Tear of the quadriceps tendon after revision or primary total knee arthroplasty is a rare complication, but when it occurs, this injury has serious functional consequences. In complete tears, the outcome of direct repair is unpredictable, and several authors recommend that the suture should be reinforced. Several techniques have been described, including the use of autografts, allografts, and synthetic mesh. The goal of this study was to assess the outcomes of a reconstruction technique augmented with synthetic mesh. A retrospective study was performed involving 3 patients who had chronic part

Previous case reports have documented the successful use of allograft for extensor mechanism reconstruction. We hypothesized that extensor mechanism reconstruction with allograft would restore extensor power and allow patients to return to a relatively high activity level. Between 2000 and 2007, 17 patients (18 knees) underwent extensor mechanism reconstruction with either nonirradiated Achilles or whole bone-patellar tendon-bone allograft at our institution. Two patients were lost to follow-up and one underwent a total knee arthroplasty and was considered a failure. The remaining 14 patients

PURPOSE: Rectovaginal fistulas constitute a serious burden for the affected patient and a major challenge for the attending surgeon. Definitive surgical treatment of the fistula depends on the size and location of the fistula, the underlying disease, and any previous therapies. In regards to complicated recurrent rectovaginal fistulas, transposition of the gracilis muscle is one of the well-established therapeutic options with a success rate of up to 70%.
MATERIAL AND METHODS: Between 01/2004 and 06/2010, ten patients diagnosed with a recurrent rectovaginal fistula were treated in the surgica

UNLABELLED: Ice hockey is a high contact sport where players are inherently at an increased risk for traumatic and time-loss injury. With its increasing popularity and high incidence of injury, further research is necessary to understand the risks and injuries associated with the sport and to develop performance-based outcome measures to guide return to play. This review, tailored to the practicing sports medicine team physician, focuses on the stepwise identification, treatment, time loss, return to play and subsequent risk of injury for the most common areas of injury: the head, shoulder, hi

The purpose of this retrospective study was to compare clinical parameters in preoperative, perioperative, and postoperative categories between valgus and varus deformities in primary total knee arthroplasty (TKA) following midvastus approach. We compared 83 patients (83 TKAs in valgus) with 949 patients (1084 TKAs in varus), with a mean follow-up of 72 months. In valgus deformity, mean age and body mass index at TKA were younger and smaller with more percentage of patellar subluxation. Cutting thickness from distal femur, tibial plateau, and lateral aspect of posterior femur were significantl

PURPOSE: The influence of anesthetic interactions on motor-evoked potentials in infants has rarely been reported. In infants, adding a small dose of sevoflurane to propofol-based total intravenous anesthesia is reasonable for reducing propofol administration. We collected preliminary data regarding the effect of low-dose sevoflurane in propofol-based total intravenous anesthesia on motor-evoked potentials in infants.
METHODS: This pilot interventional study included 10 consecutive infants requiring motor-evoked potentials between January 2023 and March 2024. The motor-evoked potential amplitu

BACKGROUND: Abnormal patellar height (patella alta) has been reported to be one of the main predisposing factors for recurrent patellar dislocation, and it can be surgically corrected by distalizing tibial tubercle osteotomy (DTTO). Rehabilitation after DTTO often includes limitations on weightbearing and restrictions on knee range of motion by means of bracing, increasing the risk of slow progression of the rehabilitation.
HYPOTHESIS: An active rehabilitation program with no restrictions on weightbearing and range of movement would yield a low risk of postoperative complications and a fast r

INTRODUCTION: Freeman-Sheldon syndrome (FSS), also known as the distal arthrogryposis (DA) type 2A, is a rare congenital anomaly. We report a unique case of the DA type 2A with mixed clinical features and the unusual presentation of bilateral congenital dislocation of the knee but had unassisted stiff knee gait.
CASE REPORT: A 5-year-old female child presented to the clinic with the complaint of inability to bend both knees since birth. She had an unassisted bipedal gait, but could not squat, cross-leg sit, run, and climb stairs without assistance. Her youngest brother had a similar presentat

BACKGROUND: Regional analgesia is worth performing in the multimodal postoperative management of hip fracture (HF) because it reduces hospital morbidity and mortality. The aim of this study is to compare the efficacy and side effects of the recently described "Pericapsular Nerve Group (PENG) Block" with those of the femoral block, which is considered the standard of care for postoperative pain control after femoral neck fracture.
MATERIALS AND METHODS: We conducted a comparative observational study at a university hospital (Saint Antoine Hospital, Sorbonne University, Paris, France), where th

BACKGROUND: Rectus femoris injuries are common among athletes, especially in kicking sports such as soccer; however, proximal rectus femoris avulsions in athletes are a relatively rare entity.
PURPOSE/HYPOTHESIS: The purpose of this study was to describe and report the results of an original technique of surgical excision of the proximal tendon remnant followed by a muscular suture repair. Our hypothesis was that this technique limits the risk of recurrence in high-level athletes and allows for rapid recovery without loss of quadriceps strength.
STUDY DESIGN: Case series; Level of evidence,

The aim of this study was to determine the effects of a newly developed reciprocal gait orthosis (RGO) with a variable constraint hip mechanism (VCHM) on the kinematics and kinetics of normal gait. The VCHM was compared with the isocentric reciprocating gait orthosis (IRGO) for walking after paraplegia. Both the VCHM and the IRGO were evaluated with able-bodied volunteers with the hip reciprocating mechanisms coupled and uncoupled. The VCHM was further evaluated with context-dependent coupling based on a finite-state control algorithm utilizing information from brace-mounted sensors. Walking p

Arthrosis following rupture of the anterior cruciate ligament has been analysed in two series. The first series was derived from a review of 150 cases of reconstruction of the anterior cruciate ligament with a follow-up of 3 years or more. Arthrosis was seen to have developed in 13.3%. The second series was concerned with 64 cases of unilateral arthrosis treated by upper tibial valgus osteotomy in whom there had been a previous rupture of the anterior cruciate ligament. The "tolerance time"--that is the time between the original ligamentous injury and the time of osteotomy--for the development
