Removal of a broken trigen intertan intertrochanteric antegrade nail.

Implant breakage is a serious complication after cephalomedullary nailing for unstable intertrochanteric fracture. Failure usually occurs at the lag screw hole in the nail body. On the other hand, lag screw failure is extremely rare and occurs around the nail-lag screw junction. We experienced rare mechanical failure of the Intertan nail, which showed breakage at the lag screw hole and failure of the integrated compression screw underneath the main lag screw.

The Immediate Intramedullary Nailing Surgery Increased the Mitochondrial DNA Release That Aggravated Systemic Inflammatory Response and Lung Injury Induced by Elderly Hip Fracture.

Conventional concept suggests that immediate surgery is the optimal choice for elderly hip fracture patients; however, few studies focus on the adverse effect of immediate surgery. This study aims to examine the adverse effect of immediate surgery, as well as to explore the meaning of mtDNA release after trauma. In the experiment, elderly rats, respectively, received hip fracture operations or hip fracture plus intramedullary nail surgery. After fracture operations, the serum mtDNA levels as well as the related indicators of systemic inflammatory response and lung injury significantly increase

Blocking screws for the treatment of distal femur fractures.

Intramedullary nailing is one of the most convenient biological options for treating distal femoral fractures. Because the distal medulla of the femur is wider than the middle diaphysis and intramedullary nails cannot completely fill the intramedullary canal, intramedullary nailing of distal femoral fractures can be difficult when trying to obtain adequate reduction. Some different methods exist for achieving reduction. The purpose of this study was determine whether the use of blocking screws resolves varus or valgus and translation and recurvatum deformities, which can be encountered in ante

Intra-articular risks of suprapatellar nailing.

To determine the risks to local anatomy near the starting point for tibial nailing during suprapatellar nailing, 15 fresh-frozen hemipelvis specimens were nailed using a suprapatellar technique. After nail passage, the menisci and articular surfaces, anterior cruciate ligament (ACL) insertion, intermeniscal ligament, and fat pad were assessed for injury. The distance from the entry portal to the menisci, articular surfaces, and ACL insertion was determined. Medial meniscus injury occurred in 1 (6.7%) specimen and medial articular injury in 2 (13%). Nails passed through the fat pad in all speci

Rush-pin fixation of supracondylar and intercondylar fractures of the femur.

A study was done of ninety-eight patients who had supracondylar and intercondylar fractures of the femur that were treated by the insertion of Rush pins. A system of classification relating the type of fracture to the method of Rush-pin fixation is described, and the importance of proper surgical technique is emphasized. The patients were followed for periods ranging from one to seventeen years (mean, 3.5 years) and 84 per cent of them had either excellent or good results, as evaluated by both radiographic and clinical examination. There were two non-unions and one deep infection. Angulation a

[The importance of magnification angiography as an investigation for the arterial supply of the femoral head after medial neck fracture (author's transl)].

In observing the development in 20 patients who had been operated upon for fracture of the neck of the femur a significant interrelation between the angiographic result and the formation of a necrosis of the head of the femur has been ascertained. The magnification angiography enables the terminal arterial region of the femoral vessels to be assessed with a still higher degree of certainty and thus appears to be a valuable diagnostic measure for the prognostic assessment of the medial fracture of the femoral neck.

[Management of humeral shaft fractures in childhood and adolescence : Consensus report of the Pediatric Traumatology Section of the DGU].

Humeral shaft fractures are rare in childhood, accounting for less than 5% of all pediatric fractures. Indirect trauma predominates in younger children, whereas direct trauma is more common in older patients. To date, no uniform recommendations exist regarding diagnostic evaluation or treatment management. Therefore, during the 13th and 14th scientific meetings of the Pediatric Traumatology Section (SKT), the available evidence was reviewed, the management strategies were critically discussed within an expert panel and a consensus was formulated. The management remains predominantly conservativ
