Latent tuberculosis infection in a Malaysian prison: implications for a comprehensive integrated control program in prisons.

BACKGROUND: Prisons continue to fuel tuberculosis (TB) epidemics particularly in settings where access to TB screening and prevention services is limited. Malaysia is a middle-income country with a relatively high incarceration rate of 138 per 100,000 population. Despite national TB incidence rate remaining unchanged over the past ten years, data about TB in prisons and its contribution to the overall national rates does not exist. This survey was conducted to address the prevalence of latent TB infection (LTBI) in Malaysia's largest prison. METHODS: From July to December 2010, all HIV-infect

Population-based contact investigation of a cluster of tuberculosis cases in a small village.

A cluster of five cases of tuberculosis (TB) in persons aged 19-23 years who were not close contacts was detected in a small village in Spain in 2006. All culture isolates had the same chromosomal-DNA restriction pattern. Contact investigations of family members, friends, workmates and schoolmates were complemented with tuberculin screening offered to the resident population born between 1976 and 1995. Expanded contact tracing detected two new cases of TB, 27 tuberculin conversions and an excess of latent tuberculosis infections (LTI) in persons born between 1978 and 1990. The contacts of two

Screening and treatment of latent tuberculosis in a cohort of asylum seekers in Norway.

AIMS: Asylum seekers are screened for tuberculosis at entry to Norway. We aimed to assess follow-up of screening results at different healthcare levels in relation to demographics, screening results and organizational factors, and how this influenced treatment of latent tuberculosis. METHODS: All asylum seekers >or=18 years with a Mantoux test >or=6 mm or positive x-ray findings who arrived at the National Reception Centre from January 2005 to June 2006, were included. Data were collected from public health authorities in the municipality where the asylum seekers had moved, and from internist

Squamous cell carcinoma arising from long-term (50-year) Lupus vulgaris: is there a need for a close medical follow-up in such chronic diseases?

Skin manifestations of tuberculous infection (Mycobacterium tuberculosis) are represented by miliary tuberculosis of the skin, tuberculous chancre, scrofuloderma, tuberculosis verrucosa cutis, periorificial tuberculosis, and lupus vulgaris (LV). Among this group, LV is the most common skin condition, diagnosed in 10% of tuberculotic patients. The authors report herein a case of squamous cell carcinoma (SCC) arising from long-standing (50-year) LV and underline the need of an extensive follow-up of tuberculotic lesions.

Disseminated erythematous papules in a renal transplant recipient: a case of disseminated tuberculosis.

The incidence of tuberculosis in renal transplant patients has been reported to be fairly low and disseminated tuberculosis presenting with cutaneous manifestations has not been reported previously in these patients. We report the case of a 62-y-old kidney transplant recipient with a long-term history of chronic immunosuppressive therapy who presented with high fever, rapidly progressing respiratory failure and disseminated erythematous pustular cutaneous lesions.

Obstacles encountered in application of the Centers for Disease Control and Prevention guidelines for control of tuberculosis in a large dental center.

BACKGROUND: The Centers for Disease Control and Prevention (CDC) and the Occupational Safety and Health Administration have designated five categories of workplaces as carrying higher than normal risk for exposure to tuberculosis (TB); "health care facilities" is one of these categories. To assist all health care facilities in developing an appropriate and effective control plan, the CDC has listed various components to be included in the overall plan-however, the components needed cannot be determined until the level of risk has been determined. The published criteria for risk assessment are

Clinical features of HIV-seropositive and HIV-seronegative patients with tuberculous pleural effusion in Dar es Salaam, Tanzania.

In a prospective study, we investigated whether human immunodeficiency virus (HIV) infection alters the clinical presentation in patients with tuberculous pleuritis. One hundred twelve of 118 patients who presented with pleural effusion suffered from tuberculosis (TB); 65 patients (58%) were HIV seropositive. Evidence of disseminated TB was found more often in HIV-positive than in HIV-negative patients (30.8% vs 10.6%, p < 0.02). Dyspnea, fever, night sweat, fatigue, and diarrhea, severe tachypnea, hepatomegaly, splenomegaly, and lymphadenopathy were significantly more common in HIV-infected t

There is no group proof of long acting antirheumatics (LAA) in laboratory animal models, and it is not to be expected without an identical rheumatoid arthritis model in animals and with regard to the heterogeneity of LAA. However, LAA are to be detected according to D-penicillamine-like, levamisol-like etc. actions, which can be disclosed in the adjuvant arthritis as well as in the B. pertussis-vaccine pleuritis in rats the latter model best by including parameters of inflammatory exudate cells. Immune Responsiveness Induced by Administration of Heat-Inactivated M. bovis and BCG Prior to Experimental Infection in African Buffalo.

African buffalo (Syncerus caffer) are the primary wildlife reservoir for bovine tuberculosis (BTB) in Africa. Infected buffalo populations spread BTB to numerous species in conservation areas as well as to cattle at park boundaries. Quarantine requirements for BTB complicate wild animal metapopulation management. Considerable interest has arisen in the vaccination of cattle and indigenous wildlife species against BTB as an additional control strategy. This pilot trial aimed to evaluate cell-mediated and humoral immune responses in African buffalo immunised against BTB. Twenty-one sub-adult Afr

Disseminated Tuberculosis With Ocular Involvement After 12 Years of Anti-TNF-α Therapy.

BACKGROUND: Ocular tuberculosis (TB) is an uncommon yet potentially vision-threatening manifestation of Mycobacterium tuberculosis, particularly difficult to diagnose in immunocompromised individuals. The increasing use of anti-tumor necrosis factor-α (anti-TNF-α) agents has markedly heightened the risk of TB reactivation, often with disseminated or extrapulmonary involvement despite negative baseline screening. CASE PRESENTATION: We describe a 52-year-old woman with rheumatoid arthritis receiving long-term adalimumab and methotrexate who developed disseminated TB with pulmonary, splenic, and

Upper respiratory tract commensal Streptococcus oralis inhibits mycobacterial growth by producing bioactive molecules and inducing macrophage autophagy.

BACKGROUND: Tuberculosis (TB) is caused by Mycobacterium tuberculosis (Mtb) and remains a major global health challenge. Although host immunity is critical for controlling infection, the role of upper respiratory tract commensal microbiota in anti-mycobacterial defense remains unclear. This study investigated whether commensal bacteria from close contacts of patients with TB, who remained persistently tuberculin skin test- (TST) and QuantiFERON-TB (QFT) Gold-negative after long-term exposure, produce factors capable of inhibiting mycobacterial growth. METHODS: Throat swabs were collected from

Comprehensive tuberculosis screening and preventive treatment in schools and congregate settings of India (2017-2024): a prospective study.

BACKGROUND: Existing Tuberculosis (TB) elimination strategies show limited impact, with suboptimal uptake of tuberculosis preventive treatment (TPT) and increasing TB incidence after the COVID-19 pandemic. Real-world evidence on reduction of tuberculosis in high-burden communities is needed to inform future TB elimination strategies. METHODS: Since 2017, a comprehensive TB screening and TPT program known as Zero TB in Kids (ZTBK) was implemented in congregate settings of Tibetan communities in India. TB disease, TB infection (TBI), tuberculin skin test (TST) conversion, and TPT uptake were me

Ankylosing spondylitis and psoriatic arthritis: revisiting screening of latent tuberculosis infection and its follow-up during anti-tumor necrosis factor therapy in an endemic area.

OBJECTIVES: To retrospectively evaluate the performance and distinctive pattern of latent tuberculosis (TB) infection (LTBI) screening and treatment in patients with ankylosing spondylitis (AS) and psoriatic arthritis (PsA) under anti-tumor necrosis factor (TNF) therapy and determine the relevance of re-exposure and other risk factors for TB development. METHODS: A total of 135 and 83 patients with AS and PsA, respectively, were evaluated for LTBI treatment before receiving anti-TNF drugs via the tuberculin skin test (TST), chest radiography, and TB exposure history assessment. All subjects w

Targeting of tolerogenic dendritic cells to heat-shock proteins in inflammatory arthritis.

BACKGROUND: Autologous tolerogenic dendritic cells (tolDC) are a promising therapeutic strategy for inflammatory arthritis (IA) as they can regulate autoantigen-specific T cell responses. Here, we investigated two outstanding priorities for clinical development: (i) the suitability of using heat-shock proteins (HSP), abundant in inflamed synovia, as surrogate autoantigens to be presented by tolDC and (ii) identification of functional biomarkers that confirm tolDC regulatory activity. METHODS: Cell proliferation dye-labelled human peripheral blood mononuclear cells of IA (rheumatoid arthritis
