Loss of Myonuclei and Transcriptional Activity During Diaphragm Atrophy in Critically Ill Patients.

BACKGROUND: Diaphragm weakness frequently develops in critically ill patients and is explained by a combination of atrophy and myofiber dysfunction. Myofibers are large syncytial cells maintained by a population of myonuclei, which provide gene transcripts to a finite fiber volume, termed the myonuclear domain. Myonuclear number is a determinant of transcriptional capacity and therefore critical for diaphragm and peripheral muscle regeneration after critical illness. Changes in myonuclear number in myofibers undergoing atrophy have not been investigated in mechanically ventilated ICU patients,

[A 74-year-old man with urinary incontinence, right leg weakness and multiple cranial nerve palsies].

We report a 74-year-old man with a lung cancer, who developed right leg weakness, neurogenic bladder, and multiple cranial nerve palsies. The patient was well until December of 1992, when he was 74-year-old, when he noted transient double vision; in February of 1993, he noted numb sensation and weakness in his right leg. Later in the same month, he developed overflow incontinence of urine and weakness in his right face. He also noted deafness in his left ear (he had a marked loss of hearing in his right ear since childhood because of otitis media). His weakness in his right leg had progressed,

Low surgical routine increases revision rates after quadriceps tendon autograft for anterior cruciate ligament reconstruction: results from the Danish Knee Ligament Reconstruction Registry.

PURPOSE: Recent registry data have demonstrated a higher revision rate of quadriceps tendon (QT) graft compared with hamstring tendon (HT) and patellar tendon (PT) grafts. Clinic routines could be an important factor for revision outcomes. The purpose of this study is to use the Danish Knee Ligament Reconstruction Registry (DKRR) to compare revision rates in patients who have undergone ACLR with QT, HT and PT grafts related to individual clinic surgical routine. METHODS: Data on primary ACLRs entered in the DKRR from 2012 through 2019 were analysed since QT graft usage started in 2012. Revisi

Associations between isometric quadriceps strength characteristics, knee flexion angles, and knee extension moments during single leg step down and landing tasks after anterior cruciate ligament reconstruction.

BACKGROUND: It is unclear of how peak knee extension torque and early rate of torque development outcomes are related to lower extremity loading and sagittal plane movement in activities of daily living and landing tasks despite consistent deficits after anterior cruciate ligament reconstruction. The purpose of this cross-section study is to assess the ability of quadriceps strength characteristics to predict movement patterns during a step down and single leg drop crossover hopping tasks. METHODS: Fifty-two individuals with a unilateral history of anterior cruciate ligament reconstruction co

Modification of a sonographic enthesitis score to differentiate between psoriatic arthritis and young healthy volunteers.

OBJECTIVES: We aimed to describe sonographic structural and inflammatory changes in entheses of patients with recently diagnosed psoriatic arthritis (PsA), patients with established PsA, and young healthy volunteers, and to investigate whether the MAdrid Sonographic Enthesitis Index (MASEI) enables us to distinguish these groups in an extreme comparison. METHOD: New and established PsA patients and healthy volunteers (aged 20-30 years) were recruited. The triceps, quadriceps, patellar, Achilles and elbow extensor tendon insertion, and plantar fascia entheses were investigated sonographically

A paucisymptomatic neuromuscular disease mimicking type III 5q-SMA with complex rearrangements in the SMN gene.

Spinal muscular atrophy is an autosomal-recessive neuromuscular disorder, causing progressive proximal weakness and atrophy of the voluntary muscles. More than 96% of the spinal muscular atrophy patients show a homozygous absence of exons 7 and 8, or exon 7 only, in SMN1, the telomeric copy of the SMN gene. We report a young male patient with neurogenic symptoms and sparse muscle fiber atrophy, suggestive of a mild form of type III spinal muscular atrophy. He was found to be a carrier of intragenic mutations in both copies of the SMN gene, exhibiting a homozygous duplication of exons 7 and 8 i

Subtotal thigh flap and bioprosthetic mesh reconstruction for large, composite abdominal wall defects.

BACKGROUND: Transposition of well-vascularized flap tissue with or without implantable mesh is often required to repair full-thickness, composite abdominal wall defects. The authors hypothesized that the combination of an inlay of bioprosthetic mesh and a subtotal thigh flap would enable a reliable reconstruction for large, composite abdominal wall defects. METHODS: The authors retrospectively reviewed data on patients who underwent repair of large, composite abdominal wall defects with bioprosthetic mesh and free or pedicled subtotal thigh flaps at a major cancer center from 2004 to 2007. Modulatory effect of gastric pentadecapeptide BPC 157 on angiogenesis in muscle and tendon healing.

Angiogenesis is a natural and complex process controlled by angiogenic and angiostatic molecules, with a central role in healing process. One of the most important modulating factors in angiogenesis is the vascular endothelial growth factor (VEGF). Pentadecapeptide BPC 157 promotes healing demonstrating particular angiogenic/angiomodulatory potential. We correlated the angiogenic effect of BPC 157 with VEGF expression using in vitro (cell culture) and in vivo (crushed muscle and transected muscle and tendon) models. Results revealed that there is no direct angiogenic effect of BPC 157 on cell

ACL Rehabilitation Plateaus Across Athlete Populations: Two Case Reports and a Clinical Framework for Identifying Functional Stagnation.

BACKGROUND AND PURPOSE: Anterior cruciate ligament (ACL) rehabilitation typically follows a structured protocol for progressive recovery. However, not all subjects achieve expected functional milestones, despite adherence. Rehabilitation plateaus-defined as stagnation in physical progress-are often underrecognized in musculoskeletal cases. The concept of rehabilitation plateaus remains underexplored in musculoskeletal physiotherapy, particularly in ACL reconstruction recovery. The purpose of this case series was to explore clinical rehabilitation plateaus that occur during ACL reconstruction r

[Postoperative delirium in the elderly].

Postoperative delirium (POD) is a disorder of attention and consciousness accompanied by cognitive impairment as a direct consequence of a medical condition, the action (or discontinuation) of a drug, toxin, psychoactive substance or the simultaneous action of several factors. To describe the current state of knowledge on delirium, the Medline PubMed database was searched for full-text articles from the years 2000-2025 using the keywords: "postoperative delirium," "prediction" and "elderly." Fifty-six articles were selected for the final analysis. Risk factors for POD are: older age, sensory i

From Pain Control to Early Mobility: The Evolution of Regional Anesthesia in Geriatric Total Hip Arthroplasty.

The evolution of regional anesthesia in total hip arthroplasty (THA) has significantly impacted perioperative management, particularly in older adults, where age-related physiological vulnerability requires optimized strategies. Adequate pain control is crucial in enhancing recovery, minimizing opioid consumption, and reducing complications. Traditional nerve blocks such as lumbar plexus and femoral nerve blocks have long been the mainstay of analgesia. However, they are associated with significant motor impairments, which delay mobilization and increase the fall risks. Introducing motor-spari

Arthroscopic Anterior Cuciate Ligament Reconstruction Using Neither a Tourniquet nor Drainage: A Perioperative Case Series Report.

INTRODUCTION: Many orthopedic surgeons recommend ischemic tourniquets during arthroscopic anterior cruciate ligament (ACL) repair to reduce blood loss and improve visibility. However, their use remains controversial due to potential complications. Similarly, the practice of postoperative drainage is debated. While its proponents argue it reduces limb swelling, DVT, adhesions, and stiffness, others contend that it may increase infection risk or harm the ACL graft and joint surfaces. MATERIALS AND METHODS: A total of 456 patients underwent anterior cruciate ligament reconstruction between Septe

Muscular tissue desaturation and pneumonia in patients receiving lung cancer surgery: a cohort study.

BACKGROUND: Post-operative pneumonia (POP) is a common complication of lung cancer surgery, and muscular tissue oxygenation is a root cause of post-operative complications. However, the association between muscular tissue desaturation and POP in patients receiving lung cancer surgery has not been specifically studied. This study aimed to investigate the potential use of intra-operative muscular tissue desaturation as a predictor of POP in patients undergoing lung cancer surgery. METHODS: This cohort study enrolled patients (≥55 years) who had undergone lobectomy with one-lung ventilation. Mus
