A Scoping New Revolution for Tinnitus Management: A Neuromonics Narrative Review.

With a prevalence ranging from 16-21%, tinnitus is an irking neurological disorder, manifesting as the perception of a roar of cacophony without an external acoustic stimulus. Although tinnitus is a non-life-threatening symptom, tinnitus's negative repercussions, such as personality disturbances and social withdrawal, are daunting concerns despite a gamut of conventional and emerging treatment options. Tinnitus management is still a critical challenge that provides opportunities for clinicians, ENT (Ear/eye, nose and throat) physicians, and other specialists such as audiologists, psychiatrists

Hearing of Otof-deficient mice restored by trans-splicing of N- and C-terminal otoferlin.

Mutations to the OTOF gene are among the most common reasons for auditory neuropathy. Although cochlear implants are often effective in restoring sound transduction, there are currently no biological treatments for individuals with variants of OTOF. Previous studies have reported the rescue of hearing in DFNB9 mice using OTOF gene replacement although the efficacy needs improvement. Here, we developed a novel dual-AAV-mediated gene therapy system based on the principles of protein trans-splicing, and we show that this system can reverse bilateral deafness in Otof -/- mice after a single unilat

Rethinking Emergent Literacy in Children With Hearing Loss.

Long-term literacy outcomes for children with hearing loss, particularly those with severe-to-profound deafness who are fitted with cochlear implants (CIs) lag behind those of children with normal hearing (NH). The causes for these long-term deficits are not fully clear, though differences in auditory access between children who use CIs and those with NH may be a partial cause. This paper briefly reviews the emergent literacy model as proposed by Whitehurst and Lonigan (1998). We then examine the development of each of Whitehurst and Lonigan's identified factors in children who use CIs and how

Speak on time! Effects of a musical rhythmic training on children with hearing loss.

This study investigates temporal adaptation in speech interaction in children with normal hearing and in children with cochlear implants (CIs) and/or hearing aids (HAs). We also address the question of whether musical rhythmic training can improve these skills in children with hearing loss (HL). Children named pictures presented on the screen in alternation with a virtual partner. Alternation rate (fast or slow) and the temporal predictability (match vs mismatch of stress occurrences) were manipulated. One group of children with normal hearing (NH) and one with HL were tested. The latter group

[Neurofibromatosis type 2].

Type 2 neurofibromatosis (NF2) is an invalidating, inherited, dominant, autosomal disease. It is commonly confused with type 1 neurofibromatosis, although the two disorders are different. All subjects who inherit a mutated NF2 gene will develop the disease, which is characterised by the growth of schwannomas, generally affecting the vestibular nerve bilaterally, as well as meningiomas and other benign central nervous system tumours, before their third decade of life. It is currently possible to identify the NF2 mutation in most affected families. Up to about 20% of NF2 patients with no family

Hearing loss diagnosis followed by meningitis in Danish children, 1995-2004.

OBJECTIVE: A higher risk of meningitis associated with cochlear implants may be explained in part by a generally higher risk of meningitis in children with severe to profound hearing loss. We investigated whether children with hearing loss have an increased risk of meningitis. STUDY DESIGN AND SETTING: A historical cohort study of all children born in Denmark between January 1, 1995, and December 31, 2004, was conducted. The cohort was selected through the Danish Medical Birth Registry, and information on hearing loss and meningitis was obtained from the National Hospital Registry. RESULTS:

Neurophysiologic bases of speech discrimination.

The mismatch negativity (MMN) is an automatic cortical evoked potential that signifies the brain's detection of acoustic change. In other words, the MMN reflects the neurophysiologic processes that underlie auditory discrimination. As such, the MMN provides an objective tool for evaluating central auditory mechanisms involved in speech perception. We are using the MMN to study the central auditory processes that encode acoustic changes important for speech perception in 1) normal-hearing adults and children, 2) individuals with impaired auditory systems (including persons with learning disabil

Cochlear dysplasia and meningitis.

Congenital dysplasias of the labyrinth of the inner ear are associated with varying degrees of hearing loss. There is a risk of a fistulous communication between the subarachnoid space and the middle ear cavity in some cases that present either as cerebrospinal fluid otorhinorrhea or as recurrent attacks of meningitis. The types of deformity where such a fistula is likely have not been clearly defined. The authors correlated the hearing state with the imaging assessment in 20 patients with congenital malformation of the labyrinth and, in particular, the cochlea. In addition the postmortem hist

Auditory brainstem implant: electrophysiologic responses and subject perception.

OBJECTIVES: The primary aim of this study was to compare the perceptual sensation produced by bipolar electrical stimulation of auditory brainstem implant (ABI) electrodes with the morphology of electrically evoked responses elicited by the same bipolar stimulus in the same unanesthetized, postsurgical state. Secondary aims were to (1) examine the relationships between sensations elicited by the bipolar stimulation used for evoked potential recording and the sensations elicited by the monopolar pulse-train stimulation used by the implant processor, and (2) examine the relationships between evo

Responses and Nonresponses in a Bound Morpheme Elicitation Task by Deaf and Hard of Hearing Children.

PURPOSE: We aimed to explore the rates of bound morpheme production at two time points (T1 and T2) by deaf and hard of hearing (DHH) preschoolers and their typically hearing (TH) peers. We further sought to describe the rates and types of unscorable responses children produced. METHOD: Sixty-four DHH preschoolers and 66 TH preschoolers participated as part of a larger, ongoing longitudinal study. Children were given the Test of Early Grammatical Impairment (TEGI) screener, which elicits productions of the third-person singular present and past tense. TEGI screeners were given twice, spaced 6

Tinnitus Treatment Using Noninvasive and Minimally Invasive Electric Stimulation: Experimental Design and Feasibility.

Noninvasive transcranial or minimally invasive transtympanic electric stimulation may offer a desirable treatment option for tinnitus because it can activate the deafferented auditory nerve fibers while posing little to no risk to hearing. Here, we built a flexible research interface to generate and control accurately charge-balanced current stimulation as well as a head-mounted instrument capable of holding a transtympanic electrode steady for hours. We then investigated the short-term effect of a limited set of electric stimulation parameters on tinnitus in 10 adults with chronic tinnitus. T

Examination of a mechanical amplifier in the incudostapedial joint gap: FEM simulation and physical model.

Implantable assembly components that are biocompatible and highly miniaturized are an important objective for hearing aid development. We introduce a mechanical transducer, which could be suitable as part of a prospective fully-implantable hearing aid. The transducer comprises a sensor and an actuator unit in one housing, located in the joint gap between the middle ear ossicles, the incus and stapes. The setup offers the advantage of a minimally invasive and reversible surgical solution. However, feedback between actuator and sensor due to mechanical coupling limits the available stable gain.

Successful airway management with use of a laryngeal mask airway in a patient with CHARGE syndrome.

CHARGE syndrome refers to an autosomal dominant disorder in patients with coloboma (C) of the eye, heart disease (H), atresia of choanae (A), retarded growth (R), genital hypoplasia (G), and ear anomalies (E). In addition to these typical features, airway abnormalities, including retrognathia, glossoptosis, and laryngeal paralysis, have been reported in patients with this syndrome. This report describes a case of CHARGE syndrome observed in a 6-year-old male patient with a difficult airway, in whom anesthesia was managed successfully using a laryngeal mask airway.

Surgical robotics in otolaryngology: expanding the technology envelope.

PURPOSE OF REVIEW: Surgical robotics arose as an extension of virtual reality and robotic technology developed by the United States Department of Defense. Current surgical robotic systems have been used to perform a variety of minimally invasive surgical procedures. RECENT FINDINGS: The Food and Drug Administration recently granted approval for the clinical use of two surgical robotic systems. Laboratory and clinical experience suggests that the use of surgical robotics is associated with some distinct advantages and disadvantages when compared with conventional open procedures.
