sports-related injuries, but receive disproportionately less attention

Hill-Sachs lesion in recurrent shoulder dislocation: sonographic detection.

In a prospective study 61 patients with recurrent anterior shoulder dislocation were evaluated by sonography, radiography, and surgery to determine the value of sonography in the detection of a HillSachs lesion. The group consisted of 57 male and four female patients with an average age of 27 years. Hill-Sachs lesion was found in 54 (88%) shoulders of the 61 surgically treated patients. Using surgical findings as the gold standard, we found sonography to be 96% (52 of 54 cases) sensitive, 100% specific (seven of seven cases), and 97% (59 of 61 cases) accurate in the diagnosis of HillSachs lesi
