Bronchopulmonary foregut malformation in 3 infants--with special references to cases in childhood.

We encountered three infant patients with bronchopulmonary foregut malformation. Two of them were associated with oesophageal atresia, and the remaining one with jejunal atresia. Two of the patients showed total lung sequestration, and the rest showed intralobar sequestration. 37 paediatric cases of this type were reviewed from the literature. Total lung sequestration was observed in 19 of the 37 cases, showing an increase in the incidence in comparison with that reported previously. There were also 7 cases associated with oesophageal atresia.

Fistulisation of an iliac pseudoaneurysm into the appendix. Presentation of a case and a review of the literature.

A case of ilioappendiceal fistula is presented. The patient had previously been operated on for a ruptured aneurysm of the common iliac artery. 21 years later he developed occult gastrointestinal bleeding without signs of infection. Colonoscopy revealed bloody faeces and an isotope scan haemorrhage in the ascending colon. Laparotomy and right hemicolectomy was performed without identifying the fistula. The head of the appendix was left attached to the scarred peritoneal wall. As the bleeding continued, a second laparotomy was performed revealing an iliac pseudoaneurysm with fistulisation into

Prevention of the development of a vesicovaginal fistula.

The cause of vesicovaginal fistulas after hysterectomy is not clearly understood. In an attempt to determine its cause, the records of 12 patients who had vesicovaginal fistula develop (after total abdominal hysterectomy) were compared with 12 consecutive patients who underwent total abdominal hysterectomy without fistula formation. Most of the patients who had vesicovaginal fistulas develop had excessive postoperative abdominal pain, distension or paralytic ileus, or both. Hematuria and symptoms of irritability of the bladder were also noted in the fistula group and prolonged postoperative fe

Multiple choledochoduodenal fistulas in the periampullary region.

Five patients with two periampullary choledochoduodenal fistulas diagnosed by endoscopic retrograde cholangiopancreatography (ERCP) are described. In two patients both fistulas were on the oral prominence of the papilla, whereas in the remaining three, one of the two fistulas was more proximal. All five patients presented with right upper abdominal pain, which was associated with chills, fever and jaundice in two. ERCP revealed choledocholithiasis in two patients, cholecystolithiasis in one, hepatocholedocholithiasis in one, and bile duct dilatation without stone in one. A previous history of

Urogenital sinus abnormalities in the female patient.

Three patients with a urogenital sinus and 1 with a cloaca presented with complex diagnostic and therapeutic anomalies. A classification of these and other vaginal anomalies is based on the theory that they all represent an arrest in normal embryological development. Six categories are described, including vaginal agenesis, urogenital sinus, vaginal atresia, vaginal septa, vaginal stenosis and cloaca.

Posterior rectotomy for villous tumours of the rectum.

Posterior extrasphincteric rectotomy gives good access to the mid-rectum 8-15 cm from the anus. The method is suitable for the excision of villous and adenomatous tumours in this region as it permits radical extirpation and consequently little risk of recurrence. Its chief disadvantage is the risk of a fecal fistula. Among 10 operated patients a fistula was seen in 2; it healed spontaneously in the course of 4 weeks in one but did not heal in the other even after a sigmoidostomy. The latter patient has recurrence of the tumour and an insufficiently radical operation is presumably the reason wh

Incidence of stricturing and penetrating complications of Crohn's disease diagnosed in pediatric patients.

BACKGROUND: The development of disease complications is poorly characterized in pediatric patients with Crohn's disease (CD). METHODS: We retrospectively determined the cumulative incidence of stricturing and penetrating complications of CD prior to first surgery utilizing data from 989 consecutively enrolled CD patients (age 0-17 years at diagnosis) collected between January 2000 and November 2003 and stored in the Pediatric IBD Consortium Registry. RESULTS: Mean age at diagnosis was 11.5 +/- 3.8 (standard deviation) years. Median follow-up time was 2.8 years. Prior to first surgery, the cu
