The prevalence of complementary and alternative medicine (CAM) use in US children with eczema is unknown. Furthermore, it is unknown whether CAM use in the United States is associated with higher eczema prevalence. We sought to determine the eczema prevalence in association with CAM usage. We analyzed data from the 2007 National Health Interview Survey that included a nationally representative sample of 9417 children ages 0 to 17 years. Overall, 46.9% (95% confidence interval, 45.6%-48.2%) of children in the United States used 1 or more CAM, of which 0.99% (0.28%-1.71%) used CAM specifically t

Fragrances are a common cause of allergic contact dermatitis in Europe and in North America. They can affect individuals at any age and elicit a spectrum of reactions from contact urticaria to systemic contact dermatitis. Growing recognition of the widespread use of fragrances in modern society has fueled attempts to prevent sensitization through improved allergen identification, labeling, and consumer education. This review provides an overview and update on fragrance allergy. Part 1 discusses the epidemiology and evaluation of suspected fragrance allergy. Part 2 reviews screening methods, em

BACKGROUND: Contact urticaria (CU) is the development of a wheal and flare on the skin after topical exposure to a particular chemical or compound. It can be diagnosed through a variety of techniques. Many chemicals that cause a type IV allergy can also cause CU. The incidence of CU to these chemicals is unknown.
OBJECTIVE: The aim of this study was to evaluate the opinions of the American Contact Dermatitis Society members regarding CU and scratch testing.
METHODS: We distributed an electronic survey to the American Contact Dermatitis Society members regarding observed prevalence of CU, fre

BACKGROUND: Mercaptobenzothiazole (MBT) and mercapto compounds are primarily used in rubber products.
OBJECTIVE: This study aimed to examine concomitant-positive rates of MBT (1% pet) and the 4-part mercapto mix (MM) (1% pet).
DESIGN: This is a retrospective cross-sectional data from the North American Contact Dermatitis Group.
RESULTS: A total of 30,880 patients were patch tested to MM and MBT. There were 333 positive reactions to MM and 427 positive reactions to MBT. Ninety-eight patients were positive to MM alone, 192 to MBT alone, and 235 reacted to both. Forty-five percent (192/427) of

BACKGROUND: Dermatitis palmaris sicca (DPS) is a common dry-fissured palmar dermatitis in Asian women. It may be an irritant contact dermatitis, but the immunophenotype of the cells in its infiltrate is unknown.
OBJECTIVE: The aim of this study was to evaluate the role of inflammatory cells in the pathogenesis of DPS.
METHODS: Patch testing was done in 68 patients with DPS, 87 subjects with hand eczema, and 31 healthy subjects. Immunophenotyping of cutaneous inflammatory cells was performed in 8 patients with DPS, 10 subjects with hand eczema, and 8 healthy individuals.
RESULTS: Positive pa

The preservative methylisothiazolinone (MI) is the American Contact Dermatitis Society Contact Allergen of the Year for 2013. Because the use of MI in cosmetics and toiletries in the United States rises, MI exposure also rises. Although it might seem likely that testing with methylchloroisothiazolinone (MCI)/MI would be adequate to pick up contact allergy to MI alone, the mix misses approximately 40% of allergy to MI, likely because of the low concentration of MI in the MCI/MI combination patch test. In Europe, several groups have documented frequency of allergy to this preservative of approxi

Patch testing is widely used in evaluating suspected contact dermatitis. One major component of a quality patch test result is a dependable, predictable allergen supply. The allergen needs to be present at a sufficient concentration to elicit a reaction in an allergic patient. To better understand the stability of patch-test allergens, we completed a systematic review of the literature. We found that there is variability in stability among patch-test allergens and that although a few have been shown to be stable, many degrade when in storage. In most cases, expiration dates should be honored.

Oat and wheat are used as ingredients in various cosmetics and personal care products because of their moisturizing properties. Impaired barrier functions in atopic dermatitis (AD) may increase the risks of sensitization to oat and wheat proteins via skin. Immediate- and delayed-type hypersensitivity reactions to oat and wheat in personal care products have been reported in previous studies, and most of those cases were patients with AD. Patch testing with oat and wheat proteins should be performed more frequently, especially in atopic children. It may help identify contact dermatitis, which m

Evidence for the effectiveness of patch testing and the need for an expanded series that provides experience and evidence-based suggestions for an extended patch testing series are examined in this review. Many of those testing with shorter allergen series are interested in expanding the spectrum of patch testing. The American Contact Dermatitis Society (ACDS) Core Allergen Series Group has arranged a group of suggested allergen groups that can be logically scaled up or down depending on the needs of the patch tester and the community being tested. This is not an "ACDS 80 Standard." We suggest

BACKGROUND: Keys are an important exposure source of metal allergens to consumers and confer a significant problem for nickel-allergic individuals because of repeated daily use.
OBJECTIVES: The aims of this study were to investigate the frequency of nickel and cobalt release in keys and to consider the effectiveness of coatings for preventing metallic allergen release from common metal allergen-releasing keys.
METHODS: Keys from a variety of common stores were nickel and cobalt spot tested. Nickel-releasing keys were coated with enamel sprays, subjected to a use test, and retested to assess

BACKGROUND: Metal hypersensitivity reactions to implanted devices remain a challenging and controversial topic. Diagnostic criteria and methods are not well delineated.
OBJECTIVE: Diagnostic criteria for hypersensitivity reactions after metallic device implantation are evaluated in this study by a multinational group of patch testers using Thyssen's previously published criteria.
METHODS: A total of 119 dermatologists at the 2012 European Contact Dermatitis Society and 2013 American Contact Dermatitis Society meetings answered a survey regarding their opinions on topics relating to metal hyp

BACKGROUND: Photoallergic contact dermatitis (PACD) is a hypersensitivity reaction that occurs when a previously photosensitized exogenous agent comes into contact with UV radiation. Currently, there are no studies profiling photoallergic reactions in Canada. Because the photoallergen profile changes over time, it is necessary to continually update our knowledge to ensure proper recognition of allergens and appropriate treatment of patients.
OBJECTIVE: This study aimed to profile photoallergic reactions in Canada.
METHODS: A retrospective chart review of all patients who underwent photopatch

BACKGROUND: Standardization of patch testing has been difficult to achieve.
OBJECTIVES: This study aimed to identify physical variations in patch test systems that could affect delivery of allergens.
METHODS: We compared the volume, depth, and contact area of 21 patch test delivery systems. We also filled a variety of patch test systems with different volumes of liquid (ferrous chloride) and petrolatum (disperse blue) allergens to investigate coverage and extrusion.
RESULTS: The depth of chambers varied from minimal to greater than 1 mm. Mean areas ranged from 50 to almost 350 mm2. In most

BACKGROUND: The question of whether quaternium-15 is a formaldehyde releaser is controversial. Understanding this relationship is critical because of the widespread use of quaternium-15 and the need to properly advise formaldehyde-allergic individuals.
OBJECTIVES: This study aimed to look for an association between allergy to quaternium-15 and formaldehyde by correlating reactions to both and to correlate the intensity of positive patch test results to formaldehyde with reactivity to quaternium-15.
METHODS: This is a retrospective analysis of 1905 patients who underwent patch testing for all
