T-cell inhibitors: a bench-to-bedside review.

Allergic contact dermatitis is the quintessential example of a delayed-in-time and T-cell-mediated immune response. In the last decade, many of the molecular events required to initiate (or block) such a response have been uncovered. Textbook and journal reviews have emphasized the costimulatory requirements, with less focus on the coinhibitory signals that are of equal importance in understanding this central event of adaptive immunity. To fill this gap, we offer a compendium of discoveries characterizing the ligand-receptor pairs inhibiting T-cell activation and of selected illnesses and the

Tea tree oil.

Tea tree oil is an increasingly popular ingredient in a variety of household and cosmetic products, including shampoos, massage oils, skin and nail creams, and laundry detergents. Known for its potential antiseptic properties, it has been shown to be active against a variety of bacteria, fungi, viruses, and mites. The oil is extracted from the leaves of the tea tree via steam distillation. This essential oil possesses a sharp camphoraceous odor followed by a menthol-like cooling sensation. Most commonly an ingredient in topical products, it is used at a concentration of 5% to 10%. Even at this

Fire-retardant clothing-related dermatitis.

BACKGROUND: Several laborers in a refinery unit undergoing a work over noted skin eruptions. Signs and symptoms included erythema, pruritus, scaling, and perifollicular inflammation on skin contacted by fire-retardant clothing (FRC). OBJECTIVE: The purposes of this report were to show a correlation between this rash outbreak and the use of FRC, to report the investigative results as to what aspect of the FRC most likely caused the dermatitis, and to present how this outbreak was ended. METHODS: Employees received questionnaires, were examined, and received patch testing, and pH testing of FR

Contact allergy to sorbitans: a follow-up study.

BACKGROUND: Sorbitan sesquioleate (SSO), an emulsifier in many corticosteroids, was previously found positive in 8.9% of 112 dermatitis patients. OBJECTIVE: The objective of this study was to present data on 24 of 591 dermatitis patients with reactions to SSO and/or sorbitan monooleate (SMO) on patch testing. METHODS: A retrospective chart review was conducted on 591 consecutive dermatitis patients patch tested from November 2008 to May 2010. In addition to being tested to a modified North American Contact Dermatitis Group standard series, all patients were tested to a cosmetic series. RESU

Fragrance series testing in eyelid dermatitis.

BACKGROUND: Allergic contact dermatitis is considered one of the most common causes of eyelid dermatitis. In addition to metals and topical antibiotics, fragrances have emerged as a leading source of contact allergy for individuals with this condition. OBJECTIVES: The objective of this study was to determine the added benefit of including a fragrance tray when patch testing patients presenting with eyelid dermatitis. METHODS: During a 4.5-year period, all patients with suspected allergic contact dermatitis involving the eyelids were patch tested with both standard and fragrance trays. RESUL

The Sino-American belt study: nickel and cobalt exposure, epidemiology, and clinical considerations.

BACKGROUND: Nickel and cobalt are common causes of metal allergy. OBJECTIVE: The objective of this study was to investigate nickel and cobalt exposure in belt buckles by testing 701 belts purchased in China and the United States and to consider the prevalence of nickel allergy and its relevance among Chinese patients. METHODS: Seven hundred one belt buckles purchased in China and the United States were tested for nickel and cobalt release. Six hundred thirty-one Chinese patients with suspected allergic contact dermatitis were patch tested and interviewed to determine clinical relevance of re

Contact dermatitis to topical medicaments: a retrospective chart review from the Ottawa Hospital Patch Test Clinic.

BACKGROUND: Topical medicaments are a common cause of allergic contact dermatitis. This study will evaluate the prevalence of contact allergy to a wide array of topical medicaments at the Ottawa Patch Test Clinic. OBJECTIVES: The objectives of this study are to report the results of positive patch testing to topical medicaments at the Ottawa Patch Test Clinic and identify common sensitizers in topical medicaments. METHODS: Patients were tested with the standard North American Contact Dermatitis screening series of 70 allergens plus supplementary allergens when indicated. A retrospective char

Allergic contact dermatitis in children with and without atopic dermatitis.

BACKGROUND: Prevalence and causes of allergic contact dermatitis (ACD) in children vary with time and geographical area. OBJECTIVE: This study aimed to determine the relevant allergens causing ACD in children and the relation between ACD and atopic dermatitis (AD). METHODS: A cohort study on 349 children (0-15 years old) patch tested over a 7-year period was conducted. RESULTS: Patch test results were positive for at least 1 allergen in 69.3% of patients and were relevant in 69.8%. The highest sensitization rate (76.7%) was observed in children who are 0 to 5 years old (n = 86, 64% females)

p-tert-Butylphenol formaldehyde resin and its impact on children.

: p-tert-Butylphenol formaldehyde resin (PTBPFR) is widely used as an adhesive owing to its rapid onset of action, durability, flexibility, and heat resistance. Moreover, because of its tackiness, it is particularly useful as a glue component on both leather and rubber products. Sensitization to this resin has been increasing since the first cases of PTBPFR contact allergy in the late 1950s, and pediatric contact allergy to this resin is a significant problem. p-tert-Butylphenol formaldehyde resin has been reported as a leading cause of shoe-associated dermatitis internationally, as well as a

The role of contact allergens in chronic idiopathic urticaria.

OBJECTIVE: The objective of this study was to determine whether contact allergens play a role in chronic idiopathic urticaria (CIU). METHODS: We conducted a longitudinal prospective study of 23 patients with CIU. Patients were patch tested to a modified North American Contact Dermatitis Group standard, fragrance, and cosmetic series; other series were tested as warranted by relevant history and physical examination. Readings were performed at 48 and 72 hours. Patients were counseled to avoid proven contact allergens and were followed up 2 to 9 months after testing. RESULTS: Twenty-one of 23
