Pathologic correlates of electronystagmographic tracings.

Electronystagmographic tracings were correlated with the histopathologic findings in the peripheral vestibular system of 66 temporal bones of 34 patients who complained of dizziness. Although some findings (e.g., spontaneous nystagmus, unilaterally reduced caloric response, and directional preponderance) seemed to have doubtful value in localizing vestibular system disease, other findings indicated a peripheral site of the vestibular lesions. Bilaterally reduced or bilaterally absent caloric responses were consistently associated with pathologic changes in the peripheral vestibular system. Sim

Hyposalivation: the roles of radioactive iodine and stapes surgery.

PURPOSE: The aim of this study is to call attention to the role that radioactive iodine ((131)I) and stapes surgery may play in causing hyposalivation. MATERIALS AND METHODS: The manner in which (131)I and stapes surgery can cause salivary damage was reviewed. A case report is presented to illustrate the involved pathophysiology. RESULTS: The case report clearly shows the significant injury to the parotid glands caused by the (131)I. However, subjective symptoms of oral dryness only developed after injury to the chorda tympani nerve (CTN) during stapes surgery. CONCLUSIONS: The loss of func

Persistent and recurrent conductive deafness following stapedotomy.

BACKGROUND: Stapedotomy produces the best hearing results of any otological operation. However, in a small number of cases the air-bone gap is not successfully closed, or conductive hearing loss recurs. OBJECTIVE: To investigate the proportion of cases in the senior author's series which required revision surgery and to determine the medium to long term success rate of surgery, taking into account the results of revision surgery. METHODS: Review of a series of 233 primary stapedotomy operations and 100 revision procedures carried out by one surgeon over a 20-year period. RESULTS: Closure of

The relationship of continuous (C) and interrupted (I) tones at threshold and most comfortable loudness (MCL) in hearing-impaired ears.

Differences in the Bekesy tracking levels of continuous (C) and interrupted (I) tones at both threshold and most comfortable loudness (MCL) were determined in a large clinical sample of persons with hearing loss. Results indicate small but significant C-I differences not only at the two levels separately but also in the tracking of C and I tones at threshold relative to MCL, regardless of frequency or hearing loss severity. Such results may have implications for both diagnostic and rehabilitative audiology.

Reporting in stapes surgery: are we following the guidelines?

OBJECTIVE: This paper highlights the importance of reporting air-bone gap closure in stapes surgery according to the American Academy of Otolaryngology - Head and Neck Surgery guidelines and reviews compliance in recent years. METHODS: A retrospective case series was conducted and the outcomes were reviewed. Closure of the air-bone gap was calculated in 204 adult patients using the aforementioned guidelines. Results were recalculated ignoring the Carhart phenomenon to determine any significant difference. Adherence to guidelines was also reported as a secondary outcome. RESULTS: Ignoring the

Early postoperative results in stapedectomy.

OBJECTIVE: To measure early audiometric changes after primary stapedotomy. STUDY DESIGN: Case series with chart review. SETTING: Academic inner-city hospitals. SUBJECTS AND METHODS: Consecutive patients operated on by the first author were included (only 1 ear in cases of bilateral surgery), and their audiometric results were reviewed. Data were analyzed from 45 ears. Air and bone audiometric measures were analyzed from 5 days post operation and 6 months post operation. Threshold shifts were quantified at 5 days and 6 months post operation. RESULTS: Overall results for the group were good,

Long-term results of the Er-Yag laser used in stapes surgery.

Stapes surgery is performed using both conventional techniques and a variety of laser systems. The Er-Yag laser is a type of pulsation laser deemed by many to be the safest for ear surgery. The aim of this study was to assess the long-term effects of using the Er-Yag laser in stapes surgery and its impact on inner ear function. The study was conducted on 95 ears (48 right and 47 left) that had undergone Er-Yag laser-assisted stapedotomy. The follow-up time was at least 3 years. Pure tone audiometry was performed on all patients. The frequency ranged from 125 to 8,000 Hz. Air conduction and bon

[Long term sequelae of otitis media with effusion during childhood].

INTRODUCTION: Otitis media with effusion is the most common cause of hearing loss in children and myringotomy with tympanostomy tube insertion is recommended procedure to deal with the problem. The objective of the present study was to determine the results of treatment, incidence and prevalence of middle ear sequelae and hearing results among children with chronic otitis media with effusion who received standard treatment with tympanostomy tubes. MATERIAL AND METHODS: The group of 97 patients treated by tympanostomy tubes insertion in the years 1999-2001 was reevaluated after mean period of

Postoperative complications in otospongiosis: usefulness of MR imaging.

BACKGROUND AND PURPOSE: Sensorineural hearing loss (SNHL) is a rare complication of stapes surgery that may arise for many reasons. Usually, the pathogenesis of SNHL can be established by clinical and CT examinations. The purpose of this study was to evaluate the utility of MR imaging when CT findings are normal or not contributive. METHODS: Eleven patients with SNHL (in some instances, associated with vertigo) after stapedectomy, in whom CT showed no well-defined cause, were examined by MR imaging. RESULTS: MR studies established the additional findings of reparative intravestibular granulo

Topical application of calcium channel blockers to reduce the progression of experimentally induced myringosclerosis and tympanosclerosis.

OBJECTIVES: This study aimed to evaluate the ability of topically applied calcium channel blockers (diltiazem) to reduce the progression of experimentally induced myringosclerosis and tympanosclerosis. STUDY DESIGN: Animal model. Experimental prospective study. METHODS: The study included 25 adult albino guinea pigs that were bilaterally myringotomized and inoculated with a suspension of Streptococcus pneumonia type 3. The right ears were treated with topical application of diltiazem, and the untreated left ears served as the control group. Otomicroscopy and remyringotomy were conducted ever

The effect of stapes mobility on hearing outcome and which procedure to choose in fixed stapes in children tympanosclerosis.

OBJECTIVES: Our objective was to evaluate the features of tympanosclerosis in children and to determine the effect of stapes mobility and the type of one-stage operation on hearing outcomes. MATERIALS AND METHODS: Fifty-one children who were performed different types of single-stage otologic surgery for tympanosclerosis between January 1997 and December 2006 were retrospectively chart reviewed. The children were divided into two groups according to the mobility of ossicular chain, especially the stapes. Stapes fixed group was also evaluated in detail according to the type of surgery that was

The unilateral stapes gusher.

A stapes gusher is an alarming situation occurring during stapes surgery for mixed hearing loss. It is characterized by the sudden profuse flow of cerebrospinal fluid in the middle ear and external ear canal after opening the vestibule of the inner ear. An adult patient with unilateral stapes gusher is described; he was operated on both ears with a good hearing result on both sides. The stapes on the side with the cerebrospinal fluid leak was malformed, especially in the posterior crus--a finding which may suggest the possibility of a stapes gusher. The small-hole stapedotomy is a safe techniq
