Exploratory Assessment of the Relationship Between Hemoglobin Volume Phase Index, Magnetic Resonance Imaging, and Functional Outcome in Neonates with Hypoxic-Ischemic Encephalopathy.

BACKGROUND/OBJECTIVE: Near-infrared spectroscopy (NIRS)-based measures of cerebral autoregulation (CAR) can potentially identify neonates with hypoxic-ischemic encephalopathy (HIE) who are at greatest risk of irreversible brain injury. However, modest predictive abilities have precluded previously described metrics from entering clinical care. We previously validated a novel autoregulation metric in a piglet model of induced hypotension called the hemoglobin volume phase index (HVP). The objective of this study was to evaluate the clinical ability of the HVP to predict adverse outcomes neonate

More Impaired Dynamic Ventilatory Muscle Oxygenation in Congestive Heart Failure than in Chronic Obstructive Pulmonary Disease.

Patients with chronic obstructive pulmonary disease (COPD) and congestive heart failure (CHF) often have dyspnea. Despite differences in primary organ derangement and similarities in secondary skeletal muscle changes, both patient groups have prominent functional impairment. With similar daily exercise performance in patients with CHF and COPD, we hypothesized that patients with CHF would have worse ventilatory muscle oxygenation than patients with COPD. This study aimed to compare differences in tissue oxygenation and blood capacity between ventilatory muscles and leg muscles and between the

Regional Cerebral Oximetry as an Indicator of Acute Brain Injury in Adults Undergoing Veno-Arterial Extracorporeal Membrane Oxygenation-A Prospective Pilot Study.

Background: Regional cerebral oxygen saturation (rScO2) measured by near-infrared spectroscopy (NIRS) can be used to monitor brain oxygenation in extracorporeal membrane oxygenation (ECMO). ECMO patients that develop acute brain injuries (ABIs) are observed to have worse outcomes. We evaluated the association between rScO2 and ABI in venoarterial (VA) ECMO patients. Methods: We retrospectively reviewed prospectively-collected NIRS data from patients undergoing VA ECMO from April 2016 to October 2016. Baseline demographics, ECMO and clinical characteristics, cerebral oximetry data, neuroradiogr

Brain preservation with selective cerebral perfusion for operations requiring circulatory arrest: protection at 25 degrees C is similar to 18 degrees C with shorter operating times.

BACKGROUND: Hypothermic circulatory arrest (HCA) is employed for aortic arch and other complex operations, often with selective cerebral perfusion (SCP). Our previous work has demonstrated real-time evidence of improved brain protection using SCP at 18 degrees C. The purpose of this study was to evaluate the utility of SCP at warmer temperatures (25 degrees C) and its impact on operating times. METHODS: Piglets undergoing cardiopulmonary bypass (CPB) and 60 min of HCA were assigned to three groups: 18 degrees C without SCP, 18 degrees C with SCP and 25 degrees C with SCP (n=8 animals per grou

Effects of electroacupuncture synchronised with motor imagery training on upper limb motor recovery and neural mechanisms in ischaemic stroke: protocol for a single-centre, randomised controlled trial.

INTRODUCTION: Approximately half of stroke survivors experience persistent upper limb dysfunction, which impairs self-care, reduces independence and lowers quality of life. Electroacupuncture is an established intervention with evidence supporting its role in improving upper limb motor function following ischaemic stroke. Motor imagery training (MIT), which activates the sensorimotor cortex through the mental rehearsal of movement, has shown promise as an adjunctive therapy in stroke rehabilitation. The concurrent application of electroacupuncture and MIT may enhance sensorimotor recovery by p

Effectiveness and mechanism of moxibustion in treating chronic non-specific low back pain: study protocol for a multicenter randomized controlled trial.

INTRODUCTION: Chronic non-specific low back pain (CNLBP) represents the most commonly encountered subtype of low back pain (LBP) in clinical practice. It has no clearly identified etiological factors and is prone to recurrence, which severely compromises patients' quality of life. Moxibustion therapy is commonly utilized in China for managing chronic pain conditions and has demonstrated favorable clinical outcomes. However, high-quality randomized controlled trials remain scarce, and the mechanism of action of moxibustion remains unclear. This severely restricts the credibility of moxibustion

The phase coherence of the neurovascular unit is reduced in Huntington's disease.

Huntington's disease is a neurodegenerative disorder in which neuronal death leads to chorea and cognitive decline. Individuals with ≥40 cytosine-adenine-guanine repeats on the interesting transcript 15 gene develop Huntington's disease due to a mutated huntingtin protein. While the associated structural and molecular changes are well characterized, the alterations in neurovascular function that lead to the symptoms are not yet fully understood. Recently, the neurovascular unit has gained attention as a key player in neurodegenerative diseases. The mutant huntingtin protein is known to be pres

A new material built with alternating Cu sulfide and (Al,Mg) hydroxide molecular sheets: hydrothermal synthesis and selected characteristics.

Two-dimensional materials with new physical phenomena are gaining popularity due to their unique properties. In recent years, a new family of layered compounds inspired by the minerals valleriite and tochilinite which are composed of alternating quasi-atomic sheets of transition metal chalcogenides (sulfides and selenides of Fe, Fe-Cu and other metals) and hydroxides of Mg, Al, Fe, Li, etc., assembled via electrostatic interaction, has arisen as a new synthetic platform for 2D materials. In this work, we synthesized a new promising material composed of alternating quasi two-dimensional sulfide

fNIRS Evaluation of Frontal and Temporal Cortex Activation by Verbal Fluency Task and High-Level Cognition Task for Detecting Anxiety and Depression.

Anxiety and depression are widespread psychosis which are believed to affect cerebral metabolism, especially in frontal and temporal cortex. The comorbidity patients of anxiety and depression (A&D) have more serious clinical symptoms. Functional near-infrared spectroscopy (fNIRS) is a noninvasive modality used to monitor human brain oxygenation, and it could be considered as a potential tool to detect psychosis which may lead to abnormal cerebral oxygen status when the brain is activated. However, how sensitive the cerebral oxygenation response to the cortex activation and whether these respon

Intravitreal controlled release of dexamethasone from engineered microparticles of porous silicon dioxide.

Dexamethasone is a glucocorticoid that is widely used in the ophthalmic arena. The recent FDA approved dexamethasone implant can provide a three month efficacy but with high rate of drug related cataract and high intraocular pressure (IOP). It seems that higher steroid in aqueous humor and around lens may be associated with these complications based on clinical fact that higher IOP was observed with intravitreal triamcinolone acetonide (TA) than with subtenon TA. We hypothesize that placing a sustained dexamethasone release system near back of the eye through a fine needle can maximize efficac

A pilot randomised controlled trial of peripheral fractional oxygen extraction to guide blood transfusions in preterm infants.

BACKGROUND: Peripheral fractional oxygen extraction (FOE) may be a better indicator of the need for transfusion than the haemoglobin concentration (Hb) because it is a measure of the adequacy of oxygen delivery to meet demand. A randomised controlled trial of the use of peripheral FOE to guide the need for blood transfusions in preterm infants was carried out to test this hypothesis. METHOD: Infants less than 1500 g birth weight who were stable and less than 2 weeks old were randomised to receive transfusions guided by either a conventional protocol based on Hb (conventional group) or a proto

Combining transcranial direct current stimulation with music therapy improves cognitive function in schizophrenia: study protocol for a randomized, double-blind, sham-controlled clinical trial.

BACKGROUND: Despite numerous pharmacological treatments, individuals with schizophrenia continue to exhibit significant residual cognitive impairments, adversely affecting the progression of the illness and their overall quality of life. Preliminary evidence indicates that transcranial direct current stimulation (tDCS) and music therapy (MT) may offer potential benefits for enhancing cognitive function in schizophrenia. This study aims to examine the synergistic efficacy of tDCS and MT on cognitive impairments in individuals with schizophrenia and to elucidate the potential mechanisms involved

Rheology of Aqueous Solutions in the Presence of Proton Exchange Membrane: Surface Tension.

Controlling the rheological properties of liquids allows for the regulation of effective movement, transport of substances, and processes in biological systems. This work presents an experimental investigation into the influence of the proton-exchange polymer membrane Nafion on the surface tension coefficient (STC) of distilled water, aqueous solutions of two methylene blue (MB) forms, and ascorbic acid (AA). Immediately upon membrane immersion in the solutions, a sharp decrease in the surface tension of distilled water, as well as of the oxidized and reduced forms of MB, occurs. The observed

Cortical activation of neuromuscular electrical stimulation synchronized mirror neuron rehabilitation strategies: an fNIRS study.

BACKGROUND: The mirror neuron system (MNS) plays a key role in the neural mechanism underlying motor learning and neural plasticity. Action observation (AO), action execution (AE), and a combination of both, known as action imitation (AI), are the most commonly used rehabilitation strategies based on MNS. It is possible to enhance the cortical activation area and amplitude by combining traditional neuromuscular electrical stimulation (NMES) with other top-down and active rehabilitation strategies based on the MNS theory. OBJECTIVE: This study aimed to explore the cortical activation patterns

Percutaneous Common Carotid Artery Access for Cardiac Interventions in Infants Does Not Acutely Change Cerebral Perfusion.

Pediatric cardiac interventions via percutaneous common carotid artery (CCA) access have been shown to be safe and effective. However, the impact of placement of a sheath in the carotid artery for interventions on cerebral perfusion is unknown. In this study we used cerebral near-infrared spectroscopy (NIRS) to analyze the effects of percutaneous CCA access for cardiac interventions on cerebral perfusion. This study is a retrospective chart review carried out at a tertiary care center on all pediatric patients who underwent percutaneous cardiac catheterization via carotid artery access from Ja
