Treatment pathways and outcomes for pediatric complex appendicitis patients presenting beyond 3 days: A retrospective cohort study.

BACKGROUND: The optimal management of complicated appendicitis in children whose symptom duration exceeds 3 days remains controversial. This study compared the outcomes of different treatment strategies in this population. METHODS: We conducted a retrospective analysis of pediatric complex appendicitis patients treated at Zhejiang University Children’s Hospital between June 2017 and May 2024. The clinical data, treatment approaches, and outcomes were analyzed. RESULTS: A total of 542 children were included in this study. Acute-phase surgery during the initial episode resulted in lower readmiss

A Single Institute Review of 4,428 Consecutive Appendiceal Resections: Incidence of Suspected and Incidental Pathology.

AIMS: To determine the incidence of non-appendicitis pathology of the appendix in an Irish context. METHODS: A single institute review of 4,428 consecutive appendiceal resections was conducted independently by two researchers. Clinical, radiological, intra-operative, and histological reports were reviewed. RESULTS: A total of four-thousand four-hundred and twenty-eight (4,428) appendicectomies including right hemicolectomies, subtotal colectomies, panproctocolectomies, ileocolic resections, and en-bloc resections were performed during the study period. Incidental malignancies occur in less t

Appendico-Sigmoid Fistula as a Rare Complication of Chronic or Recurrent Appendicitis: A Case Report.

BACKGROUND Appendico-colic fistula is a rare condition characterized by an abnormal communication between the appendix and colon, most commo … sigmoid colon with surrounding inflammatory adhesions. The patient underwent sigmoid colectomy with appendectomy, colorectal anastomosis, and diverting loop ileostomy. Histopathology demonstrated inflammatory changes without evidence of diverticula or malignancy. The postoperative course was uneventful, and ileostomy reversal was performed after 2 months. CONC …

Rising Multidrug-Resistant Pathogens in Pediatric Appendicitis: A Decade-Long Study from Southern Israel.

Appendicitis is the most common indication for emergent abdominal surgery in childhood. Intravenous antibiotic therapy reduces infections-related complications. Epidemiological data concerning bacterial pathogens are important in tailoring antibiotic stewardship recommendations. This study aims to assess trends over the years in bacterial distribution, and sensitivities (specifically multidrug-resistant [MDR] Enterobacteriaceae) in surgical appendicitis among two different ethnic communities in southern Israel. This was an observational, single-center, retrospective study. Included were children

What is the appropriate management of acute uncomplicated appendicitis in contemporary South Africa?

Acute appendicitis has afflicted humankind throughout our existence but has been a surgical disease for little over one hundred and fifty years. Leonardo da Vinci first drew the appendix in 1508, and several anatomists described it over the next two centuries. These included Berengarius in 1522, Andreas Vesalius in 1543 and Giovanni Battista Morgagni in 1719.1 Several pathologists began to define and elucidate the pathophysiology of acute appendicitis and a number of authors from the enlightenment onwards began to describe what was termed typhlitis until Fitz, described a large series of acute

Appendicectomy plus standard medical therapy versus standard medical therapy alone for maintenance of remission in ulcerative colitis (ACCURE): a pragmatic, open-label, international, randomised trial.

BACKGROUND: The appendix might have an immunomodulatory role in ulcerative colitis. Appendicectomy has been suggested as a potentially therapeutic intervention to maintain remission in ulcerative colitis. We aimed to evaluate the clinical effectiveness of laparoscopic appendicectomy in maintaining remission in patients with ulcerative colitis. METHODS: We did a pragmatic, open-label, international, randomised controlled superiority trial in 22 centres across the Netherlands, Ireland, and the UK. Patients with established ulcerative colitis who were in remission but had been treated for diseas

[Impact of the coronavirus SARS-CoV-2 pandemic on the presentation and management of acute appendicitis in children].

UNLABELLED: Acute appendicitis (AA) is the most frequent cause of acute surgical abdomen in pediatrics. During the COVID-19 pandemic, lockdown slowed surgical processes, delaying medical consultations. OBJECTIVE: To analyze the impact of the pandemic on the presentation and management of acute appendicitis. PATIENTS AND METHOD: Retrospective cohort, which included patients under 15 years of age seen in the emergency department (ED) with a diagnosis of acute appendicitis, from March to December of the pre-pandemic period of 2018 and the pandemic period in 2020. Demographic data, symptoms, tre

The nexus between appendicitis and chronic inflammatory bowel diseases: Unraveling an intriguing association.

Appendicitis is 1 of the most frequent diseases worldwide. In general, it is treated with appendectomy, which, in almost all cases, leads to the healing of the disease and averts acute complications. However, only limited data regarding long-term sequalae, including inflammatory bowel diseases following appendicitis are available. We therefore investigated the association between appendicitis and both Crohn disease (CD) and ulcerative colitis (UC). The present study included 23,991 patients with a history of appendicitis and 23,991 that did not have such a history. Patients were identified wit…

[Influence of the coronavirus 2 (SARS-Cov-2) pandemic on acute appendicitis].

INTRODUCTION: Acute appendicitis (AA) is the most common abdominal surgical emergency. No specific studies have been found that evaluate the impact of the coronavirus 2 (SARS-Cov-2) pandemic on AA and its surgical management. An analysis was made on the influence of this new pathology on the clinical course of AA. MATERIAL AND METHODS: Retrospective observational study was conducted on patients operated on for AA from January to April 2020. They were classified according to the time of the appendectomy, before the declaration of the state of alarm (pre-COVID-19), and after its declaration (po…

Volume of Irrigation Does Not Affect Rate of Abscess in Perforated Appendicitis.

Background: Although previous studies have evaluated whether use of irrigation decreases postoperative intraabdominal abscess (PO-IAA) formation, these studies treated irrigation as a dichotomous variable and concluded that no irrigation resulted in a decreased incidence of PO-IAA formation. However, a recent study found decreased incidence with small aliquots to a total volume of 6 L. We hypothesized that higher volumes of irrigation would result in a lower incidence of PO-IAA. Materials and Methods: A postoperative template was developed as a quality improvement initiative and included descr

Low-Grade Appendiceal Mucinous Neoplasms: A Single Institution Experience of 64 Cases With Clinical Follow-up and Correlation With the Current (Eighth Edition) AJCC Staging.

Background. In this single-institution study, we applied the current (eighth edition) American Joint Committee on Cancer pathologic staging criteria to 64 low-grade mucinous neoplasms of the appendix (LAMNs), examined their histopathologic features, and studied the patients' clinical outcomes. Design. Sixty-four LAMNs, with a median follow-up of 52 months, were reviewed. Results. The distribution of pathologic stages was pTis (n = 39), pT3 (n = 1), pT4a (n = 5), pT4aM1a (n = 8), and pT4aM1b (n = 11). Recurrence was observed in only 2 patients (both with pT4aM1b disease), one of whom died of di

Prospective evaluation of a clinical response directed pathway for complicated appendicitis.

AIM: Despite evidence to suggest shorter durations of antibiotics are safe in complicated appendicitis, the practice has not been widely adopted in the UK. Our aim was to determine whether a clinical pathway that tailors antibiotics to clinical condition was safe and effective. METHODS: A new post-operative pathway (NewPath) was devised that reduced mandatory intravenous antibiotics for complicated appendicitis (perforated or gangrenous) from 5 to 3 days post-operatively, provided the child was apyrexial for >12 h and tolerating oral diet. Oral antibiotics were only given if white-blood-cell

Hidden diagnosis behind viral infection: the danger of anchoring bias.

Anchoring bias is one of the most common diagnostic biases that may lead to closed-minded thinking and could result in unnecessary tests, inappropriate patient management and even misdiagnosis. A 4-year-old boy was brought to the emergency department because of shaking chills. On the basis of bilateral swollen preauricular areas, high level of serum amylase and the prevalence of mumps, he initially received a diagnosis of mumps in spite of the shaking chills. However, blood culture turned out to be positive for two different kinds of bacteria. The patient finally received a diagnosis of polymi
