USH2A-retinopathy: From genetics to therapeutics.

Bilallelic variants in the USH2A gene can cause Usher syndrome type 2 and non-syndromic retinitis pigmentosa. In both disorders, the retinal phenotype involves progressive rod photoreceptor loss resulting in nyctalopia and a constricted visual field, followed by subsequent cone degeneration, leading to the loss of central vision and severe visual impairment. The USH2A gene raises many challenges for researchers and clinicians due to a broad spectrum of mutations, a large gene size hampering gene therapy development and limited knowledge on its pathogenicity. Patients with Usher type 2 may bene

Ear Involvement in Inflammatory Bowel Disease: A Review of the Literature.

Inflammatory bowel disease (IBD) is a multisystemic disease. The ear is a rare but recognized site of extraintestinal manifestations of IBD. … -inner ear antigens). Initial therapy is usually steroids, with a step up to anti-TNF-a therapy and cochlear implantations with failure of treatment. Furthermore, Cogan's syndrome, a chronic disease characterized by deafness, vertigo keratitis and aortitis, has been associated with IBD and mainly with Crohn's disease.

Evaluation of labyrinthine fistula by MR cisternography.

Sensorineural hearing loss (SNHL) can develop after trauma and determination of its etiology is crucial for treatment planning. Computed tomography (CT) is the first-line imaging method in evaluation of trauma victims; however, its value is limited in visualization of perilymphatic fistula or in assessment of cochlear implant candidates. Herein, we present imaging findings of a patient with traumatic bilateral transverse fractures of temporal bones and resultant SNHL. This patient had a fistula between inner ear and subarachnoid space which was detected at non-contrast-enhanced MR cisternograp

Simultaneous intracochlear stimulation based on channel interaction compensation: analysis and first results.

A simultaneous paradigm for electric stimulation of the acoustic nerve based on a monopolar electrode configuration and sign-correlated pulses is presented. Simultaneous pulse amplitudes are determined by taking into account parameters of spatial channel interaction. The computation of simultaneous amplitudes requires the solution of linear systems of equations in an iterative procedure. The computation amount can be reduced significantly, if the spatial impulse responses in individual electrodes can be approximated by two exponentially decaying branches with decay constants alpha toward apex

Perception of across-frequency interaural level differences.

The interaural level difference (ILD) is an important cue for the localization of sound sources. Just noticeable differences (JND) in ILD were measured in 12 normal hearing subjects for uncorrelated noise bands with a bandwidth of 13 octave and a different center frequency in both ears. In one ear the center frequency was either 250, 500, 1000, or 4000 Hz. In the other ear, a frequency shift of 0, 16, 13, or 1 octave was introduced. JNDs in ILD for unshifted, uncorrelated noise bands of 13 octave width were 2.6, 2.6, 2.5, and 1.4 dB for 250, 500, 1000, and 4000 Hz, respectively. Averaged over

[Far-advanced otosclerosis].

OBJECTIVES: The aim of this study was to report a series of patients with far-advanced otosclerosis who were unable to benefit from hearing aids. Among patients with profound hearing loss, it is particularly useful to diagnose far-advanced otosclerosis even if relatively rare, because stapes surgery can improve hearing to a level allowing conventional hearing aid use (sometime the only choice before cochlear implant). MATERIAL AND METHODS: We retrospectively reviewed the charts of 7 patients (9 operated ears) in order to highlight diagnostic criteria, surgical indications and results of stape

[Extreme stapedectomy].

Extreme stapedectomy is an operation aimed at restoring a serviceable hearing in patients affected by far-advanced otosclerosis with profound hearing loss. Surgery is performed on the understanding that the hearing aid will still be needed after the operation. Therefore, patient's expectations should be confined to possibly improved performance with a hearing aid as a consequence of the operation. Clinical as well as audiological preoperative assessment must be very rigorous. Counselling is mandatory and a trial period with a hearing aid is strongly advisable. Our experience is based on 15 cas

Pleiotrophin increases neurite length and number of spiral ganglion neurons in vitro.

Acoustic trauma, aging, genetic defects or ototoxic drugs are causes for sensorineural hearing loss involving sensory hair cell death and secondary degeneration of spiral ganglion neurons. Auditory implants are the only available therapy for severe to profound sensorineural hearing loss when hearing aids do not provide a sufficient speech discrimination anymore. Neurotrophic factors represent potential therapeutic candidates to improve the performance of cochlear implants (CIs) by the support of spiral ganglion neurons (SGNs). Here, we investigated the effect of pleiotrophin (PTN), a well-desc

Olfactory implant: Demand for a future treatment option in patients with olfactory dysfunction.

OBJECTIVES: Therapeutic options in olfactory dysfunction (OD) are limited. Numerous studies have shown impact of OD on quality of life. Lately, various studies support benefits of olfactory training, but therapy-refractory cases leave doctors and patients locked in a stalemate. An olfactory implant (OI), in analogy to the widely successful cochlear implant, still seems far away from realization. The present study sought to evaluate the demand of OI in patients with OD. METHODS: Sixty-one patients (28 females and 33 males, mean age/standard deviation 54.9/17.6 years) with OD were recruited. We

Tinnitus Suppression by Intracochlear Electrical Stimulation in Single Sided Deafness - A Prospective Clinical Trial: Follow-Up.

INTRODUCTION: Earlier studies show that a Cochlear Implant (CI), capable of providing intracochlear electrical stimulation independent of environmental sounds, appears to suppress tinnitus at least for minutes. The current main objective is to compare the long-term suppressive effects of looped (i.e. repeated) electrical stimulation (without environmental sound perception) with the standard stimulation pattern of a CI (with environmental sound perception). This could open new possibilities for the development of a "Tinnitus Implant" (TI), an intracochlear pulse generator for the suppression of

Inner ear and maternal reproductive defects in mice lacking the Hmx3 homeobox gene.

The Hmx homeobox gene family is of ancient origin, being present in species as diverse as Drosophila, sea urchin and mammals. The three members of the murine Hmx family, designated Hmx1, Hmx2 and Hmx3, are expressed in tissues that suggest a common functional role in sensory organ development and pregnancy. Hmx3 is one of the earliest markers for vestibular inner ear development during embryogenesis, and is also upregulated in the myometrium of the uterus during pregnancy. Targeted disruption of the Hmx3 gene results in mice with abnormal circling behavior and severe vestibular defects owing t

Fast spin echo MRI in otology.

Magnetic resonance imaging (MRI) using a fast spin echo protocol (FSE) gives greatly improved spatial resolution for the inner ear and its central connections. Excellent contrast resolution between bone, neural tissue and intracranial fluids is obtained on the heavily T2 weighted images. The contents of the internal auditory meatus (IAM) are shown more clearly than on T1 weighted images, even with gadolinium enhancement. Use of the technique for congenital, vascular, granulomatous and obliterative lesions of the inner ear is described. The cochlear and vestibular nerves can be seen side by sid
