Details of 300 patients with complicated diverticular disease from 30 hospitals between 1985 and 1988 were entered into a national audit organized by the Surgical Research Society. Complications present on admission included acute phlegmon (n = 104), pericolic abscess (n = 34), purulent peritonitis (n = 40), large bowel obstruction (n = 31), faecal peritonitis (n = 23), pericolic abscess complicated by fistula (n = 28) and lower gastrointestinal bleeding (n = 40). BACKGROUND: The aim of the present study was to review the prevalence and surgical management of patients with Crohn's disease (CD) complicated by ileocolic-duodenal fistulas (ICDF).
METHODS: We performed a retrospective chart review of CD patients who underwent surgical takedown and repair of ICDF during January 2011-December 2021 at two inflammatory bowel disease referral centers.

CASE: A 50-year-old man with a history of chronic refractory periprosthetic joint infection following total hip arthroplasty, which had been treated with multiple antibiotic spacers, developed an enteroarticular fistula. He was able to avoid hip disarticulation after undergoing a laparoscopic colectomy with a diverting colostomy and a functional hip resection arthroplasty. At the 1-year follow-up, he was walking independently with crutches and had minimal pain.

This article reports a case of colovesical fistula presenting with epididymitis. A 75-year-old man with a recent conservatively managed localised diverticular perforation presented to hospital with acute pain and swelling of his left testicle and epididymis. On further questioning, the patient reported passing air in his urine. Urine cultures grew Enterococcus faecalis. Attempts to treat intractable enterocutaneous fistulae secondary to Crohn's disease are challenging and have been associated with long delays. An island pedicled anterolateral thigh (ALT) flap has been shown to achieve adequate coverage of abdominal wall reconstruction. In this case, with the assistance of a well-vascularised flap and adequate medical supportive managements, the intractable enterocutaneous fistula was closed; it then healed progressively.
